Abstract: O presente estudo teve como objetivo conhecer a experiência dos profissionais da Psicologia dentro do atual modelo de aten o em saúde mental. Foi realizado um estudo qualitativo, com a utiliza o da técnica de entrevistas semiestruturadas, tendo como participantes sete psicólogos de três Centros de Aten o Psicossocial de uma cidade de porte médio do Rio Grande do Sul. Os resultados foram analisados através da análise de conteúdo, e revelaram o impacto inicial vivenciado pelos profissionais, manifestado através dos sentimentos de medo, frustra o e impotência. Os participantes evidenciaram a importancia de um constante repensar da a o profissional, destacando a realiza o de capacita o quando da entrada do profissional no servi o e da supervis o clínico-institucional como recurso para qualificar a prática, e ressaltaram a importancia de um trabalho em equipe e do olhar multiprofissional em rela o ao usuário. Pode-se concluir que os profissionais da Psicologia têm um papel preponderante nas equipes que comp em os CAPS. El presente estudio tuvo como objetivo conocer la experiencia de los profesionales de la Psicología dentro del actual modelo de atención en salud mental. Fue realizado un estudio cualitativo, con la utilización de la técnica de entrevistas semiestructuradas, teniendo como participantes siete psicólogos de tres Centros de Atención Psicosocial de una ciudad de mediano porte de Rio Grande do Sul. Los resultados fueron analizados a través del análisis de contenido, y revelaron el impacto inicial vivido por los profesionales, manifestado a través de los sentimientos de miedo, frustración e imposibilidad. Los participantes evidenciaron la importancia de un constante repensar de la acción profesional, destacando la realización de capacitaciones en el momento de la entrada del profesional en el servicio y de la supervisión clínicoinstitucional como recurso para calificar la práctica, y resaltaron la importancia de un trabajo en equipo y de la visión multiprofesional con relación al usuario. Se puede concluir que los profesionales de la Psicología tienen un papel preponderante en los equipos que componen los CAPS. This study had as aim to know the experience of psychology professionals considering the current mental health care model. A qualitative study was carried out, using semi-structured interviews. The participants were seven psychologists of three Psychosocial Assistance Centers of a medium-sized city of Rio Grande do Sul. Abstract: Diagnostics and rehabilitation of Parkinson’s disease (PD) presents the current information pertaining to etiology, early biomarkers for diagnostics, novel methods to evaluate symptoms, multidisciplinary rehabilitation, new applications of brain imaging and invasive methods to the study of PD. Researchers have only recently begun to focus on the non-motor symptoms of PD, which are poorly recognized and inadequately treated by clinicians. The non-motor symptoms of PD have a significant impact on patient quality of life and mortality, and include cognitive impairments, autonomic, gastrointestinal, and sensory symptoms. Indepth discussion of the use of imaging tools to study disease mechanisms is also provided, with emphasis on the abnormal network organization in parkinsonism. Deep brain stimulation management is a paradigm-shifting therapy for PD, essential tremor and dystonia. In the recent years, new approaches of early diagnostics, training programmes and treatments have vastly improved the lives of people with PD, substantially reducing symptoms and significantly delaying disability. PD results primarily from the death of dopaminergic neurons in the substantia nigra. Current PD medications treat symptoms; none halt or retard dopaminergic neuron degeneration. The main obstacle to developing neuroprotective therapies is a limited understanding of the key molecular mechanisms that provoke neurodegeneration. The discovery of PD genes has led to the hypothesis that misfolding of proteins and dysfunction of the ubiquitin-proteasome pathway are pivotal to PD pathogenesis. Previously implicated culprits in PD neurodegeneration, mitochondrial dysfunction and oxidative stress, may also act in part by causing the accumulation of misfolded proteins, in addition to producing other deleterious events in dopaminergic neurons. Neurotoxin-based models have been important in elucidating the molecular cas-cade of cell death in dopaminergic neurons. PD models based on the manipulation of PD genes should prove valuable in elucidating important aspects of the disease, such as selective vulnerability of substantia nigra dopaminergic neurons to the degenerative process.
Biopsia intestinal asistida por laparoscopia como método diagnóstico en una paciente con enteritis eosinofílica

Abstract: the aim of this study is to present a laparoscopic technique for small bowel biopsy which permitted an specific diagnosis (eosinophilic enteritis) in a female patient with abdominal pain of uncertain etiology. a description of a single case about a 23 years old famale patient with clinical feature of abdominal pain and inconclusive radiographics findings is done. under general anesthesia a diagnostic laparoscopy and a full-thickness yeyunal biopsy by an asisted technique was practiced, with extracorporeal enterorraphy at two layers. we concluded that laparoscopy could be a valuable diagnostic method for inflammatory bowel diseases because specific biopsy can be done with technical simplicity and minimal morbidity.
Apendicectomía por laparoscopia en la apendicitis aguda complicada

