Effectiveness of Endovascular Coil Embolization of Ruptured Hepatic Artery Pseudoaneurysm  [PDF]

Abstract: Purpose: To assess the efficacy of endovascular management for ruptured hepatic artery pseudoaneurysm (HAP). Methods: Six cases of HAP in five patients (four men and one women; mean age, 50; range, 28 - 62) were treated with transcatheter arterial coil embolization using microcoil (Boston Scientific, Watertown, MA, USA) and Tornado Coil (Cook, Bloomington, IN, USA) with or without stent graft between January 2007 and September 2008. They were analyzed with regard to the clinical presentation, radiological findings with procedure, and clinical and radiologic outcomes. Results: All patients presented with epigastric pain or gastrointestinal bleeding. The pseudoaneurysms were ranged from 0.6 to 4.4 cm in size and located in the common hepatic artery (n = 1), junction between proper and right hepatic artery (n = 1), proper hepatic artery (n = 2), the left hepatic artery (n = 1), right hepatic artery (n = 1). Embolization was performed with microcoils in all pseudoaneurysmal sac with or without both afferent and efferent segment. A self-expandable stent (n = 1) was also used. Overall technical success was 100% (6 of 6) and complete occlusions of HAPs were achieved in 5 out of 6 cases, 83.3% of clinical success rate. Re-bleeding occurred in one case of stent graft at proper hepatic artery following coil packing for pseudoaneurysmal sac. Clinical success rate of embolization for both afferent and efferent segment was 100% (3 of 3). Procedure-related minor complications happened in 2 of 5 patients, and they were treated conservatively. Conclusion: Transcatheter coil embolization for ruptured HAP is sufficiently effective and additional embolization in both afferent segment and efferent segment can improve clinical success rate.

Abstract:
Epiploic appendages can become ischemic and infarcted, which can present acute abdominal pain that mimics other diseases, such as diverticulitis or appendicitis. Particularly, inflammation of an epiploic appendage attached to the vermiform appendix is a very rare cause of acute abdominal pain, and it is difficult to diagnose this condition preoperatively. We present a case of epiploic appendagitis of the vermiform appendix that was identified with a typical radiologic image and was pathologically confirmed. In addition, we review the literature of similar cases and analyze the clinical and radiologic features of EA of the vermiform appendix.

Abstract:
Neuroendocrine tumor (NET) arising from common bile duct (CBD) is a rare disease entity, whose origin is currently in discussion with various theories. We present a case of well-defined solid, exophytic and arterial hypervascular mass arising from CBD with relatively mild dilated central intrahepatic bile ducts in 50-year-old man, which was surgically confirmed to be extrahepatic biliary NET, and reviewed radiologic features of extrahepatic biliary NET in previous literature.

Abstract: We report here a case of an inflammatory myofibroblastic tumor in the retroperitoneum, which mimicked a germ cell tumor of the undescended testis. A 75-year-old healthy man presented with a palpable abdominal mass. On the computed tomography image, there was large, well-defined soft tissue mass in the left side of the retroperitoneum, and there was no visible left testis or seminal vesicle. After contrast enhancement, the mass appeared to be relatively homogeneous, considering its large size. With ultrasonography, it appeared as a well-defined, hypoechoic mass with intratumoral vascularity. This solid mass was surgically diagnosed as an inflammatory myofibroblastic tumor.

Abstract: Medical facilities in retirement residential areas provide diverse medical and health-related services to retirees, and they are equipped and programed with various medical services for enhancing retirees’ daily activities. To find out the current status of using medical facilities by retirees, this study surveyed retirees in their fifties, sixties, and seventies, who live in Eunpyeong-gu, Seoul. The survey data was sorted by the types of medical facilities they visit, reasons for their visit, and the types of medical facilities in a retirement residential area they would prefer to visit. The pros and cons of using private and general hospitals can be evaluated further to set up the right strategy to reconcile them; and each type of hospital can be improved by adding those missing elements that the interviewees mentioned. In the broad perspective, this indicates the need to suggest the direction of preparing residential areas that are technically planned for retirees on the basis of the analysis of Korean retirees’ needs.
Improving Urban Retirees’ Well-Being by Leisure Activities  [PDF]

Abstract: This research is to investigate urban retirees’ needs and suggest the design direction of leisure facilities. Unlike the existing approach based on deterioration of physical ability, this will allow a new type of suggestion to enable the retirees to live their second lives in a more creative way. By analyzing their common memories and events, it was found that they developed their desire for leisure from their experience of wealth and economic growths. Some preceding research pointed out that their actual leisure activities tend to be static and passive in general. Through the interview, however, this research has found that there are 45 indoor activities and 75 outdoor leisure activities, while their potential demands for leisure further extends to 88. With outdoor activities outnumbering indoor activities, the result shows the opposite tendency to the preceding research. On the other hand, their potential demands for leisure further extended to include more intellectually stimulating learning activities.

