Abstract: BACKGROUND AND AIMS: Current Guidelines recommend that cirrhotic patients should undergo upper gastrointestinal endoscopy for screening of esophageal varices. Several studies evaluated the ability of non-invasive markers to predict the presence of esophageal varices. We aimed to test the validity of platelet count-to-spleen diameter ratio (PC/SD) to predict the presence of esophageal varices as well as endoscopic signs of severe portal hypertension (large varices, red signs on esophageal varices and severe portal hypertensive gastropathy) using the 909 cut-off value. PATIENTS AND METHODS : Retrospective study of 170 cirrhotic patients, who underwent screening for esophageal varices. Liver cirrhosis etiology, laboratory, ultrasonography and endoscopy data were analyzed. STATS: Mann-Whitney test, X2, ROC curves. RESULTS: 170 patients (55 female) were included. One hundred and three (60,6%) had esophageal varices (34,1% large; 11,8% with red signs). Ninety-seven patients (57,1%) had portal hypertensive gastropathy (PHG): with severe PHG in 38 (22,3%). PC/SD <909 was documented in 93 patients. It was significantly associated with the presence of esophageal varices (P<0,001; AUROC= 0,84) and high-risk esophageal varices (large varices or red signs - P<0,001; AUROC= 0,8). The score was also significantly associated with the presence of severe portal hypertensive gastropathy (P<0,001; AUROC= 0,63). CONCLUSIONS: PC/SD is significantly associated with the presence of esophageal varices and portal hypertensive gastropathy. A PC/SD ≥909 is a good predictor of the absence of high-risk varices or severe portal hypertensive gastropathy INTRODU O E OBJECTIVOS: O rastreio de varizes esofágicas através de endoscopia digestiva alta está recomendado nos doentes com cirrose hepática. Abstract: Background and aim: Diverticular disease of the colon is a common disease, representing na important health problem in Western countries. The authors aimed to study the visceral fat and parameters of obesity in the diverticular disease of the colon. Methods: Case-control study of unselected medium-risk subjects who underwent colonoscopy for screening of colorectal cancer during 1 year. Subjects were inquired by a nutritionist about nutritional habits. Anthropometric variables were evaluated. Visceral and subcutaneous fat were assessed by ultrasound by the same gastroenterologist. Statistics: x2, t test, logistic multivariate regression, odds ratio (OR). Results: Included 303 individuals, 46.9% female, mean age 60±6.6 years. Sixty-four (21%) individual had diverticular disease of the colon. People with diverticula were signifi cantly older (P=0.01), had more visceral fat (P < 0.01), waist circumference (P=0.01) and total fat consumption (P=0.01). Logistic regression analysis showed age and visceral fat as independent risk factors for diverticular disease of the colon. The probability of occurrence of disease was 3-times higher in individuals in the 3rd tertile of age (older than 63 years old) than those younger than 56 years old (1st tertile of age) - OR 3.1, 95% CI 1.5-6.5. For visceral fat, those individuals in the 3rd tertile had a two-fold risk of having diverticular disease of the colon (OR 2.3, 95% CI 1.02-5.2) than those in 1st tertile. There was no significant difference for sex, body mass index, subcutaneous fat or fiber intake. Os autores pretenderam estudar a rela o da gordura visceral e outros parametros de obesidade na doen a diverticular do cólon. Métodos: Estudo de indivíduos n o seleccionados, de médio risco que efectuaram colonoscopia para rastreio de cancro colorectal, durante um ano. Os indivíduos responderam a inquérito nutricional por nutricionista. Foram avaliadas variáveis antropométricas. A gordura visceral e subcutanea foram avaliadas através de ecografia abdominal efectuada pelo mesmo gastroenterologista. Evidence for the presence of a kininogen-like species in a case of total deficiency of low and high molecular weight kininogens

Abstract: low and high molecular weight kininogens (lk and hk), containing 409 and 626 amino acids with masses of ~65 and 120 kda after glycosylation, respectively, are coded by a single gene mapped to the human chromosome 3 by alternative splicing of the transcribed mrna. the nh2-termini glu1-thr383 region, identical in lk and hk, contains bradykinin (bk) moieties arg363-arg371. lk, hk and their kinin products lys-bk and bk are involved in several biologic processes. they are evolutionarily conserved and only 7 patients, all apparently normal, have been reported to lack them. in one of these patients (williams' trait), a codon mutation (arg178 ? stop) has been blamed for the absence of lk and hk. however, using western blots with 2 monoclonal anti-hk antibodies, one that recognizes the region common to lk and hk and the other that recognizes only hk, i detected ~110-kda bands in the plasma of this lk/hk-deficient patient vs ~120-kda bands in normal human and ape plasmas. with polyclonal anti-lys-bk antibody, which strongly detects bk cleaved at its cooh-terminus in purified hk, i detected ~110-kda bands in the normal and the deficient plasmas. western blots with a monoclonal anti-prekallikrein (pk) antibody showed that surface activation of pk and distribution of pk activation products, both dependent on hk, were similar in these plasmas. these findings suggest that a mutant gene yielded a kininogen-like species possibly involving aberrant mrna splicing - structurally different from normal hk, but apparently with the capacity to carry out seemingly vital hk functions.
