Eficacia del uso de bioconectores para los catéteres centrales de hemodiálisis Efficacy of the use of bioconnectors for central catheters in haemodialysis

Abstract: Los catéteres tunelizados (CT) constituyen el acceso vascular de hemodiálisis (HD) en un porcentaje elevadísimo de pacientes. La utilización de CT se asocia a dos problemas fundamentales: disfunción e infecciones. Según las guías de la Sociedad Espa ola de Nefrología (SEN), en los pacientes en HD se aconseja el empleo de tapones de un solo uso con rosca de seguridad (luer-lock). En el mercado existen también bioconectores, que son dispositivos mecánica y microbiológicamente cerrados. La ventaja que ofrecen es la menor manipulación de la luz del catéter al cambiarse una vez por semana. Se realizó un estudio en nuestro centro analizando en una muestra de 14 pacientes portadores de CT, dos periodos consecutivos, un primer periodo siguiendo el protocolo de conexión convencional y el segundo utilizando los bioconectores TEGO. Se analizaron presiones arterial y venosa, flujos sanguíneos, Kt, Kt/v y tasa de bacteriemia. No se obtuvieron diferencias significativas en estos parámetros con el uso de dispositivos TEGO frente al uso convencional. Podemos concluir que el uso de tapones TEGO no afecta a los parámetros dialíticos. No obstante, debemos considerar el manejo de cualquier tipo de dispositivo como secundario en la prevención de bacteriemias, ya que se ha demostrado que son las medidas universales de asepsia las que logran una tasa de bacteriemias muy baja. The use of TC is associated to two fundamental problems: dysfunction and infections. According to the guidelines established by the Spanish Nephrology Society (SEN), the use of luer-lock catheters is recommended in HD patients. On the market there are also bioconnectors, which are mechanically and microbiologically sealed devices. The advantage they offer is better handling of the catheter lumen when changed once a week. A study was carried out at our centre analysing two consecutive periods in a sample of 14 patients with CT. In the first period the conventional connection protocol was followed and in the second TEGO bioconnectors were used. Arterial and venous blood pressure was measured, blood flows, Kt, Kt/v and bacteraemia rate. No significant differences were obtained in these parameters with the use of TEGO devices compared to conventional use. We may conclude that the use of TEGO locks does not affect the dialytic parameters. Abstract: Es imprescindible un acceso vascular adecuado para obtener una hemodiálisis óptima. El da o causado por la punción de la aguja convencional o complicaciones intradiálisis derivadas del movimiento del brazo pueden comprometer la supervivencia del acceso. Otro factor esencial en la calidad de la hemodiálisis, íntimamente relacionado con el calibre de la aguja, es conseguir un mayor flujo sanguíneo eficaz. El catéter-fístula, una nueva cánula de punción de fluoruroplástico, disminuye el da o en la pared interna del acceso y presenta un menor calibre de punción con un mayor calibre interior, lo que le permite alcanzar mayores flujos con una máxima eficacia depuradora y menores complicaciones vasculares. El objetivo de este estudio prospectivo fue comparar el uso del catéter-fístula con el uso de la aguja convencional. Se realizaron 18 sesiones de hemodiálisis con cada uno de los dispositivos en pacientes estables y con buen acceso vascular, recogiéndose en todos los casos el flujo obtenido, las presiones y otros parámetros del monitor, analíticas e incidencias. Los resultados han demostrado que la utilización del catéter-fístula disminuyó la presión arterial y la presión venosa, sin disminuir la eficacia dialítica, en consecuencia, mejoró el perfil hemodinámico. Mejoró significativamente el flujo de sangre real, el número de litros dializados y el Kt. El menor trauma mecánico producido en el acceso vascular permitió disminuir la dosis de heparina, el tiempo de hemostasia y hubo una reducción de las complicaciones vasculares: extravasación y resangrado. Siendo bien tolerada la punción, hay un leve empeoramiento en la sensación dolorosa, justificada por la inexperiencia del manejo. La mejoría en los parámetros dinámicos abren las posibilidades de conseguir mayor eficacia dialítica al hacer factible la utilización de mayores flujos sanguíneos con presiones más adecuadas. Abstract: adequate vascular access is essential in order to obtain optimal haemodialysis. the damage caused by conventional needle puncture or intradialytic complications deriving from the movement of the arm can compromise the survival of the access. another essential factor in the quality of the haemodialysis, closely related to the calibre of the needle, is obtaining a greater efficient blood flow. the fistula catheter, a new puncture cannula made of fluoroplastic, reduces the damage to the internal wall of the access and presents a lower puncture calibre with a higher interior calibre, which allows it to reach better flows with maximum efficiency and fewer vascular complications. the aim of this prospective study was to compare the use of the fistula catheter with the use of the needle. eighteen haemodialysis sessions were carried out with each of the devices in stable patients with good vascular access, recording in all cases the flow obtained, pressures and other monitor parameters, test results and incidents. the results have shown that the use of the fistula catheter reduced the arterial pressure and venous pressure, without reducing the dialytic efficiency, and consequently the haemodynamic profile improved. the actual blood flow, number of litres dialysed and kt improved significantly. the lesser mechanical trauma caused in the vascular access made it possible to reduce the dose of heparin, the haemostasis time and there was a reduction in vascular complications: extravasation and rebleeding. while the puncture is well tolerated, there is a slight worsening in the feeling of pain, explained by the lack of experience in handling. the improvement in dynamic parameters opens up the possibilities of achieving greater dialytic efficiency by making it feasible to use greater blood flows with more adequate pressures.
Food Sources and Intake of Calcium in a Representative Sample of Spanish Adults  [PDF]

