The Expanding Role of Proteasome-Based Therapy in the Treatment of Hematologic Malignancies

Abstract: The Ubiquitin (Ub)+Proteasome pathway is the major cellular pathway for the selective degradation of nuclear and cytosolic proteins. The proteasome is the catalytic core of the Ub+Proteasome pathway and has become an intriguing new target in drug development and cancer therapy for the treatment of hematologic malignancies. Successful pharmacologic inhibition of the proteasome with the boron-containing small molecule bortezomib led to US Food and Drug Administration (FDA) approval for the treatment of multiple myeloma (MM). That clinical success has propelled tremendous interest and application of proteasome inhibition to an increasing number of hematologic malignances. Inhibition of the proteasome results in the accumulation of multi-ubiquitinated proteins that are normally degraded through the tightly regulated Ub+Proteasome pathway. Such an accumulation leads to the stabilization of numerous cellular proteins that control the cell cycle, growth, proliferation and apoptosis. It is thought that the accumulation of multi-Ub~protein conjugates leads to apoptosis although there are numerous mechanisms proposed to explain how proteasome inhibition leads to cell death. However, not all patients respond to bortezomib-based therapy and moreover, those patients that do respond inevitably develop drug resistance. In addition, the mechanism of action for bortezomib remains incompletely characterized and, thus, newer proteasome inhibitors are needed and are in clinical development. The use of the proteasome inhibitor bortezomib has been expanded successfully from MM to other hematologic malignancies that include various lymphomas, Waldenstr m’s macroglobulinemia, Amyloidosis and Acute Myeloid Leukemia (AML). The proteasome is a validated therapeutic target and proteasome inhibitors modulate protein stability to effect tumor cell growth control and promote programmed cell death. Targeting the Ub+Proteasome pathway offers great promise in the treatment of hematologic and eventually solid tumor malignancies.
Structural and Functional Characterization of the Recombinant Death Domain from Death-Associated Protein Kinase

Abstract: Death-associated protein kinase (DAPk) is a calcium/calmodulin-regulated Ser/Thr-protein kinase that functions at an important point of integration for cell death signaling pathways. DAPk has a structurally unique multi-domain architecture, including a C-terminally positioned death domain (DD) that is a positive regulator of DAPk activity. In this study, recombinant DAPk-DD was observed to aggregate readily and could not be prepared in sufficient yield for structural analysis. However, DAPk-DD could be obtained as a soluble protein in the form of a translational fusion protein with the B1 domain of streptococcal protein G. In contrast to other DDs that adopt the canonical six amphipathic α-helices arranged in a compact fold, the DAPk-DD was found to possess surprisingly low regular secondary structure content and an absence of a stable globular fold, as determined by circular dichroism (CD), NMR spectroscopy and a temperature-dependent fluorescence assay. Furthermore, we measured the in vitro interaction between extracellular-regulated kinase-2 (ERK2) and various recombinant DAPk-DD constructs. Despite the low level of structural order, the recombinant DAPk-DD retained the ability to interact with ERK2 in a 1:1 ratio with a Kd in the low micromolar range. Only the full-length DAPk-DD could bind ERK2, indicating that the apparent ‘D-motif’ located in the putative sixth helix of DAPk-DD is not sufficient for ERK2 recognition. CD analysis revealed that binding of DAPk-DD to ERK2 is not accompanied by a significant change in secondary structure. Taken together our data argue that the DAPk-DD, when expressed in isolation, does not adopt a classical DD fold, yet in this state retains the capacity to interact with at least one of its binding partners. The lack of a stable globular structure for the DAPk-DD may reflect either that its folding would be supported by interactions absent in our experimental set-up, or a limitation in the structural bioinformatics assignment of the three-dimensional structure.
Strong Pinning Enhancement in MgB2 Using Very Small Dy2O3 Additions

Abstract: 0.5 to 5.0 wt.% Dy2O3 was in-situ reacted with Mg + B to form pinned MgB2. While Tc remained largely unchanged, Jc was strongly enhanced. The best sample (only 0.5 wt.% Dy2O3) had a Jc of 6.5 x 10^5 A/cm^2 at 6K, 1T and 3.5 x 10^5 A/cm^2 at 20K, 1T, around a factor of 4 higher compared to the pure sample, and equivalent to hot-pressed or nano-Si added MgB2 at below 1T. Even distributions of nano-scale precipitates of DyB4 and MgO were observed within the grains. The room temperature resistivity decreased with Dy2O3 indicative of improved grain connectivity.

Abstract:
Failure to rescue a patient that is experiencing deteriorating symptoms is a major cause of mortality in the acute care settings. The nurse is at the fore-front of prevention and recognition of patient distress. Failure to rescue patients that are displaying symptoms of deterioration is becoming a national crisis due to treatment complications and missed signs and symptoms of impending arrest [1]. New nurse graduates must have additional training to assist them in the recognition of the signs of impending crisis. This paper will discuss the need for additional training after nursing school to address this issue and assist the new graduate nurse in their transition to practice. The development of a New Graduate Nurse simulation education program for Littleton Regional Healthcare and their Alliance Hospitals was the primary focus of this project. PICOT: How will the use of simulated emergency scenarios with intermittent audit/feedback impact the knowledge level and clinical reasoning of new nurse graduates in an acute care setting? High Fidelity Simulation patient scenarios of impending crisis were developed for a 4-hour educational workshop. New nurse graduates with under 2.5 years experience attended the training with results showing that high fidelity simulation training scenarios with intermittent audit feedback assisted the new nurses to recognize signs and symptoms of impending crisis, how to develop skills to work as a team member, and gave them more confidence in reporting mistakes and voicing concerns. The literature search completed for this study shows that the new graduate nurse needs to develop clinical reasoning skills and gain experience on how to respond to patients with clinical emergencies and simulation can assist with this knowledge to practice gap [2].
