Abstract: Nowadays, there are many opportunities for the society to live a healthful and long life. At the same time, never people was so sedentary and without harmony. Without a healthy body and with “an occupied mind” the human loses exactly what more it needs: the disposal to produce, to coexist and to live a good life. In this context, the present research aimed to revise some terms related to labor gymnastics, which is focused in the prevention of risks related to hours of working and in the reduction of muscular tension levels that may be originated during a day of work. Thus, the present study will make a differentiation between the use of stretching and flexionament during labor gymnastic sessions.

Abstract: the experiments were carried out to determine sodium requirements for light (ls) and egg-brown (ebs) pullets from 7 to 12 weeks of age. it was used a completely randomized design according to a 6x2 factorial (sodium levels: 0.04; 0.10; 0.16; 0.21; 0.27 and 0.32% x 2 strains), with five replicates of six pullets per experimental unit. feed intake, water intake, daily weight gain, feed: gain ratio, rectal temperature, and comb size were evaluated, as well as feces dry matter and humidity. in the two experiments, there was no interaction (p>0.05) between sodium levels (sl) and strains on the studied variables. feed intake was not affected by ls, but it was greater in ebs than in ls birds (p <0.05). ls had significant effects on daily weight gain and feed: gain ratio according to the quadratic model, and comb size was greater in ls compared to ebs birds. the increase in dietary sodium levels increased water intake and feces humidity, whereas feces dry matter decreased. rectal temperature was not affected by the treatments. for sl and ebs, it is recommended 0.22% na or a daily intake of 123 and 140 mg of sodium, respectively, from 7 to 12 weeks of age.
Optimization of the Cultivation of Donor Mesenchymal Stromal Cells for Clinical Use in Cellular Therapy  [PDF]

Abstract: Mesenchymal stromal cells(MSC)have shown their benefits in graft-versus-host disease(GVHD), with five unsettled matters: 1) MSCs expansion in medium with Fetal Bovine Serum(FBS)and its risk of xenoreaction; 2) The number of cells indicated for therapy that is relatively high, with the need to optimize the expansion, number and time wise; 3) The utilization of third party donors; 4) Culture passage number(P);and 5) Source of the cells. This study was designed to determine the superiority of the Platelet Lysates(PL)over FBS on the expansion of MSC, the optimal cell plating density and days between each pass, and to investigate if donor total nucleated cells(TNC)obtained from the washouts of discharged bags and filters of hematopoietic stem cell transplantation(HSCT)can be expanded to be used at clinical grade. TNC were removed, plated and after the first passage were cultivated in different concentrations with FBS or PL, and the number of days to reach 80% of confluence was observed. Next, cultures with the same plating density were fed either with PL or with FBS and after seven days counted to analyze how much they had grown in that period. The proliferation of mesenchymal stromal cells in the presence of PL and SFB was averaged 11.88 and 2.5 times, respectively, in a period of 7 days. The highest concentration of plating cells using PL took less time to reach confluence as compared with the three lower ones. This study suggests that the PL is the best choice as a supplement to expand MSC and to allow the proliferation of enough number of MSC at P2 for clinical use.

Mesenchymal stem cells (MSC) are considered non-hematopoietic multipotent stem cells with self-renewal properties and the ability to differentiate into a variety of mesenchymal tissues. Optimal conditions for the culture of these cells have been the subject of investigation for several years. In particular, ideal oxygen tension levels have not been established in the literature. In physiological environments, oxygen tension may vary from 12% in peripheral blood to 1% in the deep zone of cartilage regions. In any case, oxygen tension is considerably lower in vivo when compared with the normal atmosphere of standard cell culture conditions (21%). The objective of this study was to review the literature available on MSC characteristics (cell cycle, survival, proliferation, differentiation, morphology, immunophenotype, cytogenetics) when cultured under hypoxic conditions. Our focus on optimal culture conditions is justified by the key role currently played by these cells in regenerative medicine.

This paper proposes to think the psychoanalytical clinic of aging as moved by challenges brought about by the considerable increase in the number of elderly people nowadays. By questioning the role of psychoanalysis in culture and supported by clinic experience, we think about the psychoanalytical clinic of aging while including interdisciplinary contributions and interventions. This approach defends sublimation as a means of sustaining the wish and, above all, as a means of transformation provided by social bonds, aiming to put the subject as the leader of his own history and assuming that investment is what upholds existence. There is no age for desire. Through this principle, one must “bet on life” even when the body becomes “an arena for diseases”, always pointing towards death. Thus, psychoanalysis summons “the subject” to the hearing and remembering which enable his/her own histories and reconcile him/her with the legitimacy of his/her own wish.

