Abstract: Protecting groundwater from lead contamination is an important public-health concern and a major national environmental issue worldwide. This article addresses myGeoffice Web Internet service for geographers, in general, and geo-statistics researchers, in particular, with the famous water contamination case at Jura lake, Switzerland (a typical rural-urban region). Based on 189 samples of lead (Pb), five key investigation steps for a scientific perspective of any pollution incident are presented: Descriptive analysis (including nearest neighborhood, G(h) and Kernel techniques), spatial autocorrelation (Moran location scatterplot and Moran I) and Ordinary Kriging (OK) interpolation. The uncertainty and cost assessments issues are exceptionally tackled with Indicator Kriging (including the conditional cumulative distribution function, Shannon local entropy, probabilistic intervals and E-type estimation) and Gaussian geo-simulation. The total lead pollution exhibited patterns of high and low levels of concentrations along the all lake, leading to the conclusion that water is unsuitable for human consumption, in general, and unsuitable for any living organism, in particular sub-areas. It is also hoped that future GIS readers will follow this approach for their spatial cases with myGeoffice.
Interfer o peguilado e ribavirina em doentes com hepatite crónica sem resposta sustentada ao interfer o standard e ribavirina

Abstract: Introdu o: Apesar de submetidos a tratamento, muitos dos doentes com hepatite C crónica permanecem infectados. Com o interfer o peguilado e ribavirina, a resposta virológica sustentada ultrapassou os 50%, um acréscimo de 17 a 25% comparativamente ao interfer o standard e ribavirina. Tal facto poderá fundamentar o retratamento dos doentes sem resposta a tratamentos anteriores. Objectivos: Avaliar a eficácia do tratamento com interfer o peguilado e ribavirina, após falência do interfer o standard e ribavirina. Identificar factores preditivos de boa resposta. Doentes e Métodos: Análise retrospectiva dos doentes retratados com interfer o peguilado e ribavirina após falência do interfer o standard e ribavirina. Resultados: Identificaram-se 27 doentes (78% do sexo masculino, média de idades de 43 anos). O genótipo 1 foi identificado em 23 doentes. Catorze tinham virémias superiores a 500.000 UI/L. Quinze doentes tinham efectuado interfer o standard em monoterapia. Todos cumpriram tratamento com interfer o standard e ribavirina, sendo a resposta nula em dois ter os. Com o retratamento (interfer o peguilado e ribavirina), a resposta sustentada atingiu 25,9% (7 doentes). Os factores preditivos de boa resposta foram: resposta virológica precoce ao retratamento e ausência de curso prévio de interfer o em monoterapia. Conclus es: O retratamento deverá ser considerado, sobretudo se estiverem presentes os factores preditivos de boa resposta. Introduction: Treatment failures were frequent before peginterferon and ribavirin become standard therapy for chronic hepatitis C. Overall, a sustained virologic response can now be achieved in over 50% of patients, a 17-25% higher rate than that with standard interferon and ribavirin, which may sustain a retreatment strategy for patients with no response to previous therapies. Aims: To evaluate the effectiveness of retreatment with peginterferon and ribavirin in patients who failed treatment with standard interferon and ribavirin and to identify factors associated with a favourable outcome. Patients and Methods: Retrospective analysis of patients retreated with peginterferon and ribavirin after treatment failure with standard interferon and ribavirin. Results: Twenty-seven patients were included (78% male, mean age of 43 years, 85% with genotype 1). Serum HCV RNA was higher than 500.000 IU/L in 14 patients. Abstract: Introdu o: A hepatite C é um importante problema de saúde pública. Em Portugal desconhece-se a verdadeira magnitude do problema e dispomos de poucos dados caracterizadores da doen a, o que limita a eficácia de potenciais medidas preventivas. Objectivos: Caracteriza o da popula o seguida na consulta de hepatologia com o diagnóstico de hepatite C. Avalia o da eficácia terapêutica. Doentes e Métodos: Análise retrospectiva dos doentes com o diagnóstico de hepatite C seguidos na consulta de hepatologia, por um período de 10 anos (1995 - 2004). Resultados: Dos 618 doentes avaliados com idade média de 36,7 anos, 74% eram do sexo masculino, em 74% a via de transmiss o foi a parentérica e 60,8% apresentavam genótipo 1. A associa o Peginterferon e Ribavirina obteve taxas de resposta sustentada de 38% e 85% para os genótipos 1 e 3a, respectivamente. Foram observados 13 doentes com hepatite aguda C; em 83% dos casos em que se efectuou terapêutica houve negativa o do ARN do vírus da hepatite C. Conclus o: A nossa série é composta maioritariamente por adultos jovens com hábitos toxicofílicos. Os genótipos predominantes foram os 1a/1b. A eficácia terapêutica foi semelhante à descrita na literatura. A terapêutica da hepatite C aguda acompanhou-se de uma elevada taxa de resposta sustentada. Very limited data is available on Portuguese Hepatitis C patient population, which limits the effectiveness of potentials preventive measures. Aims: Characterization of the hepatitis C outpatients. Evaluation of the therapeutic effectiveness. Patients and Methods: Retrospective analysis of outpatients with hepatitis C diagnosis between January 1995 and December 2004. Results: A total of 618 patients were evaluated with a mean age of 36,7 years, 74% was male, in 74 % the transmission route was the parenteric and 60,8% was genotype 1. With the association Peginterferon and Ribavirin the sustained virologic response rates were 38% and 85% for genotypes 1 and 3a, respectively. Acute hepatitis C was observed in 13 patients; 83% of the treated patients achieved sustained virologic response. Conclusion: Our study group is composed mainly by young adults with history of intravenous drug use. Genotypes 1a/1b were the most prevalent. The therapeutic effectiveness was similar to other series. The treatment of acute hepatitis C had a high sustained virologic response rate.
Síndrome hepatopulmonar em doente com cirrose hepática: importancia da sua pesquisa sistemática e impacto no prognóstico

