Exercise-Induced Hematuria as the Main Manifestation of Migration of Intrauterine Contraceptive Device into the Bladder

Abstract: Intrauterine device (IUD) is a common contraceptive method, due to its cost-effectiveness and low complication rates. Uterine perforation is a possible complication and IUD migration to the bladder is a rare and morbid condition. The present report describes an interesting case in which the urinary manifestations started 13 years after insertion, and the main clinical finding was exercise-induced hematuria. 1. Introduction Intrauterine device (IUD) is a widely used reversible contraceptive method, due to its cost-effectiveness and low complication rates [1]. Serious complications are rare; however migration to adjacent pelvic organs is described [2], including the bladder with stone formation and lower urinary tract symptoms [3]. We describe a case of intravesical IUD migration with urologic manifestations started 13 years after insertion, and the main clinical finding was exercise-induced hematuria. 2. Case Report A previously healthy 39-year-old woman presented with recurrent episodes of gross hematuria after middle distance running (5 to 10?km) that ceased some hours after sports activity, rest, and hydration. Moreover, she reported dysuria, urge to urinate, and suprapubic pain, which also worsened with physical activity. Those symptoms were present for the past 4 months and worsened progressively, becoming more intense in recent weeks. In her previous medical and gynecologic anamnesis, she informed that 13 years ago she underwent implantation of IUD, few weeks after cesarean delivery. After the IUD implant, she complained of suprapubic pain and vaginal bleeding treated with rest and analgesics, with remission of symptoms after approximately 1 month. She was assessed by vaginal ultrasound and informed that IUD was not visualized; twelve years after the implantation of IUD she became pregnant again and had a second cesarean. Gynecologic examination was normal, urinalysis revealed microhematuria, and leukocyturia and urine culture demonstrated the presence of Escherichia coli. She was treated with antibiotics, with remission of symptoms only for 4 weeks. Due to recurrence of the symptoms, she was referred to the urologic department for further evaluation. Ultrasonography demonstrated a 20?mm calculus in the bladder dome (Figure 1) and pelvic helical computed tomography revealed a metallic foreign body partially located intravesically with adjacent calcification (Figure 2). Cystoscopy, performed under anesthesia, confirmed the intravesical IUD (partial), complicated by stone that was grasped by forceps, and extracted completely through the endoscopy
Expression of VEGF and collagen using a latex biomembrane as bladder replacement in rabbits

Abstract: objective: to investigate the vegf expression and collagen deposition using a latex biomembrane as bladder replacement in rabbits. materials and methods: after partial cystectomy, a patch of a non-vulcanized latex biomembrane (2 x 2 cm) was sewn to the bladder of rabbits with 5/0 monofilament polydioxanone sulfate sutures in a watertight manner. groups of 5 animals were killed at 15, 45 and 90 days after surgery and the bladder was removed. sections of 5μm were cut and stained with picrosirius-red in order to estimate the amount of extracellular matrix in the graft. to confirm the presence of vegf in tissues, protein expression was determined by immunohistochemistry. results: no death, urinary leakage or graft extrusion occurred in any group. all bladders showed a spherical shape. a progressive reduction in the amount of collagen occurred in the graft area and was negatively and linearly correlated with time (p < 0.001). vegf expression was higher in grafted areas when compared to controls at 15 and 45 days after surgery and decreased with time (p < 0.001). conclusion: the latex biomembrane as a matrix for partial bladder replacement in rabbits promotes temporary collagen deposition and stimulates the angiogenic process.
Territorial hypothesis predicts the trade-off between reproductive opportunities and parental care in three species of damselfishes (Pomacentridae: Actinopterygii)