Abstract: Background: Mesenchymal stromal cells (MSC) are being tested for the treatment of immune diseases. MSC are present in several adult tissues which milieu may influence MSC behavior particularly under inflammatory conditions. Additionally, culture conditions also can modify cell function or state of activation. Methods: To address the influence of the MSC source on its characteristics, we studied a xenofree, platelet lysate supplemented MSC from dental pulp, adipose tissue and bone marrow, co-cultured with isolated T cells and PBMC subset, and studied the effect of culture animal or human supplements immunomodulatory effect. Results: All three sources were efficient in inhibiting T cells. Among all MSC sources, as also described by others, adipose MSC was capable to significantly induce Treg phenotype and decrease T CD8+. Furthermore, comparing fetal bovine serum and platelet lysate, results demonstrate that platelet lysate alone is capable to induce immunomodulatory phenotype. Additional studies have to be made to elucidate the PL immunomodulatory effect.
Case reports versus literature reviews: indicators of change in the dominant model for sharing medical knowledge

Abstract: Aims: The crisis in the medical paradigm was seen at the end of the 20th century in the variability of practices, uncertainty of medical-decision making and difficulties in defining normality. Using changes in knowledge production as a marker, our aim was to characterize this paradigm crisis and to foresee its evolution. Methods: We reviewed Medline annually between 1976 and 2004, and counted the number of published case reports versus the number of literature reviews and clinical trial articles. Results : Our results show that of the articles indexed each year the proportion of literature review articles in 1994 was greater than case reports. This proportion is currently increasing. The proportion of case reports articles has fallen by half over the last twenty years. Clinical trial articles are increasing regularly and may exceed case reports in the next decade. Conclusions : The measurement of changes in the medical paradigm provided by our results show changes in the previous equilibrium: - the qualitative approach to disease is giving way to a quantitative approach; - knowledge development is moving towards a scientific process. From being inductive it is becoming deductive. This knowledge is changing and is stratifying through accumulation and review of primary knowledge from different teams; - research into etiologic certainties specific to the mechanistic paradigm is clashing with complexity, which is translated in the use of probabilities. Beyond large numbers, however, the clinical approach and human skills of the doctor remain fundamental prerequisites for care.
Integrated care management: the experience of Kaiser Permanente and the Veterans health administration in the USA

Abstract: With a review of the literature, we described the integrated care management experiences of Kaiser Permanente (KP) and the Veterans health administration (VHA) in the USA. These types of healthcare organisation (a non-profit HMO [health maintenance organization] and a public institution) enable cost control, compatible with a high quality of care. We analysed the innovations that they provide in terms of organisation and care practices and the principles to be retained in order to improve the efficiency of the French system. The KP and VHA results can be attributed to: - A practice founded on evidence based medicine and disease management, recently adapted to chronic condi-tions. In comparison with the NHS in the United Kingdom, KP has only a third of the amount of short-stays and hospital stay durations are 20% shorter. - A single management and the absence of a sealed financial envelope. - An organisation involving integration at all levels: joint administrative management with doctors, integration of an insurance system with a care system, outpatient care with hospital care, prevention with curative care (managed care and case management). This organisational consistency enables interdependent players to see the point and advantage in coordinating their practices and working together as a team. In France, integration is slow to be established as a necessity for healthcare players. However, active disease management of the 7.5 million patients with a long-term condition could bring a drive for change. As the Haut conseil pour l’avenir de l’assurance maladie (French high council for the future of health insurance) suggests, there is an opportunity to be seized by the insurance medicine system with the protocolisation of care for these patients.

Abstract: this article aims at analyzing an entrance exam which selected portuguese teachers in a small city in the countryside of minas gerais. the analysis' main objective is to observe the relations between the linguistic contents included in the exam and the linguistic perspectives on the teaching of writing. the study employs a methodological interface which attempts to combine the qualitative analysis of the exam to the quantitative data of the candidates' performance. the theoretical framework includes literacy studies and critical discourse analysis as a support for the discussion of the concepts of literacy, autonomous literacy and literacy event.
