Evaluation of myocardial relaxation in conditions of cardiac remodeling

Abstract: The term cardiodynamics refers to dynamic events associated with cardiac contraction and relaxation. The occurring wave of excitement spreads very quickly along the entire atrial musculature and after a brief AV retention it affects all muscle cells of the ventricles. Excitation, that is, the increase in action potentials, precedes the contraction of the myocardium, which follows the 'all or none' rule. Each contraction results in relaxation of the myocardium, so that the contraction and relaxation cycles continually follow each other in succession. The entire cardiodynamics, hemodinamics, i.e. signaling mechanisms of the heart are altered in the remodeling (alternation) condition of the left ventricular myocardium, i.e. the musculature and the whole arterial wall. Remodeling of the cardiac wall and layers of the arterial wall is a negative factor, because it leads to disturbances of the cardiac contraction and relaxation cycles and incites progression of the arterial hypertension, emergence of atherosclerosis and arterial stenosis. Today, the genetic base of the cardiac remodeling is the object of intensive studies. Cardiomyopathies are primary disorders of the myocardium associated with abnormalities of the cardiac wall thickness, the size of chambers, contractions, relaxations, signal conduct and rhythm. They are the major cause of morbidity and mortality for all age groups. Mechanisms of these events on the molecular level will be discussed in the following study.
Oxidative status evaluation in elite karate athletes during training process

Abstract: Background/Aim. Oxidative stress is a state of disturbed balance between reactive oxygen species (ROS) and reactive nitrogen species (RNS) on the one hand, and antioxidative defense on the other. As a result of oxidative stress we have irreversible changes in cell function leading to pathological tissue changes, development of many diseases and fast aging process. Increased consumption of oxygen during exercise could be the cause of oxidative stress. The aim of the study was to establish oxidative status of elite karate athletes in the state of rest and after the loading, monitoring the parameters of oxidative stress and components of antioxidative defense in a treaining process. Methods. During training process, a group of 30 elite karate athletes was included in the study of prevalence. They were males, from 16 to 30 years of age. The examination was conducted both in resting condition and after the loading. Oxidative status was determined by the level of superoxide anion radical (O2-), hydrogen peroxide (H2O2) as prooxidants, and enzymes of the antioxidant defanse system: superoxide dismutase (SOD) and catalase (CAT). Results. There was a homogenous group of young athletes of normal weight and fats percentage. A significant decrease of O·2- after the load was founded, while the levels of H2O2 was significantly increased. Activity of SOD was not significantly changed after exsercise while that of CAT was significantly increased. Conclusion. In the long run a programmed physical exercise does not lead to oxidative stress, but an excessive physical load may cause its occurrence.
Marginal seal evaluation of adhesive amalgam restaurations

