Impacto del cuidado informal en la salud y la calidad de vida de las personas cuidadoras: análisis de las desigualdades de género

Abstract: objectives: to describe informal caregivers' characteristics, quantify the effect of caregiving activities on caregivers' health, and determine whether this effect differs by sex. methods: a descriptive study was carried out based on the basque health survey 2002. physical and mental health, use of health services, social health and health related quality of life (hrql) indicators were compared in caregivers (n = 836) and non-caregivers (n = 5,706). age-adjusted logistic regression models were calculated to assess the association between the caregiver and effects on health by sex, socioeconomic position and occupational status. results: female caregivers showed worse results in 6 out of 10 indicators compared with non-caregiver women, whereas male caregivers showed worse results in only four indicators compared with non-caregiving men. the associations between caregiving (caregiving vs. non-caregiving) and health results were stronger in women than in men, except in social health. when caregiving burden intensity was considered, greatly burdened men showed a similar or higher risk of poor hrql (physical in men: or = 3.0, ci95%: 1.4-6.3; women: or = 2.3, ci95%: 1.5-3.5; mental in men: or = 2.5, ci95%: 1.4-4.3; women: or = 2.5, ci95%: 1.7-3.7) and low social support (affective support in men: or = 2.6, ci95%: 1.5-4.6; women: or = 1.5, ci95%: 1.0-2.3). however, the risk of sedentariness, lack of sleep, greater use of health services and mental ill-health remained higher for women caregivers. conclusions: caregiving damages the health of informal caregivers but the risks for female caregivers are higher due to greater intensity of caregiver burden. as men's caregiving burden increases, gender inequalities decrease or invert.

Abstract: health care related infections are one of the most common adverse events among hospitalized patients. these types of infections are related to an increased morbidity, mortality, hospitalization time, and health care related costs. chlorhexidine has been proven to be useful for preventing health care related infections due to its wide antiseptic spectrum, effectiveness and safety. different studies have shown evidence about the effectiveness of chlorhexidine in the prevention of infections related to surgical sites, vascular catheter related bloodstream infections, ventilator associated pneumonia, maternal and neonatal infections and other infections caused by staphylococcus aureus. most studies have found superiority of this compound against other antiseptics in the prevention and control of health care related infections.
Women and men facing informal caregiving: differences in strategies and meaning

Abstract: The increase of life expectancy in our population has enhanced the importance of dependent elderly care. This task is carried out mainly by women who, as a matter of fact, witness the effects of social change concerning family and work. Objective: Deepen the understanding of the different strategies that women and men take in their role as caregivers through the interpretation and analysis of their own perceptions. Methodology: with the aim of better understanding the significance of all these consequences, a qualitative study based on five focus groups with both men and women caregivers was carried out. Results: the interiorization of the ‘caregiver role’ entails a greater physical and emotional burden among women, who show a higher vulnerability towards its effects. Men respond differently to this situation, which produces unlike effects among them. These differences should be considered when designing strategies focused on gender equity and improvement of dependent elderly care.
Estado de salud y capacidad funcional de la población que vive en residencias de mayores en Madrid

Abstract: objectives: epidemiologic studies on probabilistic samples of the institutionalized elderly in spain are very scarce. the aim of this study was to describe the health and functional status of a population of older people living in nursing homes. subjects and method: a cross-sectional epidemiologic study was carried out. a sample of 800 subjects living in 19 public and 26 private nursing homes in madrid was obtained through a stratified cluster method. residents, main caregivers, doctors and nurses were interviewed. cognitive status, perceived health, depression, pain, functional capacity, chronic illnesses, pressure ulcers, urinary and fecal incontinence, drug consumption and hospitalizations were assessed. results: seventy-five percent of the sample were women. the mean age was 83.4 (standard deviation, 7.3) years. forty-four percent had some degree of cognitive impairment. forty-six percent were independent or mildly dependent in basic activities of daily living. fifty-five percent reported very good or good perceived health. thirty percent had depressive symptoms. thirty-one percent had experienced daily pain in the previous 7 days. three percent had pressure ulcers. the most prevalent illnesses were hypertension (45%), osteoarthritis (34%), dementia (33%) and anxiety (27%). the prevalence of alzheimer's disease was 14%. fifty-three percent had urinary incontinence and 26% had fecal incontinence. twenty-six percent had been transferred to an emergency unit in the previous year. conclusions: the estimates of the main variables, such as cognitive impairment and functional status, were in general more favorable than those in other populations of nursing home residents in europe and north america. this is partly explained by the greater proportion of persons with a low need for care in the nursing homes in madrid.