Abstract: Se presenta un análisis de nuestra experiencia en la apendicectomía laparoscópica para la apendicitis aguda complicada. Desde junio 1995 hasta marzo 2001, practicamos 129 apendicectomías laparoscópicas por apendicitis aguda. Se incluyen en el estudio 29 pacientes, 12 mujeres y 17 hombres, en quienes el procedimiento se realizó en una etapa avanzada de la enfermedad (gangrena, perforación y/o presencia de material purulento en la cavidad abdominal). Se administró antibióticoterapia combinada (cefoperazona-sulbactam o sultamicina, más metronidazol) y se drenó la cavidad abdominal en los casos con peritonitis purulenta. Se practicaron 28 apendicectomías laparoscópicas ya que un caso (3,4 %) se convirtió a cirugía abierta. La relación hombres/mujeres fue de 1,4:1. El tiempo quirúrgico promedio fue de 60 minutos (45 a 120 min). No hubo complicaciones intraoperatorias. El promedio de hospitalización fue de 2,5 días (1 a 4 días). Se presentó una complicación infecciosa menor (3,4 %) y no hubo mortalidad en la serie. Nuestros resultados muestran que la apendicectomía laparoscópica es un procedimiento factible, efectivo y seguro en pacientes con apendicitis aguda complicada. Su implementación permite reducir la frecuencia y severidad de complicaciones infecciosas así como también el tiempo de hospitalización de estos pacientes. SUMMARY An analysis of our experience in laparoscopic appendectomy for complicated acute appendicitis is presented. From June 1995 until March 2001, 129 laparoscopic appendectomies for acute appendicitis were performed. 29 patients, 12 female and 17 male, in whom the procedure was performed in an advanced stage of the illness (gangrene, perforation, and/or the presence of purulent material in the abdominal cavity) were included in this study. A combination of antibiotics was given (Cefoperazone-Sulbactum or Sultamicine, plus Metrodinazole) as well as drainage of the abdominal cavity in cases of purulent peritonitis. 28 laparoscopic appendectomies were performed. One case (3.4 %) was converted to open surgery. The male/female ratio was 1.4:1. The average operative time was 60 minutes (45 to 120 minutes). No intraoperative complications ocurred. The average hospitalization time was 2.5 days (1 to 4 days). One minor infectious complication appeared (3.4 %). No deaths ocurred in the series. Our results show that laparoscopic appendectomy is a feasible, effective and secure procedure in patients with complicated acute appendicitis. Abstract: El objetivo del siguiente trabajo es presentar una técnica laparoscópica para la biopsia de intestino delgado, la cual permitió realizar un diagnóstico específico (enteritis eosinofílica) en una paciente con dolor abdominal de etiología incierta. Se describe el caso de una paciente de 23 a os con cuadro clínico de dolor abdominal y estudios radiológicos no concluyentes para el diagnóstico. Bajo anestesia general se realiza laparoscopia diagnóstica y biopsia yeyunal de espesor total mediante una técnica asistida, con enterorrafia extracorpórea en dos planos. Se concluye que la laparoscopia puede ser un método valioso en el diagnóstico de los pacientes con patologías intestinales inflamatorias, ya que permite realizar biopsias dirigidas mediante técnicas sencillas con mínima morbilidad. Cura operatoria por laparoscopia de hernia paraesofágica gigante

Abstract: Las hernias paraesofágicas representan el 5 % de los defectos del hiato esofágico y la reparación quirúrgica de las mismas constituye el único tratamiento definitivo disponible. En el presente trabajo se describe una técnica laparoscópica para la corrección de la hernia paraesofágica gigante utilizada en dos pacientes con buenos resultados. Se discuten los tipos de reparación descritos en la literatura haciendo énfasis en los aspectos técnicos controversiales que presenta esta cirugía. Paraesophageal hernias accounts 5 % of all esophageal hiatus defects and it’s surgical repair is the only definitive treatment available. In the present paper a laparoscopic technique for the correction of giant paraesophagical hernia used in two patients with good results is described. We also comment others surgical options doing emphasis in the controversial technical aspects that this kind of surgery have.
Esplenectomía Laparoscópica a propósito de 2 casos