Abstract: Dental amalgam is still the material most commonly used to restore posterior teeth in many countries. Amalgam is very popular because of its strength, longevity, low cost, and relative ease of handling compared to other direct restorative materials. Two important disadvantages of amalgam restaurations are lack of adhesion to tooth structure and marginal leakage. Introduction of dentin adhesive systems enabled better marginal seal between amalgam and tooth structure. Aim: The aim of this study was to estimate, in vitro conditions, the marginal seal of amalgam restaurations with dentin adhesive systems using the method of diffusion dye. Material and methods: In this study 50 extracted human teeth were used. Class II cavities were prepared on all the teeth. The teeth were divided in five groups and materials applied according to manufacturer’s instructions: Gamabond, Optibond Solo, OptiGuard, Ionosit - Base Liner and control group. All the teeth were thermocycled 200 times, at temperatures: 5 - 7oC, 37oC and 57 - 59oC. Method of diffusion dye was used to estimate the quality of marginal seal of amalgam restaurations. All teeth were immersed into 50% solution of silver nitrate, and then into 10% developing solution. All teeth were cut with a diamond disc in the meso - distal plane. Penetration dye was estimated using binocular magnifying glass that magnifies 25 times, with inserted micrometar scale. Results were statistically analysed with Student t - test. Results: The least penetration dye was recorded in the group of amalgam restaurations rebonded with OptiGuard. Higher penetration dye was noted in the groups of adhesive amalgam restaurations, where Gamabond showed lower penetration dye in comparison with Optibond Solo. Penetration dye in the group Ionosit - Base Liner was higher than in the groups where dentin adhesive systems were used: on the occlusal wall, penetration dye in comparison with Gamabond showed statistically significant difference, (p < 0,5) while penetration dye in comparison with Optibond Solo showed no statistically significant difference. On the gingival wall, penetration dye in comparison with dentin adhesive systems - Gamabond and Optibond Solo, showed a statistically significant difference, (p < 0,01). The highest penetration dye was in the control group. Conclusion: The best marginal seal of amalgam restaurations was achieved with OptiGuard. Use of dentin adhesive systems also provided good marginal seal, with better results from Gamabond compared to Optibond Solo. Using Ionosit - Base Liner good marginal seal on the occlusal wall
Condition of hearing sense and tinnitus before and after the treatment of otosclerosis

Abstract: Introduction. Otosclerosis is an osteodystrophy disease of the bony labyrinth of the ear which leads to stapes fixation and to bradyecoia of a conductive type. The microsurgical treatment by the use of a stapes prosthesis provides hearing improvement and a reduction or disappearing of tinnitus. The aim of our study was to determine the hearing condition and tinnitus before and after the treatment of otosclerosis. Material and methods. A hundred patients with otosclerosis, aged 20 to 70, were surgically treated at the Otorhinolaryngology Clinic and the Functional Diagnostics, Military Medical Academy, Belgrade. After the treatment, the audiometric examinations were performed in order to assess the hearing condition and tinnitus was assessed by a questionnaire. The subjective assessment of tinnitus was ranged as present, reduced or absent. Results. Before the treatment, hearing impairment was present in all the patients, while tinnitus was found in 91% of them. One year following the surgical treatment hearing improvement was found in 96% of the patients, and tinnitus cessation in 89% of them. Conclusion. The surgical treatment can result in hearing improvement and cessation or reduction of tinnitus.
Recurrent respiratory infections with severely damaged lung functions: Kartagener's syndrome: Case report

Abstract: Introduction Kartagener's (or Siewert) syndrome represents a clinical presentation of primary ciliary diskinesia (PCD) with 3 dominant symptoms: bronchiectasiae, chronic sinusitis and situs viscerum inversus totalis or dextrocardia, only. Siewet demonstrated the first case of this clinical phenomenon in 1904, which was recognized by Kartagener in 1933, as a congenital disorder. PCD is an autosomal recessive disease with extensive genetic heterogenity. Dyskinetic or completely absent motility of cilia predisposes to reccurent pulmonary and upper respiratory tract infections resulting in bronchiestasis. Also, infections of the middle ear are common due to lack of ciliary movement in the Eustachian tube. Men have reduced fertility due to spermatozoa with absent motility or abnormalities in the ductuli afferents. Case report Our case report represents a middle age woman, treated ambulatory for many years as a chronic obstructive pulmonary disease (COPD). The patient noted recurrent upper respiratory tract infections from her childhood, headaches and recurrent sinusitis. Also, she had extrauterine pregnancy, no children, in spite of several arteficial inseminations. Due to the clinical signs of the right heart failure and the described ECG changes, the echocardiographic examination was done, which showed the dextracardia and the ultrasonographic examination of the abdomen revealed the situs inversus, the finding being within the physiological limits. Conclusion All data regarding situs viscerum invesrus, supported by CT verification of bronchiectasiae and chronic inflammation of maxillar sinus led us to conclude that the patient suffered from Kartagener's syndrome.
The use of combined hyperbaric oxygenation and erythropoietin in the treatment of cardiac insufficiency

