Lipólisis láser y liposucción en ginecomastia: retracción cutánea eficaz y segura Laser-assisted lipolysis for gynecomastia: safe and effective skin retraction

Abstract: OBJETIVO: Valorar la eficacia de la lipólisis-láser para corregir el volumen, flacidez y excedente cutáneo sin escisión. MéTODOS: Prospectivamente, en 32 pacientes con ginecomastia, bajo anestesia tumescente y sedación se realizó lipólisis con láser de diodo 980nm, 15W en emisión continua, 8kJ a 12kJ de energía por mama. Externamente se utilizó aire frío para protección de la piel. Después se utilizó lipoaspiración convencional. No se emplearon drenajes pero si vendaje compresivo. Los pacientes evaluaron los resultados en una escala visual analógica. Dos médicos evaluaron los resultados por fotografías de antes y seis meses después, y midieron las areolas y contorno torácico. RESULTADOS: Veintitrés pacientes consideraron los resultados como Muy Bueno, siete Bueno y dos Regular. La retracción cutánea en la areola fue notable un mes después de la intervención y fue máxima a los seis meses. La valoración de los médicos fue 26 Muy Bueno, cinco Bueno y un Regular. No existieron quemaduras, isquemia, ni lesiones en areolas o pezón. CONCLUSIóN: La liposucción asistida por láser es eficaz y de ejecución sencilla, poco traumática y permite una temprana reintegración a las actividades. OBJECTIVES: To evaluate efficacy of laser lipolysis in the treatment of gynecomastia to correct breast volume, flaccidity and excess skin without its excision. METHODS: Prospectively, 32 patients with gynecomastia under tumescent anaesthesia and sedation underwent laser lipolysis with 980nm diode laser, 15W continuous emission and 8 to 12 kJ energy per breast. Externally cold air was used to protect the skin. No drainages were used but a compressive bandage. Patients evaluated results on a VAS scale. Two doctors evaluated results comparing before and 6 month after photographs and also measured the areola and chest diameter. RESULTS: Twenty three patients considered results as Very Good, 7 Good and 2 Fair Cutaneous retraction of the areola was noticeable one month after the surgery and was maximum 6 months after. Evaluation by doctors was 26 Very Good, 5 Good and 1 Fair. There were no burns, ischemia or lesions in areolas or nipples. CONCLUSION: Laser assisted liposuction is a simple and efficacious technique, barely traumatic and permits a rapid reincorporation to normal activitie.

Abstract: This paper introduces a natural extension of Kolchin's differential Galois theory to positive characteristic iterative differential fields, generalizing to the non-linear case the iterative Picard-Vessiot theory recently developed by Matzat and van der Put. We use the methods and framework provided by the model theory of iterative differential fields. We offer a definition of strongly normal extension of iterative differential fields, and then prove that these extensions have good Galois theory and that a G-primitive element theorem holds. In addition, making use of the basic theory of arc spaces of algebraic groups, we define iterative logarithmic equations, finally proving that our strongly normal extensions are Galois extensions for these equations.
Theoretical and Conceptual Approaches to Co-Regulation: A Theoretical Review  [PDF]

Abstract: During the last two decades, interpersonal regulation in natural and digital learning environments has gained importance. Ever since the first conceptual and methodo-logical precisions regarding collaborative learning were made, educational psychology has focused its interest on analyzing collective regulation of motivation, cognition, and behavior. Despite the fact that the body of research on co-regulation has grown, emerging epistemological frameworks evidence a lack of conceptual and theoretical clarity. In response to this situation, the authors propose a conceptual approach in order to address interpersonal regulation in four aspects: first, they describe three learning theories which have been used to study co-regulation. Second, the authors recommend a conceptual delimitation of terms regarding the learning theories on social regulation. Third, they highlight diffuse boundaries between theoretical ap-proaches and terms used in the literature on co-regulation. Finally, the authors suggest some challenges the researchers in this field face.
Morbilidad postoperatoria en pacientes con hidatidosis hepática no complicada: Utilización de una propuesta de clasificación de complicaciones

Abstract: background: surgery for hepatic hydatidosis is common in southern chile and it is important to know the rates of complications of the procedure. aim: to report the rates of postoperative complications in patients operated for uncomplicated hepatic hydatidosis. material and methods: prospective study of patients operated for uncomplicated hepatic hydatidosis between 1997 and 2008. the surgical techniques employed were total or partial peri cystectomies, or hepatic resection. results: one hundred sixteen patients aged 43 ± 17 years, 60% females, were included in the series. median cyst diameter was 15 cm. ten patients (8.6%) had complications. three patients had an atelectasis, two had an acute bronchitis, one had a urinary tract infection, two had surgical wound infections, one developed a residual cavity and one had an eventration. during a median follow up of 95 months, no patient died and one had a recurrence of the disease. conclusions: surgery for uncomplicated hydatidosis has a low rate of complications.

Abstract: hydatidosis is common in southern chile and it is usually located in the liver. the complications of surgical procedures for liver hydatidosis, which fluctuate from 11 to 86%, are superposed to the own complications of the disease. the frequency of surgical complications is associated with evolutionary complications of the cysts, the need to perform additional procedures to treat the disease in other simultaneous locations and previous surgical interventions for the disease. operative mortality is lower than 5%. the surgical treatment of hydatidosis is divided in four phases: isolation of the surgical zone, cyst evacuation, treatment of cyst complications and treatment of residual cavity. the conservative surgical procedures are marsupialization, cystostomy, posadas intervention and cystojejunostomy. surgical procedures involving resection are pericys-tectomy, subtotal cystectomy and liver resections. the role of laparoscopic surgery is still under evaluation.

