Incremento de la actividad timolítica del clobetasol en forma liposomal

Abstract: the comparative evaluation between the thymolytic action of clobetasol jelly in liposomal form and a free clobetasol jelly of a conventional formulation was made by using adrenolectomized rats. the 6-point method was applied to determine its biological potency. it was demonstrated that the liposomal form of clobetasol jelly has an action 2.35 times higher than the formulations of free clobetasol jelly. this allows to reduce the dosage of clobetasol from the liposomal formulation.

Abstract: Se realizó la evaluación comparativa de la acción timolítica de una jalea de clobetasol en forma liposomal en relación con una jalea de clobetasol libre de una formulación convencional, para lo cual se emplearon ratas adrenolectomizadas. Se aplicó el método de 6 puntos para la determinación de su potencia biológica y se pudo demostrar que la forma liposomal de la jalea de clobetasol tiene una acción 2,35 veces superior a las formulaciones de jalea de clobetasol libre. Esto permite disminuir la dosificación del clobetasol de la formulación liposomal. Abordaje multidisciplinario de la infección por Clostridium difficile Multidisciplinary approach of Clostridium difficile infection

Abstract: Clostridium difficile es la causa más común de diarrea infecciosa en instituciones sanitarias de adultos. Estudios recientes han mostrado un incremento en la incidencia, gravedad y recurrencia de la infección por C. difficile (ICD).Factores asociados al paciente y a la atención sanitaria contribuyen a establecer la colonización y en algunos casos la posterior progresión a enfermedad sintomática. La disponibilidad de nuevas técnicas microbiológicas ha contribuido en gran medida a mejorar el manejo de estos pacientes. Se muestra un algoritmo diagnóstico ante la sospecha de ICD basándose en la evidencia actual sobre la rentabilidad de métodos microbiológicos y radiológicos. Ante la confirmación clínica de ICD la primera medida y la más eficaz es la retirada del tratamiento antimicrobiano que tenga el paciente, si es posible. El tratamiento antimicrobiano de la ICD se basa en tres agentes clásicos, metronidazol, vancomicina y teicoplanina, y uno de reciente incorporación, fidaxomicina. En los cuadros graves se deberán instaurar medidas de soporte y monitorización adecuadas y pueden ser subsidiarios de tratamiento quirúrgico. Las estrategias de prevención y control de la infección permiten interrumpir el mecanismo de transmisión. Este manuscrito revisa la evidencia actualsobre el abordaje de esta entidad desde un punto de vista multidisciplinario. Clostridium difficile is the most common cause of infectious diarrhea in adults healthcare institutions. Recent studies have shown an increase in the incidence, severity and recurrence of C. difficile infection (CDI). Factors associated with the patient and medical care provided contribute to establishing colonization and, in some cases, subsequent progression to symptomatic disease. The availability of new microbiological techniques has contributed greatly to improving care for these patients. A diagnostic algorithm is provided for cases in which CDI is suspected based on current evidence regarding the effectiveness of microbiological and radiological methods. In cases in which CDI is confirmed, the first and most effective measure is the withdrawal of any antibiotic treatment the patient is receiving, if possible. The antimicrobial treatment of CDI is based on three classic agents: metronidazole, vancomycin and teicoplanin, along with the recent addition of fidaxomicin. Patients presenting serious symptoms, in addition to appropriate support and monitoring measures, may require surgical treatment. Abstract: case report: a 62-year-old man with previous renal transplant and immunosuppressive treatment presented with decreased visual acuity (20/100) in his left eye, corneal oedema and intraocular pressure of 46mmhg. one month later an inferior marginal dendritic keratitis appeared. corneal scraping and real-time polymerase chain reaction showed herpes simplex virus (hsv). discussion: the autoimmune corneal endotheliopathy or acute idiopathic corneal endotheliitis is characterised by corneal stromal oedema and keratic precipitates. hsv might be secreted from the trabeculum, innervated by the trigeminal nerve. this hypothesis is supported by the clinical observation that the corneal stromal oedema usually starts from the periphery.