Abstract: Introduction The aim of this research is to EVALUATE therapy efficiacy of hyperbaric oxygenation (HBO) with regenerative effects on human tissues and erythropoietin, as a nonspecific growth factor. Material and methods This study included a group of 9 patients with echocardiographically diagnosed cardiac insufficiency with ejection fraction (EF%) under 60%. HBO was used according to the protocol for patients with severe cardiac insufficiency (a total of 15 treatments). All patients also received subcutaneous erythropoietin, 2000 iv, every other day. Control echocardiography was done 30-45 days after they finished their treatments. Results The average ejection fraction before treatment was 46%. After treatment, the average EF% was 57%, so it increased by 11%. All patients felt subjectively better, with improved capacity to physical efforts. Discussion and Conclusion Ejection fraction of 30-40% is an indication for hyperbaric oxygenation therapy. The decision about the treatment is made by the physician based on his experience, general condition of the patient, frequency and severity of hypoxic episodes. If EF% is 30% or below, HBO is not recommended, because antioxidative defense mechanisms are exhausted under hypoxia and the balance of the organism should not be changed. An increase in average EF% by 11% demonstrates that combined use of HBO and Erythropoietin gives good results.
Supraselective embolization as a method for the management of renal vascular injuries

Abstract: Background: Renal injuries are most often caused by traumatic injuries, but they can also be induced iatrogenically, during renal biopsy, percutaneous nephrostomy or lithotripsy. Vascular renal injuries may be treated by embolization, nonsurgical or surgical methods. Case report. In this paper we presented a high-risk patient with poor outcome of an open surgery threatment and a prior attempt of embolization, with gross haematuria caused by traumatic renal arterio-venous (AV) fistula and pyelocalical injury who was managed by supraselective embolization of the irrigating arterial vessel. Conclusion. Supraselective embolization is a first choice method for the treatment of low grade renal vessel injuries. Even patients with the most severe renovascular lesions and major renal destruction (a grade IV injury) can be treated nonsurgically with supraselective embolization, with an excellent chance to preserve the maximal amount of functional renal parenchyma. This method is rapid, effective, tissue preserving, and likely to reduce morbidity and mortality.
Comparative analysis of audiometric and impedancemetric findings in workers exposed to the effects of various duration industrial noise

Abstract: Background/Aim. Industrial noise is produced in workshops due to factory machinery and tools used in technological processes. It has detrimental effect to primarily the organ of the sense of hearing, thus damaging hearing in oversensitive subjects. The aim of the study was to perform a detailed analysis of audiometric and, especially, impedancemetric parameters in workers exposed to the effects of industrial noise of various duration, as well as to determine similarities and differences in workers with differences in sensitivity to damaging effects of noise. Methods. The study included a homogenous group of 173 industrial workers involved in the same department of a huge workshop named 'Kova ica' exposed to noise level above a threshold intensity, and divided into three groups: the group I of 116 workers (232 ears) exposed to noise for 8 h in the workshop 'Kova ica', the group II of 41 workers (82 ears) exposed to noise only half of the working time, and the group III of 16 workers (32 ears) exposed to the same conditions and having the normal hearing sense. The group III served as a control group. Prior to the study any workers went through a thorough anamnesis and complete ORL examination, and then they were submitted to audiometric testing. The study included only the workers with no hearing damage due to diseases, injuries nor other detrimental factors in order to be sure that the hearing findings had been caused by industrial noise. Results. The results obtained by audiometric testing showed that 90.75% of the workers had hearing damage of various degrees, while 9.25 % of the workers had regular hearing although had been exposed to the same conditions. More severe hearing damage was revealed in the workers of the group I. Tympanometricly, in most ears of the group I workers (65.52%) a compliance value was found to be more than 0.9 cm3, while in the majority of those of the group II (59.75%) a compliance value was in the range from 0.5 to 0.9 cm3. In the workers of the group III with no hearing damage, a compliance value for any ears was in the range from 0.1 to 0.9 cm3. Mean value of threshold reflex (98.56 dB) at 500 Hz and 1 000 Hz in the group I was increased as compared with the group II (95.6 dB) and the group III (84.38 dB). At higher frequencies, 2 000 and 4 000 Hz, threshold reflex was increased in the group I (99.05 dB), and in the group II (97.6 dB) as compared with the group III (86.4 dB). Neurons of human nucleus accumbens