Abstract: ?根据中国和韩国3条树木年轮降水记录,本文分析了过去160a以来东亚夏季风降水的变迁历史.统计分析表明,年际间中国和韩国季风降水不存在线性相关,而在10a尺度上则表现出同步变化的趋势.1860~1890,1910~1925,1940~1960年是3个降水量较多的时段;1890~1910,1925~1940,1960~1998年,则是3个降水量较少的时段.3条降水重建序列均揭示1920年代中期东亚夏季风降水快速由多转少.同时树木年轮季风降水重建序列也被基于中国历史文献记载所划分的旱涝指数和韩国雨量记录所证实.

Abstract: If we have gone through the first generation of housing design that pursued functional optimization, ergonomics, and circulation efficiency during the last century, now we are living in the second generation where more advanced goals, such as universal design, ubiquitous design, sustainable design, and environment-friendly design, are emphasized. Although this second generation of design focuses upon the wellness of humans in accordance with environment, it still has the attitude that a more precisely designed home can guarantee a better life. What lacks in this approach is the freedom of the body; it needs to make its own choice as to how to use a space. Thus, it is suggested in this paper that what is important in designing a home is to provide alternatives in daily lives so as to make a full exploration of a given space. These alternatives can be made by offering residents an interpretable space where they can figure out space usages and routs in a constantly changing context. Two spatial devices are discussed in depths as a way to realize this interpretable house: room-to-room enfilade and ring spatial structure. By investigating some existing house plans, it is illustrated how they can guarantee the freedom of the body, and thus alternatives for the flexible domestic life.

Abstract: The light is an important element which helps people perceive objects. Therefore, it is important for architects to make the light and space be in harmony with each other. In this study, we analyzed the works of Alvaro Siza with a view to understand the conceptual value of the light expressed in his works and his principles in controlling it. According to the results of the study, the Siza’s architecture is not a mere theoretical one trapped inside formality, but is a sensual and experiential one based on the locality. He was willing to use void spaces to invite the light in free-flowing plans, in order to invigorate and extend architectural spatiality to create deeper visual effect. In addition, the refined light in his works helped visitors experience the continuous forms and spaces by their own movements, while using the changes of the light to stimulate the interest of visitors and highlight the sequence of spaces.
The gene-reduction effect of chromosomal losses detected in gastric cancers

Abstract: The pathologic specimens obtained from 145 gastric cancer patients were examined for the level of LOH using 40 microsatellite markers on eight cancer-associated chromosomes (3p, 4p, 5q, 8p, 9p, 13q, 17p and 18q).Most of the cancer-associated chromosomes were found to belong to the gene-poor chromosomes and to contain a few stomach-specific genes that were highly expressed. A baseline-level LOH involving one or no chromosome was frequent in diffuse-type gastric cancers. The chromosome 17 containing a relatively high density of genes was commonly lost in intestinal-type cancers but not in diffuse-type cancers. A high-level LOH involving four or more chromosomes tended to be frequent in the gastric cancers with intestinal and mixed differentiation. Disease relapse was common for gastric cancers with high-level LOH through both the hematogenous (38%) and non-hematogenous (36%) routes, and for the baseline-level LOH cases through the non-hematogenous route (67%).The cell-adverse effect of gene reduction is more tolerated in intestinal-type gastric cancers than in diffuse-type cancers, and the loss of high-dose genes is associated with hematogenous metastasis.Gastric cancers undergo hypermutations in simple repeat sequences and imbalanced chromosomal losses, and these genetic alterations are detected as a microsatellite instability (MSI) and loss of heterozygosity (LOH), respectively, by using highly polymorphic microsatellite markers [1-4]. The pathobiological behavior and prognosis of gastric cancers have been associated with the level of LOH and the presence of MSI [3-6]. The depth of invasion and lymph node metastasis are considerably advanced in the cases of small-sized cancers with high-level LOH, and these histologic parameters tends to be linearly related with the cancer size for the low-level LOH cases [7].