Abstract: Introdu o: A síndrome hepatopulmonar (SHP), uma complica o importante da cirrose hepática, encontra-se, provavelmente, subdiagnosticada, sendo tal facto preocupante pelas implica es prognósticas e terapêuticas que a sua identifica o suscita. Objectivos: Determinar a prevalência e impacto prognóstico da SHP em doentes com cirrose hepática. Doentes e Métodos: Estudo prospectivo de 48 doentes com cirrose hepática, incluídos consecutivamente após observa o em consulta ou internamento (sexo masculino - 77%, média etária - 59,3 anos, etiologia alcoólica - 85%, Child-Pugh A-29%, B-46%, C-25%), submetidos a gasimetria arterial e, se identificadas altera es da oxigena o, ecocardiograma de contraste. O diagnóstico de SHP foi estabelecido em presen a de gradiente alvéolo-arterial de O2 aumentado e vasodilata o pulmonar. Todos os doentes foram mantidos em seguimento. Resultados: Identificaram-se 4 doentes com SHP (8,3%). Nenhum tinha sintomas respiratórios e houve maior representa o de cirrose hepática avan ada (p=0,006). Com 7,8 meses de seguimento médio, a mortalidade foi significativa e independentemente mais elevada nos doentes com SHP (75% vs 13,6%, p=0,017). Conclus es: A SHP foi relativamente frequente, particularmente na cirrose hepática avan ada. Aausência de sintomas respiratórios nos casos diagnosticados, bem como a sua identifica o como factor independente de prognóstico, traduzida por uma mortalidade significativamente mais elevada, aconselham à sua pesquisa sistemática. Introduction: Hepatopulmonary syndrome (HPS) is an important complication of liver cirrhosis associated with worse prognosis and increased mortality. It is probably underdiagnosed, which is relevant due to the therapeutic implications associated with its identification. Aims: To determine the prevalence and prognostic impact of HPS in patients with liver cirrhosis. Patients and Methods: Prospective study of 48 consecutive patients with liver cirrhosis (77% male, 59 years average age, 85% alcoholic aetiology, Child-Pugh A-29%, B-46%, C-25%), evaluated by arterial blood gas analysis and, when abnormal, contrast echocardiography. The diagnosis was established if increased alveolar-arterial oxygen gradient and pulmonary vasodilatation were simultaneously present. All patients, with and without HPS, were followed. Results: From 48 patients studied, 4 (8,3%) had HPS. Among these patients, advanced stages of cirrhosis were overrepresented and none had respiratory symptoms. Abstract: The present paper is a review of the theoretical advances and empirical findings related to social representations according to the structural ap- proach, a research stream that aims at studying the influence of social fac- tors in thinking processes through the identification and characterization of relationship structures. The presentation of the approach begins with the baseline definitions of social representations according to a structural approach, moving on to an overview on the nature of representation ele- ments, the relationships between representations and practices, cognitive scheme dimensions, central core theory, representation transformations and interaction context effects. In addition to positioning ourselves concerning polemic topics during the review, in the final section we evaluate briefly the current state and future perspectives of structural research on social representations, mostly addressing the problem of defining consensus, the difficulty of characterizing a collective construct from individual data, and the secondary importance of content in structural laws.
The Doppler peaks from a generic defect

Abstract: We investigate which of the exotic Doppler peak features found for textures and cosmic strings are generic novelties pertaining to defects. We find that the ``out of phase'' texture signature is an accident. Generic defects, when they generate a secondary peak structure similar to inflation, apply to it an additive shift. It is not necessary for this shift to be ``out of phase''. We also show which factors are responsible for the absence of secondary oscillations found for cosmic strings. Within this general analysis we finally consider the conditions under which topological defects and inflation can be confused. It is argued that only $\Omega=1$ inflation and a defect with a horizon size coherence length have a chance to be confused. Any other inflationary or defect model always differ distinctly. Abstract: If the speed of sound were vastly larger in the early Universe a near scale-invariant spectrum of density fluctuations could have been produced even if the Universe did not submit to conventional solutions to the horizon problem. We examine how the mechanism works, presenting full mathematical solutions and their heuristics. We then discuss several concrete models based on scalar fields and hydrodynamical matter which realize this mechanism, but stress that the proposed mechanism is more fundamental and general.
