Nutri??o Entérica por Gastrostomia Endoscópica em Doentes com Idade Pediátrica: Avalia??o Retrospectiva em 40 Doentes Consecutivos

Abstract: introduction: percutaneous endoscopic gastrostomy (peg) is the gold standard in long term tube feeding of pediatric patients. however, there is still some restriction to peg placement, in some places, due to the concept that this techni-que should only be performed in pediatric gastrointestinal units. objectives: to evaluate a group of long term gastrostomy fed children followed by an enteral nutrition team which assists adults and children. evaluation included success of peg placement, safety, evolution and outcome. methods: from peg patients we selected children ≤ 16 years. we evaluated demographic data, underlying disease, gastrostomy placement, complications, clinical outcome. using spss, survival of children with stable lesions was compared with children with progressive diseases. results: gastrostomy placement was achieved in all 40 children sent to us for that purpose with neurological or muscular diseases. there were only a few major complications and surgery was never needed. du-ring the first 6 months after peg placement, every child gained weight. children with stable lesions had a longer survival (p=0,002). conclusions: peg placement was successful and safe. enteral nutrition in children was successful resulting in increased weight. child survival was related with the underlying disease. enteral feeding teams working with adults should also take care of enteral fed children, with the support of pediatricians

Abstract: looked for to investigate the existence of the postpartum home for the woman starting from his/her perception concerning the feelings, changes and situations of that period. in a qualitative approach, we investigated eight women, in the 15th and 30th days of postpartum, attended in consultation of nursing of revision puerperal in the basic unit of health (ubs/sms/rj). for analysis of the data we used the technique of content analysis, where it emerged the following categories: service of the expectations, situation of the interpersonal relationship, situation of the intrapersonal relationship (feelings and sensations; relationship with the body). we verified that the witness lived the postpartum with satisfaction, however we also identified a depreciation of the being woman and of the care of herself on behalf of the attention to the newly born. we noticed the importance of the family participation in the maternal process and the accomplishment of a postpartum attendance that it approaches the real feelings of that period, of the rescue of the autonomy on themself and other aspects of the subjectivity of the women in the postpartum.
Eventos Cerebrais Isquémicos Recorrentes e Encefalopatia, Como Forma de Apresenta??o do Linfoma Difuso de Grandes Células B

Abstract: introdu??o: o linfoma difuso de grandes células b (ldgcb) é um subtipo raro de linfoma extranodal, sem atingimento habitual da medula óssea, sangue periférico ou líquor. o sistema nervoso central e a pele s?o os locais mais frequentemente afectados. o mau prognóstico e o reduzido tempo de sobrevida habitualmente inerentes a este linfoma re?ecte a sua agressividade e pode explicar a di?culdade de diagnóstico atempado. os autores apresentam o caso clínico de um doente com ldgcb do sistema nervoso em que a forma inicial de apresenta??o se traduziu por episódios transitórios de dé?ces neurológicos, simulando episódios isquémicos transitórios (aits). caso clínico: doente do sexo masculino, 54 anos, sem antecedentes patológicos de relevo, admitido por episódios transitórios de dé?ces sensitivo-motores. o estudo vascular inicial foi negativo. a ressonancia magnética cerebral (rmc) mostrou duas les?es inespeci?cas, uma no lobo frontal e outra na regi?o periventricular. um mês depois o doente desenvolve quadro de deteriora??o progressiva das fun??es cognitivas e paraparésia simétrica. realizou eeg, que revelou encefalopatia; o doseamento sérico da desidrogenase do lactato, da proteína c reactiva e das proteínas totais no líquor estavam alteradas; foi excluído vasculite, neoplasia oculta, quadro infeccioso e doen?a autoimune. o doente desenvolveu um quadro de hiperbilurrubinémia, com hepatoesplenomegalia revelada por tc abdominal e a biópsia hepática mostrou in?ltra??o do parênquima hepático por linfoma difuso de células b gigantes. no período que se seguiu a este diagnóstico o doente iniciou um quadro rapidamente progressivo de encefalopatia e de falência multiorganica, acabando por falecer ao 42o dia após a admiss?o. conclus?o: o caso clínico apresentado traduz a di?culdade inerente ao diagnóstico do linfoma angiotró?co. real?a-se a forma de manifesta??o invulgar da doen?a que mimetiza a presen?a de doen?a cerebrovascular.

Abstract: pertussis, or whooping cough, is a highly infectious respiratory disease, endemic all over the world, caused by bacteria bordetella pertussis. the authors describe the case of a 36 day old infant, brought to the emergency room due to respiratory distress and cyanosing cough, admitted with suspected pertussis infection complicated by bacterial pneumonia, having begun large spectrum antibiotics. labs showed hyperleukocytosis and thrombocytosis. she was transferred to an intensive care unit because of a worsened state, dying 20 hours after admission with pulmonary hypertension and haemorrhage. pertussis was diagnosed by pcr. over the last few years, there has been an increase in the number of cases of pertussis. adolescents and adults have become an under-recognized but significant source of infection, particularly to small unvaccinated infants. the authors underline the importance of recognizing pertussis, so that even in its atypical presentation, one can suspect, treat, report and prevent a disease that is not, and is not expected to be, eradicated any time soon. new vaccination strategies are necessary, to avoid this disease, which can be fatal to the most vulnerable.
Collective prenatal consultation: a new proposal for comprehensive health care