Abstract: the emergence of community-acquired methicillin-resistant staphylococcus aureus (ca-mrsa) as a cause of severe infections has been described in the recent years. in 2006, the first report of skin and soft tissue infection by ca-mrsa was published in colombia. herein, two additional cases of ca-mrsa are reported with a clinical course characterized by rapid progression, prolonged stay in the intensive care unit and complication of pneumonia with the onset of empyema. both adult patients developed acute renal failure, and were treated with linezolide; the subsequent clinical response showed adequate treatment response. molecular characterization of the isolates indicated the presence of the meca gene carrying the cassette sccmec type iv and the production of the toxin panton-valentine leukocidin.
Caracterización molecular de un brote por Klebsiella pneumoniae productora de CTX-M-12 en la unidad de cuidado intensivo neonatal de un hospital colombiano

Abstract: introduction. molecular characterisation of klebsiella pneumoniae strains is a tool that assits in the reduction of the disemination of drug resistance and the control of nosocomial infections that are caused by this pathogen. objective. molecular description of an outbreak of nosocomial infection caused by klebsiella pneumoniae in a neonatal intensive care unit in a tertiary level hospital in bogotá. methods: eleven klebsiella pneumoniae isolates were analysed. production of extended spectrum beta-lactamases was verified by agar diffusion tests. isoelectric points of the enzymes were determined by isoelectric focusing. the blactx-m-12 gene was detected by pcr and pulsed field gel electrophoresis genotyping was done. results. all the isolates were extended spectrum beta-lactamase producers. pulsed field gel electrophoresis and box-pcr genotyping grouped two isolates from hospital objects and eight infection-causing isolates into a single epidemic clone. the isolate from a thermometer was not grouped into the epidemic clone and showed a different resistance pattern. isoelectric focusing revealed simultaneous beta-lactamase production having different isoelectric points. pcr amplification revealed the presence of the blactx-m-12 gene in the 11 isolates studied. conclusion. this is the first report of a molecularly characterised outbreak of ctx-m-12-producing klebsiella pneumoniae from colombia. the results of this study provide additional evidence of the global dissemination of ctx-m esbl and the need for epidemiological follow-up in our hospitals.
Cerclaje profiláctico en mujeres con nacimientos prematuros espontáneos previos, asociados con infección bacteriana ascendente

Abstract: aims: determine the effectiveness of prophylactic cerclage in women with singleton pregnancies, cervix >25 mm and a history of spontaneous premature deliveries associated to ascending bacterial infection (abi). methods: women with singleton pregnancies and history of preterm births, with no fullterm deliveries, associated to abi. cases with >25 mm cervical length at admission were included. cervical cerclage performed on patients derived before 20 weeks of pregnancy was compared to the expectant management of women submitted at a later gestational stage with sustained cervical length of >25 mm. pregnant women with <25 mm cervix at referral, with cervical shortening <25 mm at expectant management, and women with previous preterm birth without placental histology were excluded. results: 51 patients were included, 23 with cerclage and 28 without cerclage. prophylactic cerclage significantly reduced the frequency of premature birth <37 weeks, 4.3% vs 35.7% and <34 weeks, 4.3 % vs 28.6 % and histologic chorioamnionitis 4.3% vs 32.1% (9/28), or (95%ci) 0.08 (0.09-0.70), 0.11(0.01-0.99) and 0.01 (0.01-0.83), respectively. conclusions: in patients with preterm births associated to abi, singleton pregnancy and cervical length >25 mm, prophylactic cerclage reduces the frequency of premature delivery <37 and <34 weeks as well as histologic chorioamnionitis.
Frutos y semillas medicinales vendidas en plazas de mercado de Bogotá, Colombia

Abstract: Se realizó el inventario de los frutos y semillas mediante encuestas semiestructuradas a cerca del uso medicinal y la caracterización de los vendedores, en siete plazas de mercado de Bogotá. Se emplearon técnicas etnobotánicas cuantitativas para determinar la validación social de la información. Se registraron 54 posibles especies pertenecientes a 29 familias y 45 géneros; además, fueron recopilados 80 nombres comunes. Se registraron 33 especies en fruto, 20 en semillas y solo una especie fue comercializada como fruto y semilla. Se establecieron dos tipos de plazas de mercado y se crearon cuatro grupos de frutos y semillas medicinales de acuerdo a los índices utilizados.
