Satisfacción laboral de la enfermería en las prisiones espa?olas

Abstract: background: there are no available studies assessing job satisfaction amongst nursing staff in spanish prisons. the aim of this study is to establish overall levels of job satisfaction and determine each of the components. method: cross-sectional and multi-centre descriptive study conducted in spanish prisons. a font roja satisfaction questionnaire adapted by j. arranz for the study was used to measure degrees of job satisfaction using a likert's scale. a parametric test was used and a regression model was constructed for predictive ends. results: 376 nurses answered the questionnaire (participation rate 62.7%; response rate 76.7%) 67 centres took part (91.8%). the average satisfaction mark was 2.84 (cl 95%: 2.81-2.87). the lowest ranked components were job variety 1.66 (cl 95%: 1.58-1.74), job-related stress 2.15 (cl 95%: 2.08-2.23) and control over job 2.77 (cl 95%: 2.73-2.82). the highest ranked aspect was job satisfaction, averaging 3.52 (cl 95%: 3.44-3.58). conclusions: the average satisfaction mark for prison nursing staff was low when compared to other groups of health care professionals, which implies the need for corrective measures.

Abstract: introduction: the board of directors of the spanish association of primary care paediatrics (aepap) decided to address an analysis comparing the situation of child and adolescent assistance in primary care in different regions through the data provided by the corresponding regional federated societies. material and methods: in spring 2010 we conducted a survey using web 2.0 technology with 16 expert paediatricians on the status of child and adolescent primary health care in spain, we chose one paediatrician for each of the spanish autonomous region. they answered 39 selected consensuated questions seeking organizational and intercommunity health care differences. results: we found significant discrepancies in the way of organizing consultations in primary care between different regions. it generates iniquities to children, their families and professionals. we defined weak points as the posts with most unfavourable features such as general practitioners in paediatric offices, shortage of paediatric nursing, long work hours (work in the evening), low wages and lack of access to the internet. the worst communities in this regard are andalucia and madrid with three of these weak points, followed by catalonia and murcia. we have only found four communities without weak points of care. conclusion: to reduce these differences in order to avoid important gaps in health resources devoted to children and adolescents falls on health authorities.
Análisis de perforantes de la epigástrica inferior profunda con Angio TC 3D, Eco Doppler color y Doppler simple de ultrasonidos en colgajo DIEP: resultados preliminares

Abstract: the aim of this report, is to validate the angio-ct technique with three-dimentional reconstruction as a preoperative planning tool, after comparison with doppler ultrasound and color- duplex. between january 2006 and march 2007, we studied 11 consecutive patients (13 diep) in whom a prospective comparative followed up was performed comparing, the findings observed using doppler ultrasound, colour-duplex and angio-ct with three-dimentional reconstruction. surgical procedure findings were considered as the reference value. the evaluation procedure included the election of the most suitable perforator vessels, taking account of the location, caliber and anatomical relationships with the muscle. the angio-ct with three-dimentional reconstruction showed 100% specificity (ci 95% 75.3-100), and very high positive predictive value. in this way, this technique could be considered as an excellent tool for preoperative evaluation of perforators flaps. color-duplex showed that in 46,1% of the patients (ci 95% 19,2-74%) the selected perforator matched with the intraoperatively selected. using doppler ultrasound only in 30,8% of the cases (ic 95% 9,1-61,4%) this result was observed. in this report, angio-ct with three-dimentional reconstruction has revealed as a very high specificity technique, that allows a great account of important information, only comparable with anatomical dissection and ahead from doppler ultrasound and color-dupplex.

Abstract: Nuestro objetivo es la validación de la Angio TC tridimensional como herramienta de planificación de los colgajos DIEP, comparándolo con el Doppler de ultrasonidos (US) y eco Doppler color. Entre enero de 2006 y marzo de 2007 se realiza en 11 pacientes (13 DIEP) un estudio comparativo prospectivo entre el Doppler de US, eco Doppler color y Angio TC con reconstrucción tridimensional, utilizando como dato de referencia los hallazgos intraoperatorios. En dicho proceso se localiza la mejor perforante que pueda servir como pedículo al colgajo DIEP en función de su localización, calibre, trayecto y relaciones anatómicas con respecto al músculo. La Angio TC con reconstrucción tridimensional, demuestra una especificidad del 100% (IC 95% 75.3-100) lo que le convierte en una prueba con un alto valor predictivo positivo y una excelente herramienta en la planificación de los colgajos de perforantes. El eco Doppler color determinó, que tan sólo en un 46,1% (IC 95% 19,2-74,9) de los pacientes, la perforante seleccionada por la prueba de forma preoperatoria, coincidía con la perforante elegida en quirófano. Con el Doppler de US, en un 30,8 % (IC 95% 9,1-61,4) de los colgajos estudiados, coincidía la mejor perforante escogida de forma preoperatoria, con los hallazgos obtenidos tras la disección del colgajo. En el presente estudio, la Angio TC tridimensional se ha mostrado como una técnica con una gran especificidad que proporciona valiosa información, sólo comparable con la disección anatómica y por delante de pruebas como el Doppler de ultrasonidos y el eco Doppler color. Abstract: eosinophilic gastroenteritis is a rare disorder, characterized by eosinophilic infiltration of any part of the gastrointestinal tract. clinical presentation is heterogeneous, depending on the site, length and depth of inflammation in the intestinal wall. allergies and peripheral eosinophilia may be present. radiographic and endoscopic findings are variable, nonspecific and only contribute to support the diagnosis. corticosteroids are the mainstay of therapy. prognosis is good and there is no increased risk of gastrointestinal malignancy.

