Armamento no letal: un dilema tanto médico como ético

Abstract: the contemporary scientific and technical revolution has given humanity an important series of achievements in practically all the spheres. the scientific thinking and action that originated it was directed to progress for peace, wellbeing and the development of the productive forces. this papers deals with the development of a new type of weapon considered as "non-lethal". according to this denomination, it is presented as a "tolerable" weapon comparing its relative incapacity for producing health casualties with the multiple health casualties caused by the use of conventional weapons. emphasis is made on how harmful and deceptive the denomination of this new kind of weapon is and the attention is called on the the fact that clinical figures with combined injuries (conventional weapon plus non-lethal weapon) may appear in future wars and that the personnel giving medical attention to these people under those circunstances may have little knowldege about them.. finally, it is stressed that contemporary medicine is in a dilemma from the ethical point of view, since it has to recognize and treat clinical figures resulting from the misused effects of the attainments of the scientific and technical revolution.

Abstract: La revolución científico-técnica contemporánea ha dotado a la humanidad de un importante conjunto de logros en prácticamente todas las esferas. El pensamiento y accionar científico que la originó estaba destinado al progreso en función de la paz , el bienestar y el desarrollo de las fuerzas productivas. El artículo habla del desarrollo de un nuevo tipo de armamento el cual se denomina "no letal". Por la propia denominación de este, tiende a ser presentado como un ejemplo de armamento "tolerable" comparando su relativa incapacidad para la producción de bajas sanitarias con la conocida, mediante el empleo del armamento convencional y el de producción de bajas sanitarias múltiples. Se enfatiza en lo nocivo y enga oso de la denominación de este nuevo tipo de armamento y se alerta sobre el peligro que en guerras futuras emerjan figuras clínicas con lesiones combinadas (arma convencional más armamento no letal) de escaso conocimiento por el personal que en esas circunstancias tengan que prestar la asistencia médica a dichos damnificados. Por último, se hace hincapié en la encrucijada que en el orden ético se enfrenta la medicina contemporánea al tener que reconocer y tratar figuras clínicas resultantes de los efectos mal empleados de los logros de la revolución científico-técnica. Construcción y validación del cuestionario vulnerabilidad estrés

Abstract: a psychometric test was constructed and validated to determine stress. the test denominated stress vulnerability questionnaire was basically based on the psychometric battery of the systemic approach method for evaluating stress. an initial questionnaire composed of 63 items and 3 subscales was made to conduct a pilot study. on depurating the questionnaire, the final form contained 39 items and it was subjected to a study to find out reliability and validity. a trifactorial structure coinciding with the previous design was determined. the internal consistency was 0.92 according to cronbach's alpha and 0.90 according to spearman-brown's coefficient.. a test-retest correlation of 0.97 was attained and it was also significantly correlated to the external criteria of vulnerability-neuroticism scale of eysenck's test, spielberg's anxiety trait and the clinical criterion. it was concluded that the stress vulnerability questionnaire is a valid and reliable instrument to measure vulnerability to stress based on the systemic approach method.

Abstract: Se realizó la construcción y validación de un test psicométrico para la determinación de vulnerabilidad al estrés. El test, denominado cuestionario vulnerabilidad estrés, fue realizado básicamente a partir de la batería psicométrico del método de enfoque sistémico de evaluación del estrés. Se confeccionó un cuestionario inicial de 63 ítems y 3 subescalas con el que se realizó un estudio piloto; al depurarlo, la forma definitiva fue redactada con 39 ítems y sometida a un estudio para determinar fiabilidad y validez. Se determinó una estructura trifactorial coincidente con el dise o previo. La consistencia interna fue de 0,92 según el alfa de Cronbach y de 0,90 con el coeficiente de Spearman-Brown. Se encontró una correlación test-retest de 0,97 y se correlacionó además, de forma significativa con criterios externos de vulnerabilidad-escala de neuroticismo del test de Eysenck, ansiedad como rasgo de Spielberger y el criterio clínico. Se concluyó que el cuestionario vulnerabilidad estrés constituye un instrumento válido y fiable para medir vulnerabilidad al estrés sobre la base del método de enfoque sistémico. Marcadores contra los virus de hepatitis B y C en una población de donantes voluntarios

Abstract: a retrospective study on the presence of markers against hepatitis b andc virus was conducted in 34 711 voluntary blood donations made at the blood bank of "dr. luis díaz soto" military central hospital between 1995 and 2000. 524 (1.5 %) of them proved to be positive for the studied tests: 230 samples (0.66 %) were positive for hbsag. 287 (0.82 %) for anti-hcv and in 7 (0.02 %) it was detected the presence of both viral markers. more cases with anti-hcv than with positive hbsag (p < 0.001) were diagnosed. there was a trend towards the reduction of the total of seropositives to virus b and a mild increase to anti-hcv. 57 seropositve samples (25 hbsag, 31 anti-hcv and 1 with both markers) corresponded to military men of different ranks. the greatest number of cases wad found between 21 and 40 years old and with blood groups o+, a+ and b+. it was concluded that the selection of donors is an important way to avoid the spreading of an infection by such agents, and that it also allow to identify a number of cases that do not manifest clinically, but that may become a source of infection of this disease and provoke a reduction of the fighting capacity in case of significant outbreaks.

Abstract: an analysis is made on the psychiatric military practice and the experience accumulated in the last war conflicts. it is analyzed how they have contributed to the study of the psychosocial factors and stress in the military setting. it is considered as necessary to continue approaching stress, but making emphasis on the study of the predisposing factors, mainly of the psychosocial ones, to attain an adequate prevention.

Abstract: Se hace un análisis sobre la práctica y la experiencia psiquiátrica militar en las últimas contiendas bélicas. Se analiza cómo han contribuido estas al estudio de los factores psicosociales y el estrés en el medio militar. Se considera que es una necesidad en este medio continuar el abordaje del estrés, pero insistiendo en el estudio de los factores predisponentes, fundamentalmente psicosociales para una adecuada prevención.
