Infarto encefálico por embolia aérea: Caso clínico

Abstract: ischemic stroke due to embolic air is uncommon. there are few reports of patients with air embolic stroke as a complication of endoscopic procedures. the temporal relationship between the stroke and this procedure is the most important clue for the diagnosis. ct scan and mri of the brain are confirmatory tests. the morbidity and mortality is high. patients should be hospitalized in a critical care service and treated as soon as possible with oxygen in a pressure camera. we report a 52 years old woman with an ovarian cancer that, during an upper gastrointestinal endoscopy, had a severe alteration of consciousness that did not respond to the use of flumazenil. a ct scan showed multiple areas of air embolism in the watershed area between anterior and middle right cerebral arteries. a conservative treatment was decided and the patients died 48 hours later

Abstract: background: thrombophilia is an alteration of hemostasis that increases the risk to venous or arterial thrombosis. this condition may be the underlying cause of retinal vein thrombosis. aim: to study the presence of thrombophilia in patients with retinal vein thrombosis. patients and methods: prospective study of 55 patients aged 22 to 86 years, with retinal vein thrombosis (central or branch). antithrombin iii, coagulant protein c, functional protein s, resistance to activated c protein, homocysteine, prothrombin g20210a gene, lupus anticoagulant and anticardiolipin antibodies were measured in all. results: seventeen patients had thrombophilic markers (antiphospholipid syndrome in seven, hyperhomocysteinemia in six and resistance to protein c in three). of these 17 patients, 53% had high blood pressure, 35% an abnormal serum lipid profile and 23% a personal history of thrombosis. the thrombosis was central in 12 (ischemic in four) and of a branch in five (ischemic in two). conclusions: thrombophilic markers must be assessed in patients with retinal vein thrombosis (rev méd chile 2005; 133: 167-74)

Abstract: background: surgical resection is the only treatment associated with long-term cure in patients with liver metastasis from colorectal cancer, achieving a 30% to 40% five years survival. aim: to evaluate the results of liver resection for metastatic colorectal cancer in our centre. patients and methods: retrospective study. epidemiological, perioperative and follow up data of patients undergoing liver resection for metastatic colorectal cancer between january 1990 and july 2007 were assessed. we compared the results between two periods; period 1 (1990-1997) and period 2 (1998-2007). results: sixty six patients aged 61± 12 years (46 males) underwent 75 resections. an anatomical excision was performed in 54 (72%) cases, a right hepatectomy in 18, an extended right hepatectomy in 11, a left hepatectomy in 1, and a segmentectomy in 24. in 24 (32%) patients the liver resection was simultaneous with the colorectal cancer resection. operative time was 221±86 min. hospital stay was 11±5 days. postoperative morbidity was 35% and surgical mortality was 0%. resection margin was free of tumor in 53 (80%) patients. five years overall and hepatic disease-free survival was 38% and 23%, respectively. in period 2, more anatomical resections than in period 1 were performed (77% and 55%, respectively, p =0.04), without an increase in complications (35% and 34%, respectively; p =ns), but with a better five years survival (45% and 21 %, respectively, p =0.04). conclusions: five years survival for excision of liver metastatic colorectal cancer in our center is similar to that reported abroad. during the second period there has been a trend toward more extensive resections which was associated with a better survival, without an increase in complications or mortality .

Los cefalópodos tienen dos estrategias de defensa principales: la primera consiste en reducir las probabilidades de ser detectados por un depredador, mientras que la segunda se centra en evitar la captura. El objetivo de este estudio fue conocer los aspectos básicos del comportamiento del pulpo Robsonella fontaniana frente al depredador potencial Schroederichthys chilensis (pintarroja). Los experimentos se realizaron bajo condiciones de laboratorio para determinar la eficacia del camuflaje para evitar al depredador. Los resultados muestran que la cripsis y un aumento del periodo de inactividad son utilizados por los pulpos como mecanismo de defensa contra los depredadores.

Abstract:


We expose a case of a woman with hypertension and hypokalemia. The differential diagnosis should include primary hyperaldosteronism, diuretics or lazantes intake, secondary hyperaldosteronism. In this patient, additional tests performed show no cause of hormonal disruption and the whole picture is due to a high intake of licorice. Glycyrrhetinic acid, the active component of licorice, inhibits renal IIbeta-hydroxisteroid dehydrogenase. This allows cortisol to stimulate mineralocorticoid receptors. Licorice ingestion should be considered in the differential diagnosis of hypertension with hypokalemia.
