Cultura organizacional de enfermería dominante en un hospital de la Secretaría de Salud de Morelos, México

Abstract: objective: to identify dominant and preferred organizational culture models at second level nursing services in morelos. material and methods: a cross-sectional study was conducted between april and june, 2008. nursing caregivers from public health services of morelos state participated. we gathered and analyzed data about socioeconomic characteristics and organizational culture, and we applied the xi2 and kappa coefficient statistical tests using stata version 10 software. results: we interviewed 30 nurses, randomly selected from a total of 397, with at least one year of seniority. it was identified that the dominant organizational culture is one of power and the preferred culture is one of performance. a kappa positive coefficient of 0.17 (p = 0.047) was found between the dominant model and the preferred performance model. conclusions: identifying said organizational culture models helps to understand performance differences in the management of nursing care services currently provided to users of the hospital and to aspire to improvements in the service itself.

Abstract: objective. to evaluate the modes of detection of breast malignancies in mexico city and to estimate the number of patients diagnosed in advanced stages. material and methods. this cross-sectional study was conducted between 1994 and 1996, among 256 women with a histological diagnosis of breast cancer, at three tertiary level public hospitals in mexico city. personal interviews were conducted to collect data on reproductive characteristics, clinical history, and breast cancer modes of detection. data analysis consisted of percentages, odds ratios, and 95% confidence intervals. results. ninety percent of women sought medical care after identifying a breast lump by themselves. only 10% of patients had a stage-i tumor; all 27 cases first identified by a physician were in stage ii-b and higher. conclusions. an increasing trend of breast cancer mortality in mexico city will persist, unless a greater proportion of tumors in situ is detected; this would require enhancing breast cancer screening programs and conducting an intense educational intervention among women at risk. the english version of this paper is available at: http://www.insp.mx/salud/index.html
Identificación de lesiones mamarias malignas en México

Abstract: Objetivo. Evaluar la forma en que se detectan las lesiones mamarias malignas en la ciudad de México y estimar el número de pacientes que son diagnosticadas en etapas avanzadas de dicho tumor. Material y métodos. Estudio transversal realizado en 256 mujeres entre 1994 y 1996, a quienes se les hizo un diagnóstico histopatológico de cáncer mamario en tres hospitales públicos de tercer nivel en la ciudad de México. Se les entrevistó personalmente y se obtuvo información acerca de sus características reproductivas, clínicas y la forma en la que fue detectado el cáncer mamario. Se calcularon porcentajes y razones de momios, con un intervalo de confianza de 95%. Resultados. El 90% de las mujeres identificaron por sí mismas la presencia de un abultamiento en el seno. Del total de las pacientes, sólo 10% fueron diagnosticadas con tumores en estadio I y 27 casos fueron identificados por el médico; estos últimos resultaron con tumores de estadios II B en adelante. Conclusiones. La situación actual sugiere que la mortalidad por cáncer mamario en la ciudad de México mantendrá su tendencia al incremento, a menos de que se logre aumentar la proporción de mujeres diagnosticadas en la etapa de los tumores in situ, para lo cual se requieren cambios en el funcionamiento de los servicios y un vasto esfuerzo educativo entre la población susceptible de desarrollar la enfermedad. Abstract: objective: to identify the meeting sites of new sexual partners in two southern border cities in mexico. material and methods: a descriptive epidemiologic study was carried out in may 2001, by surveying key informants in two border cities. each reported site was visited to interview a resident subject. subjects socializing in a subsample of sites were also interviewed. results: the key informants of the community reported 134 meeting sites in chetumal and 111 in ciudad hidalgo. both sites had scarce hiv/aids prevention activities. characteristics were obtained for 89 sites in chetumal and 42 in ciudad hidalgo. almost 21% of interviewees reportedly met a new sexual partner in these sites in the past four weeks. conclusions: preventive actions are needed in meeting sites of new sexual partners. this method may provide information to plan for future interventions.

A interdisciplinaridade apresentada contribui para a análise dos processos cognitivos envolvidos no comportamento dos produtores e dos usuários de sistemas de tratamento da informa o no sentido de compreender a informa o, como ela é percebida, recepcionada e comunicada.[English] The present is an essay on the conceptual and theoretical discussion of information, which is the study object of Information Science in its cognivitist approach; it emphasizes its relation with knowledge representation as well as its interdisciplinarity. Concepts and their attributes are analyzed according to the epistemologic theorists of Information Science, especially Saracevic, who establishes cognitive and computing sciences as two of its pillars. The others are librarianship and communication. The interdisciplinarity presented herein contributes to the analysis of the cognitive processes involved in the behavior of producers and users of information treatment systems towards understanding information, how it is perceived, received, and communicated.

Abstract: End of life and changes in medical bioethics The issue of the end of life includes —necessarily— an analysis of human dignity in its social and legal dimension as a parameter and value of the cultural history of countries and societies. The vagueness of this term has created, in particular, many difficulties in the effort to define its boundaries. In particular, the use of human dignity to understand the dynamics of the end of life faces the issue of medical paternalism and theory of informed consent in the doctor-patient relationship. El tema del final de la vida incluye —necesariamente— un análisis de la dignidad humana en su dimensión social y jurídica, como parámetro y valor de la historia cultural de los países y de las sociedades. La indeterminación de este concepto ha creado, en lo concreto, muchas dificultades en el esfuerzo de concretar sus confines. En particular, el uso de la dignidad humana para comprender las dinámicas del fin de la vida enfrenta el tema del paternalismo médico y la teoría del consentimiento informado en la relación médico-paciente.
Compact radio sources in the vicinity of the ultracompact HII region G78.4+2.6

Abstract: Using the Very Large Array (VLA) at 3.6~cm we identify four new compact radio sources in the vicinity of the cometary HII region G78.4+2.6 (VLA~1). The four compact radio sources (named VLA~2 to VLA~5), have near-infrared counterparts, as seen in the 3.6 $\mu$m Spitzer image. One of them (VLA~5) clearly shows evidence of radio variability in a timescale of hours. We explore the possibility that these radio sources are associated with pre-main sequence (PMS) stars in the vicinity of the UC HII region G78.4+2.6. Our results favor the smaller distance value of 1.7 kpc for G78.4+2.6. In addition to the detection of the radio sources in the vicinity of G78.4+2.6, we detected another group of five sources which appear located about 3' to the northwest of the HII region. Some of them exhibit extended emission.
