Abstract: Este estudio constituye una propuesta de corte cualitativo para explorar los ejes de racionalidad con que los alumnos de la Universidad Autónoma de San Luis Potosí (UASLP) construyen sus concepciones y expectativas, y analizan las implicaciones de la acción tutorial que los programas de tutoría implementan en su institución. De los resultados generados se encontró que los alumnos conceptualizan la acción tutorial en cinco ámbitos de atención: a) académico profesional, b) escolar y de aprendizaje, c) de integración y permanencia, d) vocacional y e) desarrollo personal y social. En cuanto a las implicaciones mencionan que las problemáticas operacionales que más inciden en la implementación de los programas de tutoría tienen que ver con la organización y la disposición del tutor para impartir las actividades tutoriales; estas debilidades, enmarcadas en la práctica de los programas de tutoría, denotan la preocupación y el deseo de los alumnos porque haya mejora y cambio en su práctica, sobre todo en los aspectos de su organización.

Abstract: introduction: the guides' great proliferation, protocols and other intrumentos of support on the clinical decision in the prevention and treatment of the sores by pressure (upp) it is accompanied of a wide variability that reflects inconsistencies and low quality of these documents. the evaluation of the guides of clinical practice (gpc) on deterioration of the cutaneous integrity in spain becomes difficult due to the fact that frequently there is absence of key elements for his identification. this way before using it she is interested in assuring the user of gpc that it is possible to qualify as a trustworthy qualit guide, valid, useful and. the gpc must be adapted to the population the one that is going to be applied. this brings i obtain the need to submit them to a process of validation of his quality. objectives: to determine the quality of the cpg on upp by the spanish version of the instrument agree.comparar the quality of the gpc pu used in the sns. material and methods: this one study tries to submit to evaluation by means of the intrumento agree of 8 gpc on deterioration of the cutaneous integrity, used in different hospitals of the national system of health. results: al to compare the punctuations standardized by areas, we obtain that 5 gpc overcomes 60 points, being catalogued like "very recommended", 3 gpc with punctuations between 60-30 points, catalogued since "recommended with conditions". no one found no gpc with punctuations lower than 30 points, catalogued like " not recommended ". conclusions: 1. the process of drawing up a cpg in the deterioration of the skin integrity, it is costly, requires much time and human resources, as well as staff with experience in the development of guides, skills in bibliographic search and critical reading of articles, which poses a significant barrier to the development of these. 2. the adaptation of the gpc high-quality, based on the existing evidence, it is an effective option and efficient that avoids unnecessa
Guías de actuación clínica en el deterioro de la integridad cutánea: dicen todas lo mismo? Clinical guidelines on the deterioration of the skin integrity: do they all say the same?

Abstract: Introducción: la gran proliferación de guías, protocolos y otros intrumentos de apoyo a la decisión clínica en la prevención y tratamiento de las úlceras por presión (UPP) está acompa ada de una amplia variabilidad que refleja inconsistencia y baja calidad de algunos documentos. La evaluación de las guías de práctica clínica (GPC) sobre úlceras por presión en Espa a se hace difícil debido a que frecuentemente hay ausencia de elementos clave para su identificación. Así, al usuario de la GPC, antes de utilizarlas, le interesa averiguar si una guía es de calidad, válida, útil y fiable. Las GPC deben estar adaptadas a la población a la que se va a aplicar. Esto trae consigo la necesidad de someterlas a un proceso de análisis de su calidad. Objetivos: determinar la calidad de las GPC sobre UPP mediante la versión espa ola del instrumento AGREE. Comparar la calidad de las GPC de UPP utilizadas en el Sistema Nacional de Salud (SNS). Material y métodos: este estudio pretende someter a evaluación mediante el intrumento AGREE (Appraisal of Guidelines Research and Evaluation) de 8 GPC sobre deterioro de la integridad cutánea, utilizadas en distintos hospitales del Sistema Nacional de Salud. Resultados: al comparar las puntuaciones estandarizadas por áreas, obtenemos que 5 GPC superan los 60 puntos, siendo catalogadas como "muy recomendadas", tres GPC con puntuaciones entre 60-30 puntos, catalogadas como "recomendadas con condiciones". No se encontró ninguna GPC con puntuaciones inferiores a 30 puntos, catalogadas como "no recomendadas". Conclusiones: 1. El proceso de elaboración de una GPC en el deterioro de la integridad cutánea, es costoso, requiere mucho tiempo y recursos humanos, así como personal con experiencia en el desarrollo de guías, habilidades en la búsqueda bibliográfica y en lectura crítica de artículos, lo que supone una barrera importante para la elaboración de estas. 2. La adaptación de las GPC de alta calidad, basadas en la evidencia existentes, es una opción efectiva y eficiente que evita la duplicación innecesaria de esfuerzos y disminuye la variabilidad asistencial con el consecuente ahorro económico para el sistema de salud. 3. Si aparecen nuevas evidencias sobre las recomendaciones en la prevención y el tratamiento del deterioro del la integridad cutánea, es conveniente incorporarlos a la GPC adaptada. 4. Las guías con mayor rigor metodológico en su elaboración y que, por tanto, podrán ser recomendadas para su utilización son: Guía de Práctica Clínica de Enfermería: Prevención y Tratamiento de UPP y otras Heridas Crónicas. Abstract: A 17-year-old nulligravida woman presented to the emergency department complaining of a six months’ history of severe dysmenorrhea. The patient underwent a laparoscopic excision of a right ovarian endometrioma in another hospital one year ago, 24 months after menarche at 14 years of age. Transvaginal ultrasonography, magnetic resonance imaging, hysteroscopy as well as laparo- scopy revealed a unicornuate uterus with a noncommunicating right rudimentary horn, a dilated right fallopian tube and a novel 40 mm right ovarian endometrioma. An excision of the right fallopian tube, the rudimentary horn and the endometrioma was performed. Since endometriosis may be originated by retrograde menstruation, an obstructive müllerian malformation should be ruled out at the diagnosis of endometriosis, particularly in adolescents. It may lead clinicians to provide an initial definitive treatment for avoidingrapid recurrent endometriosis.