Abstract: Background/Aim. Nucleus accumbens is a part of the ventral striatum also known as a drug active brain region, especially related with drug addiction. The aim of the study was to investigate the Golgi morphology of the nucleus accumbens neurons. Methods. The study was performed on the frontal and sagittal sections of 15 human brains by the Golgi Kopsch method. We classified neurons in the human nucleus accumbens according to their morphology and size into four types: type I - fusiform neurons; type II - fusiform neurons with lateral dendrite, arising from a part of the cell body; type III - pyramidal-like neuron; type IV - multipolar neuron. The medium spiny neurons, which are mostly noted regarding to the drug addictive conditions of the brain, correspond to the type IV - multipolar neurons. Results. Two regions of human nucleus accumbens could be clearly recognized on Nissl and Golgi preparations each containing different predominant neuronal types. Central part of nucleus accumbens, core region, has a low density of impregnated neurons with predominant type III, pyramidal-like neurons, with spines on secondary branches and rare type IV, multipolar neurons. Contrary to the core, peripheral region, shell of nucleus, has a high density of impregnated neurons predominantly contained of type I and type IV - multipolar neurons, which all are rich in spines on secondary and tertiary dendritic branches. Conclusion. Our results indicate great morphological variability of human nucleus accumbens neurons. This requires further investigations and clarifying clinical significance of this important brain region.
Kumulativisti ko nasle e i njegove aktuelne forme - prilog razumevanju kumulativisti kog na ina vi enja nau nih razvoja

Abstract: (nema ki) Obwohl er wohl den ltesten Standpunkt hinsichtlich der Interpretation wissenschaftlicher Entwicklungen darstellt, wird der Kumulativismus merkwürdigerweise bis in die gegenw rtige Diskussion hinein meistens unzureichend klar bestimmt. In dieser Folge wird seine Behandlung im Rahmen moderner Wissenschaftsheorie von einigen Miβverst ndnissen und groben Vereinfachungen begleitet. Die vorliegende Abhandlung bezweckt eine Er rterung konzeptioneller Eigentümlichkeiten sowie Kapazit ten der kummulativistischen Auslegung wissenschaftlicher Entwicklungen. Der Verfasser geht hierbei insbesondere auf Kritiken von K.R. Popper ein, die inzwischen die Schule gemacht haben. Diesen entgegen, wird nachgewiesen, dass die kummulativistische Deutung wissenschaftlicher Entwicklungen an keine induktivistische Methodologie der Theorienbildung gekoppelt zu werden braucht. Es erweist sich, dass in der wissenschaftstheoretischen Interpretation wissenschaftlicher Entwicklungen von zwei Gestalten des Kumulativismus die Rede sein kann: vom Whewellschen rigiden umfassenden Kumulativismus einerseits, der nur sehr eingeschr nkt auf die tats chliche Wissenschaftsgeschichte anwendbar ist, von einem partiellen und flexiblen Kumulativismus, der praktisch mit dem Popperschen evolutionistischen Modell koinzidiert und der breiter und vollst ndiger anwendbar ist. Die von Popper eingeleitete evolutionistische Interpretation der Wissenschaftsgeschichte zeigt sich als die am besten haltbare Form, in der die Idee einer Akumulation wissenschaftlichen Wissens im Rahmen wissenschaftlicher Entwicklungen heute vertretbar ist.