Abstract: RESUMEN. La esplenectomía laparoscópica constituye el método quirúrgico de elección cuando la extirpación del bazo está indicada por patologías hematológicas benignas, aunque no se excluyen las enfermedades malignas, y condiciones como hipertensión portal y esplenomegalia. Se presentan dos casos intervenidos en el Hospital Universitario de Caracas, describiendo la técnica utilizada; y se realiza una revisión de la literatura internacional, comparando distintas variables tales como pérdida sanguínea, tiempo de hospitalización, tiempo de regreso a actividades cotidianas, analgésicos post-operatorio, tiempo operatorio y complicaciones; dichos aspectos son relacionados con aquellos obtenidos en nuestra experiencia. Se concluye que la esplenectomía laparoscópica ofrece mejores resultados estéticos, menor tiempo de hospitalización, menor dolor post-operatorio y reinserción laboral temprana. ABSTRACT: Laparoscopic splenectomy is the surgical procedure of election when the extirpation of the spleen is indicated due to benign hematological pathologies, eventhough malignant diseases are not excluded, as well as conditions like portal hypertension and splenomegaly. We present two cases that have been operated at the Hospital Universitario de Caracas, describing the technique used; and reviewing the world literature and comparing different variables, such as blood loss, time of hospitalization, time in returning to daily routines, post-surgical analgesics, surgical timing, and complications; these aspects are compared to those of our experience. We conclude that laparoscopic splenectomy offers better esthetic results, less time of hospitalization, less post-surgical pain, and earlier return to work.
Colangiografía Intraoperatoria por Laparascópia con Cateter de Cholangiocath y de Teflón

Abstract: En el siguiente trabajo se describe una técnica para la realización de colangiografía intraoperatoria en el curso de colecistectomía laparoscópica por un mismo cirujano en 20 pacientes consecutivos. En la era de la colecistectomía laparoscópica, la colangiografía intraoperatoria constituye un método efectivo y seguro para diagnosticar la litiasis coledociana concomitante, así como también aclarar dudas anatómicas; esto permite tratar al paciente con un procedimiento único mediante las técnicas laparoscópicas de exploración de la vía biliar extrahepática y tomar las previsiones necesarias para evitar las temidas iatrogenias de las vías biliares. La técnica descrita en este trabajo resultó en un 100% de colangiografías completas, representa un método sencillo que permite el diagnóstico de patología en el conducto biliar principal y también facilita la adquisición de habilidades al cirujano que se entrena en técnicas laparoscópicas. The following article describes a technique performed by the same surgeon in 20 patients, for intraoperative cholangiography during the course of a laparoscopic cholecystectomy. In the era of laparoscopic cholecystectomy, the intraoperative cholangiography is an effective method to diagnose lithiasis of the common bile duct as well as to explain any anatomic doubts. This enables us to treat the patient with only one procedure by means of the laparoscopic techniques of exploration of the extrahepatic bile duct tract and take the necessary caution to avoid any possible iatrogenic injury that may be caused. The technique described here, resulted in 100% of complete cholangiographies and represents a simple method that not only allows the diagnosis of the bile duct pathology, but also helps the surgeon who is training in laparoscopic techniques, acquire the necessary abilities.
Density functional study of oxygen vacancies at the SnO2 surface and subsurface sites

Abstract: Oxygen vacancies at the SnO2(110) and (101) surface and subsurface sites have been studied in the framework of density functional theory by using both all-electron Gaussian and pseudopotential plane-wave methods. The all-electron calculations have been performed using the B3LYP exchange-correlation functional with accurate estimations of energy gaps and density of states. We show that bulk oxygen vacancies are responsible for the appearance of a fully occupied flat energy level lying at about 1 eV above the top valence band, and an empty level resonant with the conduction band. Surface oxygen vacancies strongly modify the surface band structures with the appearance of intragap states covering most of the forbidden energy window, or only a small part of it, depending on the vacancy depth from the surface. Oxygen vacancies can account for electron affinity variations with respect to the stoichiometric surfaces as well. A significant support to the present results is found by comparing them to the available experimental data.
Spin Channels in Functionalized Graphene Nanoribbons

Abstract: We characterize the transport properties of functionalized graphene nanoribbons using extensive first-principles calculations based on density functional theory (DFT) that encompass both monovalent and divalent ligands, hydrogenated defects and vacancies. We find that the edge metallic states are preserved under a variety of chemical environments, while bulk conducting channels can be easily destroyed by either hydrogenation or ion or electron beams, resulting in devices that can exhibit spin conductance polarization close to unity.
