Abstract: the spanish royal decree 183/2008 on specialised medical training in health sciences has meant a substantial change to this kind of training in spain. since its publication, the teaching units (tus) have started a process of adaptation to this decree, a process that is complex and that requires much effort in view of the many significant changes it introduces. the process is complicated even more because the competent institutions have failed to develop regulation on most of the decree's contents, leaving the tus with no guidelines. as an initiative of the education committee on specialised training of the hospital universitario virgen de la arrixaca we present the results of the participant's teamwork (key informers) in a workshop on the update of the regulation on educators in specialised health training that took place in murcia in february 2010. in this case we focus on the proposals developed for the head of residents in training and teachers, with the aim of providing support to the heads of studies and teachers in this adaptation.

New regulations regarding Postgraduate Medical Training in Spain: perception of the tutor's role in the Murcia Region

Abstract: A comprehensive questionnaire, including demographic characteristics, academic and research achievement and personal views about their role, was sent to 201 RTs in the Murcia Region of Spain. The responses are described using median and interquartile ranges (IQR);There were 147 replies (response rate 73%), 69% male, mean age 45 ± 7 yrs. RTs perception of the residents' initial knowledge and commitment throughout the program was 5 (IQR 4-6) and 7 (IQR 5-8), respectively. As regards their impact on the PMT program, RTs considered that their own contribution was similar to that of senior residents. RTs perception of how their role was recognised was 5 (IQR 3-6). Only 16% did not encounter difficulties in accessing specific RT training programs. Regarding the RTs view of their various duties, supervision of patient care was accorded the greatest importance (64%) while the satisfactory completion of the PMT program and supervision of day-to-day activities were also considered important (61% and 59% respectively). The main RT requirements were: a greater professional recognition (97%), protected time (95%), specific RT training programs (95%) and financial recognition (86%);This comprehensive study, reflecting the feelings of our RTs, provides a useful insight into the reality of their work and the findings ought to be taken into consideration in the imminent definitive regulatory document on PMT.Postgraduate Medical Training (PMT) based on the North American Residency Model was introduced in Spain in the 1960s and '70s [1] although a regulatory framework was not established until 1984 and 1995 [2] and this also defined the role of the Residents' Tutor (RT). Formal PMT Programs were introduced in the Murcia Region in the 1970s and nowadays include over 500 residents in more than 40 specialities. Abstract: objectives. study the use of non-invasive ventilation (niv) in patients with acute respiratory failure in intensive care units (icus) in spain. methods. a questionnaire was sent to 254 icus, after which, they were invited to participate in a multicenter, retrospective study, providing detailed information on ventilated patients. results. answers were received from 123 hospitals. of these, 119 used niv, although its use varied greatly. niv is the treatment of choice in 89% of the units for chronic obstructive pulmonary disease (copd), in 79% for acute pulmonary edema (ape), in 53% for postextubation failure, in 53% for pneumonia 53%, and in 17% for acute respiratory distress syndrome (ards). it was used occasionally in copd in 11% of the units, and in 21% of the units for ape. eighteen hospitals provided additional information on 432 ventilated patients, 232 (54%) of whom received niv as first line therapy. presence of pneumonia or acute respiratory distress syndrome (ards) was an independent predictive factor of niv failure (ora = 5.71; ci 95%, 1.83-17.8; p = 0.003). admission in a unit with experience in niv in > 50 patients/year (ora = 0.22; ci 95%, 0.07-0.63; p = 0.005) and a higher pao2/fio2 ratio after one hour of ventilation (ora = 0.98 per point; ci 95%, 0.97-0.99; p < 0.001) were protector factors. conclusions. in spain, niv is widely used but it may continue to be underused in copd and ape. the diagnosis of pneumonia or ards was an independent predictive risk factor. admission in an icu with niv in more than 50 patients/year also have higher pao2/fio2 ratio after one hour of ventilation were predictive factors of success.

Abstract: Actualment, ens trobem immersos en un procés de globalització i tecnificació a què les administracions públiques, com a part integrant de la societat, no poden romandre alienes. Totes les administracions, en els seus diferents nivells i àmbits d’actuació, han introdu t les noves tecnologies de la informació i la comunicació en la seva activitat, i això ha posat en marxa un procés de transformació de caràcter cultural, organitzatiu i jurídic que ha fructificat en un nou model de gestió de l’activitat pública conegut amb el nom d’administració electrònica. L’ús de les noves tecnologies i la transició d’una administració en paper a una administració electrònica han comportat una sèrie de reformes del nostre ordenament jurídic: noves normes i la reforma d’altres de ja existents que regulen els aspectes tècnics derivats de la utilització de les noves tecnologies per part de l’Administració, així com els drets i les garanties dels quals estan revestits els ciutadans quan es relacionen amb l’Administració per mitjans telemàtics. Amb aquest teló de fons, podem definir una llista de principis que han de regir l’actuació de l’administració electrònica i en la qual s’erigeixen com a protagonistes els qui persegueixen garantir la igualtat i la no-discriminació dels ciutadans, i combatre la denominada bretxa digital. We are currently immersed in a process of globalization, with an increasing use of technology. Public authorities, as an integral part of society, cannot remain indifferent to this process. All public authorities, at their varying levels and spheres of action, have incorporated the new information and communication technologies, launching a process of cultural, organizational and legal transformation that has come to fruition in a new model for managing public sector activities known as “electronic administration.” The use of new technologies and the transition from a “paper” government to an electronic government has entailed a series of reforms of our legal system: new regulations, the reform of existing regulations governing the technical aspects derived from the use of new technologies by government authorities, and the rights and guarantees that inure to citizens when they contact government authorities telematically. With this background, we can define a cast of principles governing the behavior of an electronic administration. Abstract: the work presents the concept of a structural universal and the criticisms that have been leveled against it. a structural universal is a property had by an individual due to the nature of its proper parts and due to the relations obtaining between those parts. mellor has argued that there is no reason to accept such universal in addition to the basic universals that compose them. david lewis has argued -on the other hand- that it has not been satisfactorily explained how universals are composed by other universals. the composition by which a structural universal is given cannot be a set-theoretical construction or a mereological sum. several proposals to explain the nature of structural universals are discussed. finally it is argued that a structural universal should be understood as a complexion of higher-order universals.

Abstract: the work presents and criticizes the argument developed by Graeme Forbes for the necessity of origin. In first place, the argument is studied in detail, considering in particular its presuppositions and the relation it has with the argument of Saul Kripke and Nathan Salmon for the same conclusion. In second place, the difficulties of Forbes’s argument are discussed, comparing it with the criticism by Teresa Robertson. It appears that it is (i) at best, inconclusive, and (ii) in the worst case, incoherent.

Abstract: it seems obvious -or almost- that we do possess a great amount of modal knowledge. we know a lot about necessary facts obtaining in mathematics, and we also know a lot about the contingent character of "middle size dry goods", for example. the question is how to explain that knowledge. in particular, this work will discuss the impact that the dilemma posed by paul benacerraf in the mathematical case would have over modal statements, considering the conceptions about modal facts and possible worlds advanced by david lewis and saul kripke.