Abstract: Malignant fibrous histiocytoma (MFH) is a rare tumor of the heart and the patients with these tumors usually have a poor prognosis. We report a case of MFH with an origin from the left superior pulmonary vein, involving the left atrium and protruding through the mitral valve, which needed urgent surgery. Complete resection was performed but local recurrence was detected one month later.
Gender influence in the quantity of drugs used in primary care

Abstract: objective: to analyze whether for an equal health problem there are gender differences in the drugs used in an adult population attended in the health care centers of the valencian community (spain). methods: a cross-sectional analytical study was carried out between february-august 1997. the independent variables were: socio-economic parameters, frequency of visits, and self-perceived or diagnosed health problems, and the dependent one the number of drugs consumed. results: of the 812 patients considered, 801 consumed medication. eighty percent of the health problems for which drugs were used involved 5 apparatuses and systems (mean: 3.3 health problems/patient). the 5 most consumed therapeutic groups accounted for 81% of the total (mean: 4.5 drugs/patient). significant differences were recorded, with greater female consumption in the central nervous system and genitourinary tract therapeutic groups, and with greater male consumption in relation to the respiratory system and systemic anti-infectious therapy. drug use increased with age, lowest educational level, lowest professional categories, and with the highest frequency of visits to the physician. significant differences were also recorded in drug use by occupational status, marital status and family structure. the multivariate analysis showed the number of health problems and the frequency of visits accounted for 82.2% of the variability of the variable ?number of drugs consumed?. the variability accounted for by gender was explained by the variable health problems, the main factor underlying drug consumption among women and men. conclusion: the main finding is probably that women do not use larger numbers of drugs than men if health problems and frequency of visits are controlled.

Abstract: the objective of the present paper is to deal with the existing problem for the acquisition of the hiv/aids antiretroviral drugs in the world context, mainly with the situation of the developing countries, which have to invest enough economic resources in the health sphere that are not available at present in the poorest countries in order to obtain global acces to treatment. this is motivated by the high prices of the antiretroviral drugs set by the pharmaceutical transnationals at the world market this aspect has a considerable impact in the increase of the treatment costs and it is one of the main obstacles for their availabilty in the third world countries. due to the dramatic panorama of hiv/aids in the world, the international community has pronounced itself in favor of reaching different agreements between the antiretroviral drugs producers and the goverments of the developing countries to attain an important reduction of the antiretroviral treatment cost. there are also local initiatives of some underdeveloped nations to produce these drugs to guarantee a sustained global access of patients to this therapy. all these efforts are directed to a common purpose: to have the greatest amount of antiretrovirals in the therapeutic arsenal for treating hiv/aids, as well as to look for different ways for the acquisition of these drugs under more favorable economic conditions than the existing at the international pharmaceutical market for the poorest countries, making possible the obtention of the health benefit for patients at a more reasonable cost for the damaged economies of the underdeveloped countrties.
Imported malaria in an area in southern Madrid, 2005-2008

Abstract: Descriptive retrospective study performed at HUSO. Data sources are registries from the Microbiology Department and malaria cases notified to the Preventive Medicine Department. Analysed parameters were: administrative, demographical, related to the stay at the endemic country, clinical, microbiological diagnosis method, pregnancy, treatment and prophylaxis, co-infections, and days of hospital stay.Fifty-seven patients diagnosed with malaria were studied. Case distribution per year was 13 in 2005, 15 in 2006, 15 in 2007 and 14 in 2008. Thirty-three patients were female (57.9%) and 24 male (42.1%). Mean age was 27.8 years. Most of the malaria cases were acquired in Nigeria (49.1%) and Equatorial Guinea (32.7%). 29.1% of the patients were immigrants who had arrived recently, and 61.8% acquired malaria when travelling to their countries of origin to visit friends and relatives (VFR). Majority of cases were diagnosed between June and September. Microscopy was positive in 39 cases (68.4%) immunochromatography in 42 (73.7%) and PCR in the 55 cases where performed. Plasmodium falciparum was responsible for 94.7% of the cases. The more frequent symptoms were fever (77.2%), followed by headache and gastrointestinal symptoms (33.3%). Nine cases needed hospital admittance, a pregnant woman, three children, four VFR and an African tourist, but all evolved favourably. Chemoprophylaxis data was known from 55 patients. It was taken correctly in one case (1.8%), in five (9.1%) the prophylaxis was improper while the others 49 (89.1%) cases had not followed any anti-malarial prophylaxis.Children, pregnant women and the VFR have the highest risk to present severe malaria and to need hospital admittance. Another important risk factor for acquiring malaria is incorrect prophylaxis. The first place for malaria acquisition was Nigeria and the main species causing malaria was P. falciparum.Approximately, 50% of the world's population, mostly that living in the world's poorest countries, is