Abstract: The present study aimed to assess calcium intake, dietary sources of this nutrient and its adequacy with respect to the dietary reference intake (DRI) in a representative sample of Spanish adults. In this study 418 adults (18 to 60 years) from 15 Spanish provinces were studied. Energy and nutrient intake was determined using a 24-hour recall questionnaire for two days. Adequacy of calcium intake was assessed using the established DRI for calcium. Anthropometric data (weight and height) were measured and the body mass index was calculated. Seventy eight percent of the participants in the study did not meet the DRI for calcium. Additionally, 33.7% of the participants did not meet the 67% of the DRI. The daily intake of calcium was 916.6 ± 288.1 mg/day, which represented the 81.3% of the DRI of calcium. Interestingly, subjects who had higher intake of calcium were taller. Additionally, it has been observed that individuals with normal body weight (BMI <25 kg/m2) had higher intakes of dairy products in comparison with overweight and obese individuals (BMI ≥ 25 kg/m2). The main food sources of calcium were dairy products (58.7% of calcium), cereals (13.6%) and vegetables (6.5%). Less than 1% (0.5%) of the calcium intake came from dietary supplements. It was observed that individuals who met the DRI for calcium had a significantly higher intake of dairy products (551.3 ± 240.4 g/day) than individuals who did not meet the DRI of calcium (305.0 ± 150.3 g/day). Calcium intake was inadequate in this sample of the adult Spanish population. Therefore, an increase in the consumption of dairy products, as well as cereals, vegetables and food items fortified with calcium seems to be necessary to achieve an adequate intake of calcium and to prevent diseases caused by calcium deficiency.

Situación de folatos en escolares espa oles y su asociación con la presencia del hábito tabáquico en los padres Folate status in spanish schoolchildren and its association with parental smoking habits

Abstract: Objetivo: Estudiar la situación de folatos de escolares espa oles y su asociación con la presencia del hábito tabáquico en los padres. Métodos: Se estudiaron 511 escolares entre 9 y 12 a os de Madrid (Espa a). La información sobre el hábito tabáquico de los padres fue autodeclarada. La ingesta de energía y de nutrientes (especialmente folatos) se calculó utilizando el método de registro de alimentos durante tres días (de domingo a martes) y en dos de ellos (lunes y martes) se procedió a controlar los alimentos consumidos en el comedor escolar, utilizando el método de pesada precisa individual (pesando alimentos servidos y restos dejados en el plato). La ingesta de folatos se comparó con la ingesta recomendada (IR). Se analizaron las concentraciones séricas de folatos, vitamina B12 y homocisteína. Para el análisis estadístico se utilizaron los test de t-student, Mann Whitney, correlación de Pearson y Spearman, ANCOVA, MANCOVA y regresión lineal múltiple. Se consideraron valores significativos de P < 0,05. Resultados: El consumo de verduras, hortalizas y de frutas, la ingesta de folatos y su contribución a las ingestas recomendadas (I/IR%) y las concentraciones séricas de folatos fueron mayores en los hijos de madres no fumadoras que en los hijos de madres fumadoras. El 13,3% de los escolares presentaron cifras de folatos séricos inferiores a las indicadoras de deficiencia moderada (6 ng/mL) y un 0,26% mostraron deficiencia severa (3 ng/mL). Teniendo en cuenta variables interferentes se observó una correlación negativa y significativa entre las concentraciones séricas de folatos de los escolares y el hábito tabáquico de la madre (r = -0,257, p = 0,01). Conclusión: La presencia del hábito tabáquico en la madre condiciona la ingesta dietética y las concentraciones séricas de folatos en sus descendientes. Aim: To study the folate status in a group of Spanish schoolchildren and its association with parental smoking habits. Methods: A group of 511 schoolchildren between 9 and 12 years old from Madrid (Spain) were studied. Parental smoking habits were self-reported by a questionnaire. Abstract: this article discusses the symbolic system developed by african captives and which still partially persists in colombia's pacific coast region. it introduces the concept of the homo-biosphere because its polymorphic production models, which integrate people with nature, promote environmental sustainability. the information on which the article is based comes from three bodies of knowledge: first, information collected over 15 years by the research project, "people of the baudó, co-existence and ecological polyphony"; second, data compiled for a temporary exposition in colombia's national museum entitled "wakes and living saints among black, afro-colombian, raizal and palenquero communities" (which ran from august 21 to november 2, 2008); and third, information gathered by a group of community councils from the baudó river valley (the colectivo territorial afrochocó) as part of a special report to colombia's ministry of the environment that sought to mitigate the negative cultural and environmental effects of building a new highway between the coffee-growing regions of caldas, risaralda and quindío and a new port on the pacific coast to be developed on the gulf of tribugá.
Caracterización de la cognición creativa en jóvenes con retraso escolar y deprivación social

Abstract: this actual research has as purpose to characterize the creative cognition over a children and young population in a certain situation which lacks of an educational level according to their due age and also lacks of some social coverage. the research is based on some theoretical arguments focusing on cognition, especially from "the genoplore model" and computable model. on the research a sample model was given to evaluate creativity in a group of children and teenagers between nine and seventeen years old within an educational level between primary and high school. they were randomly selected from several foster homes. we found through this research that this kind of population has some cognitional creativity with a normal distribution and a slightly asymmetry towards the left side, making plausible then central hypothesis of the cognitive and computable theories which assures that creativity is also distributed in all human subjects and not only in some exceptional ones, which indirectly indicates that creative cognition persists and can be found itself distributed without regarding the children and teenagers' vulnerability.