Abstract: celiac disease is an autoimmune enteropathy, triggered by the ingestion of gluten, with a genetic predisposition. there is a strong association with major histocompatibility complex class ii alleles dq2 and dq8-dr4, but there are other factors involved. the epidemiology of the disease has increased and it is now considered the most common food intolerance in the world. diagnosis may be established in older patients. there is a wide spectrum of clinical presentation, with a systemic and commonly mild character. the most serious complications are malignant tumours, namely lymphomas. serological tests are important, given their high sensitivity and specificity, in diagnostic evaluation and to monitor adherence and response to treatment. there are several histological patterns, including intraepithelial lymphocytosis, crypt hyperplasia, and different stages of villous atrophy. the conventional treatment is a lifelong gluten-free diet, which improves symptoms, laboratorial tests and pathological lesions in most patients. in severe cases it might be necessary parenteral nutrition and corticosteroid therapy. immunosuppression is beneficial in some situations of refractory celiac disease. some alternatives to gluten restriction have been suggested, but they are still experimental.
Influence of motivation on academic performance of students undertaking vocational training

Abstract: Vocational training in Spain is not longer an option for those students who do not reach the right level to attend secondary school, and has turned into a professional alternative of high quality and with high levels of employability. The analysis of the motivations that foster student learning is very useful in order to design intervention proposals which may improve the quality of student learning. This research study is a response to this approach. We present a theoretical review of the basics of motivation and its relationship with learning, followed by an empirical study of considerable length, whose results are striking: both mid-level and higher level vocational course students require supervision and specific training in all aspects related to motivation and learning strategies.

Abstract: Conceptions of teaching is becoming a topical subject because of its impact on the quality of learning. In the last two decades there has been an exponential growth in the number of research studies focused on developing a corpus of knowledge and results. From the review of some of these studies, we find a number of taxonomies that help classify hierarchically the conceptions teachers have regardless of the subject or academic level they teach. Conceptions are either teacher- or student-oriented. This paper presents the most relevant features of the conceptions teachers may develop when teaching.

Abstract: The beliefs and conceptions of teaching and learning held by teachers is a research topic which could mean a step forward in our understanding of important factors for improving the quality of Education. For over two decades, a number of researchers have achieved results through studies which offer a corpus of solid knowledge about beliefs and conceptions of teaching and learning which has resulted in establishing new andinteresting interpretations of that relationship. In this paper, we present the ideas about beliefs and conceptions held by a group of researches about teaching and learning.

Abstract: A doen a celíaca é uma enteropatia auto-imune, induzida pela ingest o de glúten, estando subjacente uma predisposi o genética. Existe uma forte associa o com as moléculas do complexo major de histocompatibilidade DQ2 e DQ8-DR4 mas outros factores est o seguramente envolvidos. O contexto epidemiológico da doen a expandiu-se e, actualmente, é considerada a intolerancia alimentar mais comum no mundo. O diagnóstico pode ser estabelecido em idades avan adas. Reconhece-se um amplo espectro de manifesta es clínicas, de carácter sistémico e muitas vezes pauci-sintomático. As complica es mais graves s o o desenvolvimento de neoplasias, designadamente linfomas. Os marcadores serológicos desempenham um papel importante, mercê da sua elevada sensibilidade e especificidade, na investiga o diagnóstica, assim como na monitoriza o da ades o e resposta à terapêutica. Est o identificados vários padr es histológicos que englobam linfocitose intra-epitelial, hiperplasia das criptas e diversos graus de atrofia vilositária. O tratamento assenta numa dieta sem glúten, indefinidamente, o que conduz na maioria dos casos a remiss o clínica, laboratorial e histológica. Nas formas graves poderá estar indicada alimenta o parentérica e corticoterapia. A imunossupress o é benéfica nalgumas situa es de doen a celíaca refractária. Têm sido sugeridas alternativas à restri o de glúten mas ainda se encontram no domínio experimental.
