Ventilación de alta frecuencia oscilatoria en barotrauma resultante de un síndrome de dificultad respiratoria aguda

Abstract: introduction: the inappropriate use of mechanical ventilation in acute respiratory distress syndrome may increase the primary lesion and complicate it with a persistent air leak capable of obscuring the diagnosis. the oscillatory high frequency ventilation is an available modality to rescue a refractory air leak at conventional mechanical ventilation. the aim of this paper is to report the effect of this ventilatory support on gas exchange. and on the evolution of the air leak in patients with acute respiratory distress syndrome. methods: the ventilatory support was applied to all the patients admitted between 1999 and 2006 due to acute respiratory distress syndrome, with persistent or recurrent barotrauma that altered the gas exchange. the time of persistence of the air leak, as well as the mortality and morbidity for this group, were described. results: 19 patients whose average age was 17 months were ventilated. before starting ventilation, pao2/fio2 was 66; oxygenation rate was 24, and paco2 was 75 mm hg. its mean duration was 111 h. the air leak was eliminated in 79 % of the cases and it significantly improved the gas exchange. survival at 30 days was 89 %. conclusions: high frecuency ventilation is useful in most of the patients presenting this syndrome complicated with refractory barotrauma, and it is an unquestionable therapeutical option.

Abstract: INTRODUCCIóN. El uso inapropiado de ventilación mecánica en el síndrome de dificultad respiratoria aguda puede amplificar la lesión primaria y complicarse con un escape aéreo persistente, capaz de opacar el pronóstico. La ventilación de alta frecuencia oscilatoria es una modalidad disponible para el rescate de un escape aéreo refractario a ventilación mecánica convencional. Este trabajo tiene como objetivo reportar el efecto de este soporte ventilatorio sobre el intercambio gaseoso y evolución del escape aéreo en pacientes con síndrome de dificultad respiratoria aguda MéTODOS. Se aplicó este soporte ventilatorio a todos los pacientes que ingresaron entre 1999 y 2006 a causa de síndrome de dificultad respiratoria aguda, con barotrauma persistente o recurrente, que alteró el intercambio gaseoso. Se describió el tiempo de persistencia del escape aéreo y la morbilidad y mortalidad para este grupo. RESULTADOS. Se ventilaron 19 pacientes, cuya mediana de edad fue de 17 meses. Antes de comenzar la ventilación, la PaO2/FiO2 fue de 66; el índice de oxigenación de 24 y la PaCO2, de 75 mm Hg. La duración de esta presentó una mediana de 111 h. Se abolió el escape aéreo en un 79 % de los casos y pudo mejorar significativamente el intercambio gaseoso. La sobrevida a los 30 días fue del 89 %. CONCLUSIONES. La ventilación de alta frecuencia es útil en la mayoría de los pacientes afectos de este síndrome complicado con barotrauma refractario y constituye una opción terapéutica indiscutible. Low-Q whispering gallery modes in anisotropic metamaterial shells

Abstract: Anisotropic and inhomogeneous metamaterial shells are studied in order to exploit all their resonant mode richness. These multilayer structures are based on a cylindrical distribution of radially dependent constitutive parameters including an inner void cavity. Shell, cavity and whispering gallery modes are characterized, and special attention is paid to the latter ones. The whispering gallery modes are created at the boundary layers of the shell with the background and energy localization is produced with highly radiative characteristics. These low-Q resonant states have frequencies that are independent of the shell thickness. However, their quality factors can be controlled by the number of layers forming the shell, which allows confining electromagnetic waves at the interface layers (internal or external), and make them suitable for the harvesting of electromagnetic energy.
Experimental evidence of giant pure optical activity in a metasurface based on a complementary twisted cross configuration

Abstract: This work presents an experimental study of giant and pure optical activity in a periodic structure consisting of twisted crosses and complementary crosses patterned on the sides of a copper coated dielectric board. Additionally, a multilayer system is proposed and numerically studied to broaden the transmission bandwidth. Our results show that a dual band behavior can be obtained due to coupling effects between the layers whilst maintaining the dispersionless giant optical activity and negligible circular dichroism. We theoretically study the effect of the separation between layers and its influence on the transmission spectra.
Experimental evidence of total absorption by a thin absorbing layer deposited on a patterned metallic surface

Abstract: This work presents the experimental demonstration of total absorption by a metal-dielectric metasurface. Following the theoretical proposal [A. D\'iaz-Rubio et al. Phys. Rev. B 89, 245123 (2014)], we fabricated a metasurface consisting of a low absorbing dielectric layer (made of FR4) deposited on top of a metallic surface patterned with a square distribution of coaxial cavities. For P-polarized waves, it is observed a low frequency peak with perfect absorption. The behavior of this peak has been experimentally characterized for different dielectric layer thicknesses, coaxial cavity lengths and incidence angles. The experimental results are in excellent agreement with numerical simulation and support the previous theoretical findings.

Abstract: this paper addresses 'absolute knowing', the process whereby the experiences of consciousness reach their highest point, as hegel discusses in the phenomenology of spirit. the objective is to analyze this concept both in its epistemological and cartesian perspective, that is, as fundamental knowledge and in its religious meaning, that is, as knowledge of the absolute. with these two perspectives hegel seeks to open the path towards speculative thinking where subject and object find their final identity and towards the 'sublating' (aufheben) of religion into philosophy in the threefold sense of suppressing, elevating and preserving it. this paper also deals with some of the main objections to the above mentioned twofold perspective in phenomenology.

Abstract: along the second half of xix century, in the southwest of colombia, they were built at least 30 bridges in arch of brick shape: a complex structural typology that doubtless required of an ensemble in specific technical knowledgements for its execution. in what way it was then carried out and there one constructive tradition able to reach a madurity that allowed to achieve works significant importance? through an investigate process that combinated a field work and the documentary archives searching, it obtained to determine its origin in the figure of an italian priest who arrived to popayan city in 1859, and whose practice teachings to workers and local foreman served as base in a process of singular technology appropiation. through the historiographic report of the work of serafin barbetti and the analysis of his work positioning it among the technical culture of his epoch it gained to determine its role as a transmitter agent of knowledgements and the adaptations that made it successful. this work is framed in an investigation line much more connected with the history of the constructive technique and its mechanisms of assimilation and diffusion.

Abstract: this article seeks to show how during much of 18th century teachings on perspective taking place at the academies of military engineers had to rid itself of the symbolic character it bore during many years for/ architects, painters, and sculptors. through the case study of the royal academy of mathematics of barcelona, it is evident that this discipline became - along with statistics, mechanics, and other mathematical sciences - a reachable universe of knowledge for engineers in modern times.