Abstract: paternal care is rare in most animals, but common in fishes, including pomacentridae. one way of explaining it is through the territorial hypothesis, which predicts that paternal care may evolve when caring for a territory with eggs will not cost more than territories without eggs. here we describe the reproductive behavior of three pomacentrids from s?o paulo, brazil, and evaluate if the territorial hypothesis explains their behavioral variation through five predictions. we checked if females have multiple spawns (prediction 1), if they forage more (prediction 2) and if territory defence demands more when eggs are present (prediction 3). we also described how visible eggs are (prediction 4) and how available nesting sites are (prediction 5). chromis multilineata differed from abudefduf saxatilis and stegastes fuscus by not guarding the eggs, spending less time with territoriality and fanning eggs; and more time feeding and foraging. females of the three species are iteroparous. females' a. saxatilis and s. fuscus foraged more than males, which spent the same amount of time in territorial defence before and after eggs were delivered. these two species also have very conspicuous egg clutches on clean rock surfaces defended by them. on the other hand, males and females of c. multilineata spent the same time foraging, while territoriality took more effort after eggs were present and it spawns cryptic eggs amid sargassum, a common and disperse micro-environment in s?o paulo rocky shores. territorial hypothesis explains why paternal care by a. saxatilis and s. fuscus lasts longer than by c. multilineata.

Una forma de explicar este fenómeno es la hipótesis territorial, que predice que el cuidado paterno puede evolucionar cuando la protección de un territorio con huevos no cuesta más que la de territorios sin huevos. Aquí se describe el comportamiento reproductivo de tres pomacentrídios de S o Paulo, Brasil, y se evalúa si la hipótesis territorial explica la variación de su comportamiento a través de cinco predicciones. Se analizó si las hembras tienen múltiples desoves (predicción 1), si forrajean más (predicción 2) y si la defensa del territorio es mayor cuando los huevos están presentes (predicción 3). Abstract: this article aims to analyze a case of outward-bound move of state governments in the realm of external loans during the period of the so-called old republic (1890-1930) in brazil. based on the definition of "financial paradiplomacy," it argues that the brazilian historical experience in this regard would have been a typical case of paradiplomatic action. broad statistical survey of foreign loans borrowed by states, the rescue of primary sources about the subject and historical perspective give relevant support to characterize the period as a time of early financial paradiplomacy.
Produ o de dados de tráfego sintéticos através de algoritmo genético e simula o microscópica

Abstract: Resumo: Neste artigo, prop e-se um método para produzir, por meio de um simulador, uma série de dados de tráfego sintéticos que representem diversas condi es operacionais e comportamentais, como os fornecidos por sensores instalados nas vias. O método proposto baseia-se num algoritmo genético (AG), que automaticamente calibra um modelo de simula o microscópico. Entre os cromossomos (solu es) gerados pelo AG na calibra o do simulador, s o escolhidos os que produzem resultados marginalmente inferiores ao ótimo. As distribui es de valores dos parametros que formam os cromossomos deste subconjunto s o usadas para gerar, através de uma amostragem de Monte Carlo, parametros para as replica es da simula o que s o usadas para produzir os dados sintéticos. A viabilidade do método proposto é demonstrada através de um estudo de caso, no qual o simulador usado foi o TWOPAS e os dados de tráfego foram obtidos de sensores instalados no km 98 da SP98, uma rodovia de pista simples no estado de S o Paulo.Abstract: The paper proposes a method for generating, by a simulator, synthetic traffic data which represents a wide range of operational and behavioral conditions, such as those obtained from detectors installed on roads. The proposed method uses a genetic algorithm (GA) to calibrate a traffic simulation model automatically. A subset of chromosomes (solutions) generated by the GA during the calibration of the simulator containing those that produce results marginally inferior to the best solution is used to produce distributions of feasible values for the simulation model parameters. The distributions of simulation model parameters contained in the chromosomes of this subset of solutions are used to produce, by Monte Carlo sampling, parameters for the simulations that generate the synthetic data. The feasibility of the proposed approach is demonstrated by a case study using TWOPAS and data from detectors installed on SP98, a two-lane highway in the state of S o Paulo, Brazil.

Abstract: decisive factors affecting the recent increase in formal employment in brazil. this paper gives a general overview of the evolution of labour market indicators between 1995 and 2005 in brazil. it shows an overall increase in formal employment rates from 2001 to 2005, as opposite to what had happened from 1995 to 1999. it is argued that such recent trends might indicate the reconfiguration of the labour market in better terms, with potential positive consequences to the finance performance of the social security sector. the paper also examines some of the major factors associated with this new trend and their chances to maintain such tendency in the near future. it's important to notice that all of them may be subject to some kind of political management by the state. in other words, we suggest that there are suficient instruments and operative skills in the brazilian state to make these and others factors work in favour of a more persistent strategy of development with social inclusion through labour.