Abstract: this article describes the collective prenatal consultation as a new healthcare methodology, which is performed according to government standards, but collectively. relaxation and sensitization techniques are used, as well as group dynamics, including a collective exam of the pregnant women. the collective consultation is carried out in a welcoming environment, which provides clarification and socialization of experiences and information, centered on those women. the healthcare professional records every obstetric parameter and behavior in the patient's prenatal card and history file. priority is given to the principle of integrality and citizenship, with the aim to break the biomedical care paradigm, thus favoring humanized and comprehensive care to the women.
Violência vivenciada pelas adolescentes em situa o de rua na ótica dos profissionais cuidadores do abrigo

Abstract: Diante da relevancia e particularidade da violência na restri o ao exercício da cidadania das adolescentes em situa o de ruae na vulnerabiliza o à sua saúde, o objetivo do estudo foi compreender a problemática da violência vivida pelas mesmas naótica dos profissionais cuidadores de abrigo. Trata-se de pesquisa qualitativa do tipo descritivo, cujos dados foram produzidosatravés de entrevistas junto aos profissionais cuidadores de um abrigo para adolescentes da rede municipal do Rio de Janeiro-RJ em 2008. Os depoimentos foram interpretados à luz da análise de conteúdo, resultando em quatro categorias que explicitam a vis o dos profissionais acerca dos tipos de violência vividos pelas adolescentes: violência coletiva, violência juvenil, violência familiar e violência de gênero. Concluindo, os profissionais cuidadores do abrigo compreendem a problemática da violência vivida pelas adolescentes em situa o de rua de forma ampliada e apresentam algumas interrela es e concomitancia dos vários tipos de violência, configurando a complexa trama da violência que atravessamarcadamente as vidas dessas adolescentes.

Abstract: INTRODU O: A gastrostomia endoscópica percutanea (PEG) é preferencial para nutri o entérica (NE) prolongada em pediatria. Contudo, persiste o preconceito que a coloca o e NE por PEG em crian as deverá reservar-se às unidades de gas-trenterologia/endoscopia pediátrica. Tal parece-nos contrário às necessidades da realidade portuguesa. OBJECTIVO: Caracterizar retrospectivamente uma popula o pediátrica submetida a NE por gastrostomia, acompanhada por equipa que assiste adultos e crian as, incluindo sucesso e seguran a da técnica, evolu o ponderal e sobrevida. MéTODOS: Seleccionaram-se doentes desde 1 mês a 16 anos (média:4,17 mediana:3). Avaliaram-se dados demográficos, patologia subjacente, sucesso técnico, complica es, evo-lu o clínica e ponderal. Comparou-se a sobrevida entre crian as com les es estáveis e com doen as progressivas, utilizando-se o SPSS. Resultados: Nas 40 crian as referenciadas foi possível colocar a PEG. Ocorreram raras complica es major, resolvidas sem cirurgia. A evolu o ponderal foi favorável nos primeiros 6 meses. Crian as com les es estáveis têm sobrevida mais longa que com doen as progressivas (p=0.002). CONCLUS ES: A coloca o de PEG foi bem sucedida e segura. A NE resultou em evolu o pon-deral favorável. A sobrevida relacionou-se com a natureza da les o subjacente. Equipas de NE que assistem adultos devem assumir com naturalidade os doentes pediátricos, em colabora o com pediatras e outros profissionais envolvidos na nutri o infantil. Abstract: reactivation of infection with hepatitis b virus (hbv) is a potentially serious complication of immunosuppression, which should be identified and efficiently prevented. there have been an increasing number of cases of hbv reactivation in patients receiving immunosuppression in the context of rheumatic diseases such as rheumatoid arthritis or systemic lupus erythematosus. the recommendations in this area should be individualized taking into account two aspects: immunosuppressive regimens used (high or low risk of reactivation) and the different stages of hbv infection: chronic hepatitis b, inactive hbv carrier, occult hepatitis b infection defined by hb surface antigen (hbsag) negative and antibody anti-hb core (anti-hbc) positive. in patients with rheumatic diseases that will start high risk immunosuppressive drugs, we propose a universal screening with serological tests for hepatitis b (hbsag, anti-hbs and anti-hbc). patients with chronic hepatitis b (hbsag positive, hbv dna l 2000 iu / ml) should initiate antiviral therapy. inactive hbv carriers (hbsag positive, hbv dna < 2000 iu / ml, normal aminotransferases) exposed to high risk immunosuppressive therapy should undergo prophylaxis of hbv reactivation. prophylaxis should be started 2 to 4 weeks before the beginning of immunosuppressive therapy and maintained for at least 6 to 12 months after its suspension. it is recommended to use entecavir or tenofovir as first line antiviral agents. in inactive hbsag carriers under low-risk immunosuppressive therapy and patients with hbsag negative / anti-hbc positive (hbv infection in the past or occult inspection), the strategy should be monitoring of viral reactivation with aminotransferases and hbv dna determination in every 6 months.

Abstract: the current demographic circumstances and its consequences in the social system, from family relationships to the functioning of the macro structures, as well as health care, makes it necessary to reconsider the elder's role in the portuguese context. to promote life quality when physical decline is inevitable implies considering multidimensional criteria, such as maintaining the social network and significant activities. the productive aging theory emphasizes both the contributions that elders can give to society, through the goods they produce, as well as the well-being and life quality that come from this process (kaye, butter & webster, 2003). however, in portugal there is still no planning for the implementation of strategies that can direct and promote the use of the resources that elders can give, which is something that needs to be modified.