Abstract: Caso clínico: Varón de 62 a os de edad con trasplante renal y tratamiento inmunosupresor que acude por disminución de la visión (20/100) en ojo izquierdo, edema corneal y presión intraocular de 46mmHg. Un mes después aparece queratitis dendrítica marginal inferior. El raspado corneal y la proteína C reactiva demuestran la presencia de un virus del herpes simple (HVS). Discusión: La epiteliopatía corneal autoinmunitaria o endotelitis idiopática corneal se caracteriza por edema corneal y precipitados queráticos. Se cree que el HVS podría ser secretado desde el trabeculum, inervado por el nervio trigémino, apoyado clínicamente por la progresión del edema estromal desde la periferia. Abstract: introduction: cryotherapy is an alternative to superficial thermotherapy based on the application of cold as a therapeutic agent, although it is quite clear it is widely used for pain relief. however, only relies on indirect mechanisms of action and without a scientific basis to support its clinical application. based on this an analysis will be made of the evidence regarding the effectiveness of cryotherapy by means of a systematic review of randomized controlled trials. objective: to determine if there is scientific evidence to endorse the analgesic effect of cryotherapy for the management of musculoskeletal pain. strategy of search: the words/phrases included in the search were, randomized controlled trials (rcts), systematic reviews (sr) and metaanalysis (mt), the databases used were: medline/pubmed, pubmed central, sciencedirect, biomed central, cochrane library plus and dare. results: we selected a total of eight studies, including randomized controlled trials and systematic reviews. conclusions: there is moderate evidence supporting that the use of cryotherapy reduces pain and improves recovery times in ankle sprain and soft tissue injuries associated with sport, but there is limited evidence for its use in knee osteoarthritis, rheumatoid arthritis and low back pain syndrome.

Abstract: Introducción: La Crioterapia es una modalidad de Termoterapia Superficial que se basa en la aplicación del frío como agente terapéutico, si bien es cierto, es ampliamente utilizada para el alivio del dolor, su indicación clinica tradicionalmente se ha sustentado en mecanismos de acción indirectos, sin una base científica o estudios clínicos que avalen su efectividad, se realizará una síntesis de la evidencia relativa a la efectividad de la crioterapia a través de una revisión sistemática de ensayos clínicos aleatorizados. Objetivo: Determinar si existe evidencia científica que avale el efecto analgésico de la crioterapia para el manejo del dolor de origen musculoesquelético. Estrategia de búsqueda: Se incluyeron en la búsqueda Ensayos Clínicos Aleatorizados (ECA), Revisiones Sistemáticas (RS) y Metaanálisis (MT), las bases de datos usadas fueron: MEDLINE, ScienceDirect, Biomed Central, Cochrane y DARE. Resultados: Se seleccionaron un total de ocho estudios, incluyendo ensayos clínicos aleatorizados y revisiones sistemáticas. Conclusiones: Existe moderada evidencia que el uso de la crioterapia disminuye el dolor y mejora los tiempos de recuperación en esguince de tobillo y en lesiones de tejidos blandos asociados al deporte, la evidencia es limitada para el uso en Osteoartritis de rodilla, Artritis Reumatoide y Síndrome de Dolor Lumbar. Abstract: El territorio mexicano se caracteriza por una amplia diversidad ambiental debido a diversos factores fisiográficos (ubicación geográfica, relieve, clima, tipo de suelo, vegetación). En México, los procesos de cambio en el uso del suelo se derivan de la interacción de amplios factores geográficos, económicos, políticos, sociales, demográficos y culturales. El objetivo de este trabajo fue identificar y evaluar los cambios en el uso del suelo ocurridos en la microcuenca del río San José, ubicado en los municipios de San Felipe del Progreso y San José del Rincón, en el Altiplano Mexicano, entre los a os 2000 y 2008. Se utilizó el software Arc Gis y funciones matemáticas para calcular los cambios de uso de suelo y evaluar la pérdida de áreas boscosas próximas a zonas agrícolas. El cambio más significativo se presenta sobre áreas forestales transformadas en agricultura de temporal.

Abstract: the hemophagocytosis syndrome is a disorder of the phagocytic-mononuclear system characterized by a systemic and benign histiocytic proliferation with hemophagocytosis phenomenon. this is a case diagnosed with a hemophagocytic lymphohistiocytosis in a newborn aged 17 presenting with hepatosplenomegaly, pancytopenia and cytomegalovirus-positive serology. diagnosis is made; is treated with hemophagocytic lymphohistiocytosis protocol a total remission of patient.

Abstract: objetive: to evaluate the efficacy and security of polypropylene meshes in the repair of urogenital prolapse. material and method: retrospective and non-randomized study in 106 patients which had different kinds of urogenital prolapse repaired using polypropylene meshes between april 2005 and january 2007. the follow-up was carried out by two visits to the hospital, 2 and 6 months after surgery. the variables analyzed were age, parity, menopause presence, kind of surgical technique, surgical time, time at hospital and complications. afterwards, the information was analyzed descriptively. results: average age was 64.4 years. the rate of multiparity and menopause women was 91.51% and 92.45% respectively. the most used surgical technique was the anterior mesh with tension-free band (34.90%), to repair the associated urinary incontinence. the rate of intraoperatory complications was 2.83%, immediate complications was 37.73% and late complications was 21.69%. the success rate after 6 months was 80-100% depending on the technique. conclusions: there is a low rate of intraoperatory and 6 months after the repair in the reconstructive surgery of pelvic floor for the urogenital prolapses using polypropylene meshes, which makes this technique a secure and effective option for the treatment of this problem.
