Chronic kidney disease among high school students of Kinshasa

Abstract: In an epidemiological cross sectional study, a random sample of 524 pupils (263 boys, mean age of 18.7 ± 1.4 years) from school environment of Kinshasa were studied. Recorded parameters of interest were anthropometric, proteinuria, serum creatinine and estimated glomerular filtration rate (eGFR) according to the Schwartz formula using uncalibrated creatinine levels from one random measurement. CKD was defined as the presence of kidney damage (daily proteinuria ≥ 300 mg) and/or reduced kidney function (eGFR < 60 ml/min/1.73 m2). Concordances between eGFR according to Schwartz, Cockcroft-Gault (C-G) indexed for BSA and modification of diet in renal disease (MDRD) study equations were computed using the kappa coefficient.The prevalence of CKD by the Schwartz formula was 1.5%. By stage, 0.8% had CKD stage 1 (proteinuria with normal eGFR) and 0.8% had CKD stage 3 (eGFR, 30 to 59 ml/min/1.73 m2). The prevalence of proteinuria ≥ 300 mg/day was 1% (one case had 2.7g/day). Agreement between eGFR according to Schwartz formula and the MDRD formula was excellent (kappa: 88.8%). Although correlations between all formulas were excellent (0.99; 0.87, and 0.89), agreement was poor between eGFR according to Schwartz and C-G indexed BSA equation (kappa: 52.7%) and, poorer with C-G unadjusted for BSA (kappa: 26.9%).In the large African city of Kinshasa, 2% of high school students have CKD. This high prevalence rate emphasizes the need for appropriate detection and prevention measures in this vulnerable young age population group.
Financial Risk Protection and Universal Health Coverage: Evidence and Measurement Challenges

Abstract: Financial risk protection is a key component of universal health coverage (UHC), which is defined as access to all needed quality health services without financial hardship. As part of the PLOS Medicine Collection on measurement of UHC, the aim of this paper is to examine and to compare and contrast existing measures of financial risk protection. The paper presents the rationale behind the methodologies for measuring financial risk protection and how this relates to UHC as well as some empirical examples of the types of measures. Additionally, the specific challenges related to monitoring inequalities in financial risk protection are discussed. The paper then goes on to examine and document the practical challenges associated with measurement of financial risk protection. This paper summarizes current thinking on the area of financial risk protection, provides novel insights, and suggests future developments that could be valuable in the context of monitoring progress towards UHC.

Abstract: Background: Although systemic hypertension is rarely seen in steady state sickle cell disease (SCD), relative hypertension has been reported to be associated with an increased risk of cardiovascular and renal complications. Objective: To determine the prevalence of BP patterns and assess factors associated with relative hypertension insickle cell anemia (SCA) adult patients. Methods: Clinical data and office BP were obtained from 103 consecutive steady-state SCA adult patients (mean age 26 ± 7.9 years, 66% females, 22.3% on hydroxyurea) attending four healthcare centers providing SCD-specific care in Kinshasa. Seated BP was measured using an automated electronic device. Three consecutive blood pressure measurements were taken with 2 minutes interval between readings and the average of the 2 last readings was considered for the analyses. Normal BP, relative hypertension and systemic hypertension were defined as BP < 120/70 mmHg, 120 - 139/70 - 89 mmHg and ≥140/90 mmHg, respectively. Results: Normal BP, relative hypertension and systemic hypertension were observed in 56 (54%), 43(42%) and 4 (4%) of SCA patients, respectively. Abstract: La recherche dans le domaine de l’adaptation a toujours d faire face à un obstacle considérable : la variation sémiotique. S’il existe bien une tradition qui considère le(s) film(s) en tant que texte(s), les instruments spécifiques de l’analyse du discours sont rarement utilisés dans la pratique. Cela pourrait être applicable en faisant appel à des modèles multimodaux. Un modèle systémique-fonctionnel, se fondant sur la comparaison des significations, est donc proposé ici puis testé sur Les Liaisons dangereuses, le roman et le film. Adaptation studies as a field of research has had to contend with the considerable obstacle of semiotic variation. Although a tradition of viewing film(s) as text(s) does exist, the instruments of discourse analysis have seldom been applied to concrete examples. This perspective could be embraced by resorting to multimodal models. A systemic-functional model, based on the contrast between meanings, is proposed here and subsequently tested on Dangerous Liaisons, the novel and the film. La investigación en el área de la adaptación debió enfrentarse siempre a un obstáculo mayor : la variación semiótica. Si es cierto que existe una tradición que considera a la(s) película(s) como texto(s), las herramientas particulares para analizar el discurso son raramente utilizadas en la práctica. Esto podría llevarse a cabo apelando a modelos pluri-modales. Un modelo sistémico-funcional que se basa en la comparación de los significados será planteado aquí para luego ser puesto a prueba en Les Liaisons dangereuses (Las relaciones peligrosas), tanto en la novela como en la película.

Abstract: La division des deux Corées est souvent considérée comme le dernier vestige de la guerre froide, vingt ans après l'effondrement de l'URSS et la fin de sa rivalité avec les états-Unis. Si l'antagonisme entre les deux blocs a indéniablement contribué à l'enracinement et au maintien de la division, cette dernière a aussi été structurée par des forces et des logiques qui, sans être étrangères à la guerre froide, la dépassaient et continuent aujourd'hui de s'exercer. Comprendre la frontière coréenne revient donc à s'aventurer par-delà la guerre froide, non seulement sur le plan chronologique mais également du point de vue de l'analyse afin de restituer le r le des acteurs de premier plan que sont les deux Corées elles-mêmes et les autres puissances d'un ordre régional désormais multipolaire.(The division of the two Koreas is often seen as the last vestige of the Cold War twenty years after the collapse of the former Soviet Union and its rivalry with the United States. Abstract: Some recent Babar results on semileptonic decays are presented. They focus on the determination of the CKM matrix elements |V_{ub}| and |V_{cb}| in inclusive and exclusive b --> u l nu and b--> c l nu decays, and on form factors measurement in exclusive c--> s l nu decays.
Search for the rare decays Bs,d->mumu at LHCb

Abstract: A search for the Bs->mumu and Bd->mumu decays is presented using ~300 pb-1 of pp collisions at sqrt{s} = 7 TeV collected by the LHCb experiment at the Large Hadron Collider at CERN. The measured upper limit for the branching ratio of the Bs->mumu decay is BR(Bsmumu) < 1.3 (1.6) x 10^{-8} at 90% (95%) confidence level (CL), while in the case of the Bd->mumu decay the measured upper limit is BR(Bdmumu) < 4.2 (5.1) x 10^{-9} at 90% (95%) CL. A combination with the 2010 dataset results in BR(Bsmumu) < 1.2 (1.5) x 10^{-8} at 90% (95%) CL.
Charm semileptonic decays at the B factories

Abstract: A review of charm semileptonic decays results obtained at B factories is presented. It focuses on form factors measurements in $D^0\to K^- \ell^+ \nu$, $D^0\to \pi^- \ell^+ \nu$, $D_s^+ \to K^+K^- e^+\nu_e$, and $D^+ \to K^-\pi^+ e^+\nu_e$. For the last two decay channels, the contribution and characteristics of other components in the final state, in addition to the main vector contribution, is also studied.
