Abstract: this topic deserves particular attention due to the participation of free radicals in an increasing number of affections of great medicosocial interest. the conceptual aspects linked to these mechanisms of generation of the forms of the free radicals and their consequence at the molecular level due to their action in lipids, proteins and nucleic acids are presented. their expression is approached from the pathogenic point of view, associated with the generation of biomolecules mediators of their action and cellular repercussion. the system of antioxidant defense the body has for its protection is also dealt with. the oxidative stress and the methods available at present to measure the oxidative damage are defined and, at the same time, some of the antioxidants, their mechanisms of action and preventive function in different diseases are made known.

Abstract: La participación de los radicales libres en un número creciente de afecciones de gran interés médico-social, hace que el tema merezca un tratamiento diferenciado. Se presentan los aspectos conceptuales vinculados con los mecanismos de generación de las formas de los radicales libres y su consecuencia a nivel molecular por su acción en lípidos, proteínas y ácidos nucleicos. Se trata la expresión de estos desde el punto de vista patogénico, vinculado con la generación de biomoléculas mediadoras de su acción y repercusión celular; así como el sistema de defensa antioxidante de que dispone el organismo para su protección. Se define el estrés oxidativo y los métodos para medir el da o oxidativo que se disponen en la actualidad, y paralelamente se da a conocer de manera elemental algunos de los antioxidantes, su mecanismo de acción y función preventiva en diversas enfermedades. Administración de cartílago de tiburón a pacientes con función inmunológica comprometida por radiaciones ionizantes

Abstract: a simple open non placebo-controlled randomized prospective study which included 38 patients seen in the oncological outpatient service of "?dr luis díaz soto" higher institute of military medicine and and unequivocally diagnosed (clinically and histologically) with epithelial solid neoplasia. after beginning specific oncologic radiotherapy and verifying that initial absolute granulocytes and/or lymphocytes counts were below 30% of the normal values in patients chosen for the study, they were assigned to 4 groups of treatment with pure shark cartilage powder (scp) for 12 weeks. group 1(n=15) was treated for 3 months with an oral dose of 6g per day of scp; group 2(n= 8) and 3(n=8) were given a daily dose of 12 g and 18 g respectively during the same period of time whereas group 4(n= 7) was not treated (control group). the evaluation of the immunologic performance at the beginning and at the end of the treatment using circulating immunocomplexes (cic) and active rosette assay and spontaneous rosette assay were made in all the groups.in comparison with the control group, the administration of any of the dose prescriptions of pscp brought about significant quantitative values in immune parameters. it was observed that cic titers increased 2.7 times, ar values by 9% and er by 15% in patients treated with 6 g of scp a day. cic titers increased 4 times, ar values by 12% and er values by 29% in the group treated with 12g. dose of 18g/d of pscp was associated with intolerance.

Abstract: Se realizó un estudio prospectivo, aleatorizado simple y abierto, no controlado por placebo, el cual reclutó 38 pacientes atendidos en la Consulta Externa de Oncología del Instituto Superior de Medicina Militar "Dr. Luis Díaz Soto", con diagnóstico inequívoco (clínico e histológico) de neoplasia sólida epitelial, que después de comenzar terapéutica radiante oncoespecífica y constatar en el paciente elegido para el ensayo, conteos absolutos iniciales de granulocitos y/o linfocitos por debajo del 30 % de los valores normales, se asignaron a 4 grupos de tratamiento con polvo desecado puro de cartílago de tiburón (PDCT) durante 12 semanas. El grupo 1 (n=15) recibió tratamiento durante 3 meses con una dosis de 6 g/d por vía oral de PDCT; el grupo 2 (n=8) y grupo 3 (n=8) recibieron durante el mismo período una dosis diaria de 12 y 18 g respectivamente. El grupo 4 (n=7) no fue tratado con PDCT (grupo control). Se realizaron en todos los grupos evaluación del performance inmunológico, al inicio y al final del tratamiento con inmunocomplejo circulante (ICC), prueba de roseta activa (RA) y espontánea (RE). En comparación con el grupo control, la administración de cualquiera de los esquemas de dosis de PDCT evocó respuestas significativas en el orden cuantitativo de los parámetros inmunológicos. Se observó que los pacientes tratados con 6 g/d de PDCT experimentaron un ascenso en 2,7 veces los títulos de ICC, el 9 % los valores de RA y el 15 % los valores de RE. El grupo de 12 g aumentó en 4 veces los valores de ICC, el 12 % los valores de RA y el 29 % los valores de RE. Dosis de 18 g/d de PDCT se asoció con intolerancia. Abstract: the cuban military doctrine claims the defense sustainment to fulfill its missions in the war stages of all the population. the military invulnerability of our country needs that the man even in complex situations, to learn to live together in healthy environments. the above stated guarantees the maintenance of a favorable hygienic-epidemiologic situation allowing the development of productive, combative and mobilization abilities of troops and of communities, an essential requirement in pace and in war times. the aim of present paper is to deepen in vulnerability and sustainment features and its implementation in management at local level to health benefit of the human groups. the stability of health situation under exceptional conditions is one of the objectives of the hygienic-epidemiologic task taking into account the infectious-contagious diseases subordinate to environmental deterioration produced due to defense lengthening where the non-communicable diseases are a leading cause of inability and mortality. identification and solution of vulnerabilities is a priority strategy to confront the future complex scenarios where the main participants are the ecological changes associated with the environmental pollution, as well as other health and technologic threats. the incorporation of vulnerability and sustainment principles contribute to confront, with a territorial approach, any emergence situation in our country.

