Abstract: rifaximin is an antibiotic recently approved for the treatment of hepatic encephalopathy in adults. in children more than 12 year-old, it has been approved for travelers' diarrhea and it is also widely used in infammatory bowel disease. we report, to our knowledge, the frst case of a pediatric patient who received rifaximin for hepatic encephalopathy with good clinical outcome.

Habitat mapping and conservation threats to river dolphin in Karnali River of Nepal

Abstract: This study was carried out in the Karnali River of Nepal with the aims of preparing habitat map of dolphins in the Karnali River and assessing conservation threats to dolphin at the local level. The habitat map of the dolphin was prepared on the basis of study and local sightings. Altogether 100 households were surveyed around the Karnali River living within 3 km from the river distance. Questionnaire survey, group discussion, field observation and key-informants interviews were conducted to assess conservation threats to dolphin at the local level. Statistical tools like Pie chart and bar diagram were used to analyze the data. Use of poison in the river, commercial and domestic consumption of fishes, and high dependency of people in the river are emerging as the threats to dolphin conservation. Similarly, the increasing trend of using chemical fertilizer in the agriculture land is also adding up to long term negative impacts on dolphin population. Regular habitat monitoring of the dolphins should be done in major areas to acquire timely information on status and distribution of dolphins for dolphin conservation. Abstract: In this paper, we analyze the queueing behaviour of wavelength division multiplexing (WDM) Internet router employing partial buffer sharing (PBS) mechanism with self-similar traffic input. In view of WDM technology in networking, each output port of the router is modelled as multi-server queueing system. To guarantee the quality of service (QoS) in Broadband integrated services digital network (B-ISDN), PBS mechanism is a promising one. As Markov modulated Poisson process (MMPP) emulates self-similar Internet traffic, we can use MMPP as input process of queueing system to investigate queueing behaviour of the router. In general, as network traffic is asynchronous (unslotted) and of variable packet lengths, service times (packet lengths) are assumed to follow Erlang-k distribution. Since, the said distribution is relatively general compared to deterministic and exponential. Hence, specific output port of the router is modelled as MMPP/Ek/s/C queueing system. The long-term performance measures namely high priority and low priority packet loss probabilities and the short-term performance measures namely mean lengths of critical and non-critical periods against the system parameters and traffic parameters are computed by means of matrix-geometric methods and approximate Markovian model. This kind of analysis is useful in dimensioning the router under self-similar traffic input employing PBS mechanism to provide differentiated services (DiffServ) and QoS guarantee.

Abstract: hepatoxicity of isoniazid, mainly in association with rifampin, is a rare secondary effect of tuberculostatic treatment. in the united states, it accounts for 0.2% of all pediatric orthotropic liver transplant, and 14% of transplants for drug hepatotoxicity. we report the case of a 10 year-old patient who presented with acute liver failure requiring orthotropic liver transplant after forty days of tuberculostatic treatment with isoniazid, rifampin and pyrazinamide.
Bacteriological Profile of Sepsis Outbreak in the NICU of a Tertiary Care Hospital in Western Nepal

Abstract: Introduction: Neonatal sepsis is a serious problem for the neonates who are admitted to the intensive care and outbreak of sepsis is not uncommon. This paper aims to describe a sepsis outbreak as a result of too many admissions, overcrowding of babies with limited working staffs in the unit and compares microorganisms with their antibiogram in newborn and environmental samples. Materials and Methods: Prospective observational study from 1st April -15th August 2011 in Neonatal Intensive Care unit of Manipal Teaching Hospital, Pokhara and included all babies admitted for sepsis. Results: There were 103 (57.22%) episodes of neonatal septicemia (Term =47.8%; Preterm = 84.8% p<0.001), 14/20, 70% of whom died of sepsis. 47.52% had early onset sepsis and 52.42% had late onset sepsis and 39.8% had nosocomial infection. The predominant isolates in newborn (NB) were E. coli, staphylococcus aureus and Klebsiella pneumonia and in environmental sample it was Klebsiella pneumonia and Staphylococcus. aureus. Imipenem, Vancomycin, Netilmycin, Tobramycin and chloramphenicol were sensitive (S-100%) while Carbenicillin and Piperacillin, Ampicillin, most cephalosporins, Penicillin were resistant (R-100%) to organisms in newborn and environmental samples. Other S-100% antibiotics for newborn were Ceftazidime, Ciprofloxacin and Gentamycin while S-100% for environmental sample isolates were Cephoperazone, Cloxacillin, Cefuroxime and Tetracycline. Other (R – 100%) antibiotics for newborn were Amoxicillin and Amoxyclav and for environmental sample were Gentamycin and Erythromycin. Conclusions: Sepsis is a severe problem for neonates. Periodic evaluation of bacterial antibiotic susceptibility and judicious selection of antibiotics is necessary to reduce the resurgence of multidrug resistant strains. El ídolo de El Lomo (Cogolludo, Guadalajara)

Abstract: This paper discusses a carved stone found in El Lomo, a prehistoric settlement near Cogolludo (province of Guadalajara) in central Spain. The natural shape, of the stalagmitic rock from which the idol of Cogolludo was made resembled some prehistoric pebbles in which a man-made mark along the middle suggests they were connected with some kind of fertility rite; in the case of the idol from El Lomo, this intention was reinforced by decorating the top of the stone with a perpendicular carving, making it resemble a horned idol. The symbolic pieces are gene rally associated with Eneolithic ritual practices. The Cogolludo carving was found in a pit associated with potsherds corresponding to the first period of the site's occupation (El Lomo I). This phase is C-14 dated to about 2100 B.C Se estudia un bloque de piedra esculpido, procedente del poblado de El Lomo (Cogolludo, Guadalajara). Es un fragmento de estalagmita que, en su estado natural, podía sugerir una semejanza con los ídolos ovoides , sobre guijarros, que se caracterizan por llevar una ancha acanaladura longitudinal. Esta sugerencia se completó por el procedimiento de tallar en la parte superior del bloque otra hendidura profunda que le confiere la apariencia de un ídolo de cuernos . Estas piezas de intencionalidad simbólica son propias de ambientes eneolíticos. El ídolo de Cogolludo apareció en una hoya que corresponde precisamente a la primera fase, eneolitica, del poblado, a juzgar por las cerámicas que la acompa aban. Para esta fase (El Lomo I) contamos con dos fechas radiocarbónicas en torno al a o 2100 a.C.