Abstract: Introducción: La cirugía de la hidatidosis hepática (HH) es habitual en hospitales del sur de Chile. El número de publicaciones relacionadas es escasa y los resultados disímiles. Hablar de morbilidad postoperatoria es complejo debido a la diversidad en la gravedad e implicancias para el paciente; basado en ello, Clavien publicó una clasificación basada en la gravedad de la morbilidad postoperatoria. El objetivo de este estudio es reportar la morbilidad postoperatoria en pacientes intervenidos por HH no complicada aplicando la clasificación de Clavien. Material y Método: Estudio de cohorte prospectiva. Se incluyeron pacientes intervenidos quirúrgicamente por HH no complicada, en el Hospital Hernán Henríquez Aravena de Temuco, entre 1997 y 2008. La variable resultado principal fue desarrollo de morbilidad postoperatoria aplicando la escala de Clavien. Otras variables de interés fueron estancia hospitalaria, mortalidad y recurrencia. Las técnicas quirúrgicas empleadas fueron periquistectomía total o subtotal y resección hepática. Se calcularon porcentajes, medidas de tendencia central y dispersión; intervalos de confianza de 95% e incidencia. Resultados: 116 pacientes, con mediana de edad de 40,5 a os; 60,3% de género femenino. La mediana del diámetro ultrasonográfico de los quistes fue de 15,0 cm. El tiempo quirúrgico promedio fue de 115,4 ± 23,2 min. La incidencia de morbilidad fue de 8,6%; 90% de los cuales eran Grado I o II de Clavien. La etiología fue 5,2% de complicaciones médicas y 3,4% de complicaciones quirúrgicas. No se registró mortalidad. Con una mediana de seguimiento de 95 meses, se verificó recurrencia en 1 paciente, lo que representa una incidencia de 0,9%. Conclusiones: Se describe la morbilidad postoperatoria de una cohorte de pacientes con HH no complicada con énfasis en la gravedad de esta. La morbilidad observada es inferior a la publicada y de bajo nivel de gravedad. Abstract: La hidatidosis es una zoonosis endémica en el sur de Chile, por lo que tenemos la oportunidad de tratar un número importante de pacientes, especialmente hepática, localización más común de esta enfermedad. La hidatidosis hepática (HH) tiene morbilidad y mortalidad propia debido a sus complicaciones evolutivas, a lo que se agrega el riesgo de complicaciones asociados a los procedimientos quirúrgicos, cuya morbilidad se ha reportado entre 11% y 86%; la que se ha relacionado con intervenciones previas por HH, complicaciones evolutivas de los quistes, la necesidad de realizar procedimientos adicionales como el tratamiento de la enfermedad en otras localizaciones simultáneas. La mortalidad reportada es inferior al 5%. Sin embargo, tanto morbilidad como mortalidad persisten altas a pesar de los avances tecnológicos terapéuticos. El tratamiento quirúrgico de la HH se puede dividir en cuatro fases: aislamiento de la zona quirúrgica, evacuación del quiste, tratamiento de las complicaciones del quiste; y tratamiento de la cavidad residual. Por otro lado, la cirugía de la HH se puede clasificar en procedimientos conservadores (marsupialización, quistostomía, operación de Posadas y quistoyeyunostomía) y resectivos (periquistectomía, quistectomía subtotal y resecciones hepáticas). Overexpression of the Trichoderma brevicompactum tri5 Gene: Effect on the Expression of the Trichodermin Biosynthetic Genes and on Tomato Seedlings

Abstract: Trichoderma brevicompactum IBT 40841 produces trichodermin, a trichothecene-type toxin that shares most of the steps of its biosynthesis with harzianum A, another trichothecene produced by several Trichoderma species. The first specific step in the trichothecene biosynthesis is carried out by a terpene cylcase, trichodiene synthase, that catalyzes the conversion of farnesyl pyrophosphate to trichodiene and that is encoded by the tri5 gene. Overexpression of tri5 resulted in increased levels of trichodermin production, but also in an increase in tyrosol and hydroxytyrosol production, two antioxidant compounds that may play a regulatory role in trichothecene biosynthesis, and also in a higher expression of three trichothecene genes, tri4, tri6 and tri10, and of the erg1 gene, which participates in the synthesis of triterpenes. The effect of tri5 overexpression on tomato seedling disease response was also studied.

Abstract: this essay looks at the extensive academic literature on the subject of boss rule in restoration spain (1875-1923), often regarded as a key phenomenon in explaining spanish electoral behaviour and the development of the spanish political system in the liberal era. it begins by showing how the question of boss rule is a core element in modern political analysis and commentary, and in the different interpretations to be found in its early historiography. the main purpose of the essay is to put forward a revision of the main historiographic trends in the study of boss rule - here called the ?new political history? and ?agrarian social history? - particularly in relation to their opposing conceptions of boss rule as a specific form of clientelism, their descriptions of political elites, and the ties between political and economic powers. these trends share a common language and common conclusions which make it possible to compare them with similar phenomena in other countries. finally, possible future lines of research are suggested. rui ramos - oligarchy and boss rule in oliveira martins, joaquín.