Abstract: La doctrina militar cubana demanda garantizar la sostenibilidad de la defensa para el cumplimiento de sus misiones en las etapas de la guerra de todo el pueblo. La invulnerabilidad militar del país necesita, que el hombre, aún en condiciones complejas, aprenda a convivir en ambientes saludables. Lo anterior garantiza el mantenimiento de una situación higiénico-epidemiológica favorable que posibilite el desarrollo de las capacidades productiva, combativa y movilizativa de las tropas y de las comunidades, requerimiento indispensable tanto en tiempo de paz como de guerra. Es propósito de este artículo, profundizar en los conceptos de vulnerabilidad y sostenibilidad, y su aplicación en la gestión a nivel local para beneficio de la salud de los grupos humanos. La estabilidad de la situación de salud en condiciones excepcionales es uno de los objetivos del trabajo higiénico-epidemiológico, el cual debe tener en cuenta, las enfermedades infectocontagiosas supeditadas al deterioro ambiental que se produce al prolongarse la defensa y las enfermedades no transmisibles que son una importante causa de incapacidad y mortalidad. La identificación y solución de las vulnerabilidades constituye una estrategia priorizada para enfrentar los complejos escenarios futuros, que tienen como principales protagonistas los cambios ecológicos asociados a la polución ambiental, además de otras amenazas tecnológicas y sanitarias. La incorporación de los principios de la vulnerabilidad y la sostenibilidad contribuyen, a enfrentar, con un enfoque territorial, cualquier situación de emergencia en el país. índice de Winsor en la detección precoz de arteriopatía oclusiva periférica de miembros inferiores

Abstract: an analytic, prospective and longitudinal study was conducted to evaluate the systolic blood pressure index or winsor's index in the early detection of peripheral occlusive arteriopathy of the lower limbs among patients admitted at "dr. luis díaz soto" military hospital, from 1988 to 1999. the sample was composed of 100 patients (number of admissions between october and december, 1999). once the patients were selected, a form was filled in including general data, risk factors, signs and symptoms and the hemodynamic clinical examination. every patient underwent a lipid study with serum cholesterol and triglycerides determination according to the established methodology. the complex of hemodynamic study comprised the tracing of the flow speed sign and the reading of the systolic pressure of the limbs to obtain the so-called blood pressure index. it was found that the vascular risk factors most commonly associated with circulatory disorders of the lower limbs were in order of frequency arterial hypertension, dyslipidemia and diabetes mellitus. smoking, obesity, circulatory and trophic alterations and paresthetic manifestations were observed in most of the patients, accompanied with a moderate pathological result in the measurement of the systolic pressure index, which shows that this procedure is a method for screening, diagnosing and evaluating peripheral arterial diseases.

Abstract: Se realizó un estudio analítico, prospectivo y longitudinal para evaluar el índice de presión sistólica o índice de Winsor en la detección precoz de arteriopatía oclusiva periférica de miembros inferiores, en pacientes ingresados en el Hospital Militar Central "Dr. Luis Díaz Soto" durante el período comprendido de 1998 a 1999; la muestra estuvo constituida por 100 pacientes (número de ingresos en el período de octubre a diciembre de 1999). Una vez seleccionados los pacientes, se les confeccionó una planilla de vaciamiento, en la cual se incluyeron: datos generales, factores de riesgo, signos y síntomas y el examen clínico hemodinámico. Se les realizó a cada paciente el estudio lipídico con las determinaciones de colesterol y triglicéridos séricos acorde con la metodología establecida. El complejo de estudio hemodinámico comprendió rastreo de la se al de velocidad de flujo y medición de la presión sistólica de las extremidades para obtener el llamado índice de presión. Se encontró que los factores de riesgo vasculares más frecuentes, asociados con los trastornos circulatorios en miembros inferiores, fueron en orden de frecuencia la hipertensión arterial, las dislipidemias y la diabetes mellitus. La mayoría de los pacientes examinados, en los que se hallaron práctica de tabaquismo, obesidad, alteraciones circulatorias, tróficas y manifestaciones parestésicas, se acompa ó de un resultado patológico en la medición del índice de presión sistólica, de forma moderada, lo que determina que este proceder es un método de despistaje, diagnóstico y valoración de enfermedades arteriales periféricas.
