Contradicciones sociales expresadas en la práctica de la Atención Farmacéutica Hospitalaria en Cuba

Abstract: introduction: the clinical functions and the practice of pharmaceutical care at hospital setting are limited in cuba. although there is an approach to the conception of these functions in the current manuals of procedures, the pharmacists mostly carry out administrative and managing functions in addition to those of drug supply. objective: to analyze the social contradictions (causes) that have an effect on the practice of pharmaceutical care at hospital, particularly in santiago de cuba, in order to draw up strategies for improvement of this activity. methods: a tool facilitating the analysis of problems and their solutions, the cause-effect diagram and the weighed voting consensus method were applied to determine the main and secondary causes that hinder the above-mentioned practice at hospital. the main analyzed causes were human resources, methods, management styles, material resources, working means and conditions. results: the results of the weighed voting revealed that the highly weighed causes in descending order were managing methods and styles with 37 points, the human resources with 33 points and the material resources with 18.5 points. conclusions: the identified contradictions mark paths of research to solving the stated problems, which will allow creating a model of principles for the development of pharmaceutical care at hospital setting, on the basis of evaluations and assessments carried out and on the principles of the good practice of pharmacy.

Abstract: Introducción: en Cuba el ejercicio de las funciones clínicas y la práctica de la Atención Farmacéutica a nivel hospitalario son limitadas pues, aunque existe un acercamiento a la concepción de estas funciones en los actuales manuales de procedimientos, los profesionales farmacéuticos realizan mayormente funciones administrativas y de dirección, además de las relacionadas con el suministro de medicamentos. Objetivo: analizar las contradicciones sociales (causas) que repercuten en la práctica de la Atención Farmacéutica Hospitalaria, específicamente en Santiago de Cuba, para trazar líneas estratégicas que permitan el perfeccionamiento de esta actividad. Métodos: se aplicó una herramienta que facilita el análisis de problemas y sus soluciones, el diagrama de causa-efecto, así como el método de consenso de votación ponderada, para determinar las causas principales y secundarias que frenan dicho ejercicio a nivel hospitalario. Las principales causas analizadas fueron los recursos humanos, los métodos y estilos de dirección, los recursos materiales, los medios y las condiciones de trabajo. Resultados: los resultados de la votación ponderada mostraron que las causas de mayor ponderación en orden descendente fueron las relacionadas con los métodos y estilos de dirección con 37 puntos, los recursos humanos con 33 puntos y los recursos materiales con 18,5 puntos. Conclusiones: las contradicciones identificadas marcan caminos de investigación para resolver las problemáticas planteadas, lo que permitirá desarrollar un modelo que establezca principios para el desarrollo de la Atención Farmacéutica a nivel hospitalario dise ado sobre la base de las evaluaciones y las valoraciones realizadas y los principios de las Buenas Prácticas de Farmacia. Alteraciones Hepáticas en Pacientes con Dengue Hepatic Alterations in Patients with Dengue

Abstract: La caracterización clínica del Dengue es muy variable debido a las múltiples alteraciones inducidas por el virus en el organismo. Se ha reportado incremento en los niveles de transaminasas similares a los producidos por el virus de Hepatitis en pacientes con Dengue de zonas hiperendémicas de Asia. Los objetivos del trabajo fueron determinar alteraciones en las pruebas de funcionalismo hepático en pacientes con Dengue y relacionar clínica y serológicamente la infección viral. A 62 casos con confirmación serológica para Dengue, se les realizó historia clínica, hematología completa, Tiempo de Protrombina (TP) y Tiempo Parcial de Tromboplastina (TPT), cuantificación de transaminasas (ALT y AST) y bilirrubina en suero. En relación a las características clínicas el 38,7% de los pacientes con Dengue Clásico (DC) y Fiebre Hemorrágica por Dengue (FHD) refirieron dolor abdominal y 2 con FHD presentaron hepatomegalia e ictericia. En los resultados de laboratorio se observó leucopenia (72,5%) en ambas formas de Dengue y en pacientes con FHD, trombocitopenia severa (<50.000×mm3) y TP y TPT alargados en 70,9%, 23,0% y 42,3%, respectivamente. El valor de transaminasas resultó hasta cinco veces más elevado que el valor normal (p<0,005) en 36,8% de los pacientes con DC y en 74,4% con FHD, con predominio de AST en ambos grupos. Los resultados sugieren da o hepático durante el curso del Dengue. Esto permite afirmar que la infección por virus Dengue puede causar disfunción hepática aguda y debe ser considerada en el diagnóstico diferencial con otras infecciones virales con disfunción hepática. Clinical features of Dengue are very variable due to multiple alterations induced by the virus in the organism. Increased levels of transaminases similar to those produced by the Hepatitis virus have been reported in patients with Dengue from hiperendemic zones in Asia. The objectives of this study were to determine alterations in the liver tests in patients with Dengue and to relate them to the disease, clinically and serologically. Clinical history, hemathological tests serum transaminases (ALT y AST) and bilirubin assays were performed in 62 patients with clinical and serological diagnosis of Dengue. According to clinical features 38.7% of the patients with classical (CD) and hemorrhagic (DHF) forms of Dengue reffered abdominal pain and 2 patients with DHF had ictericia and hepatomegaly. Laboratory test findings showed leucopenia in 72.5% in both forms of Dengue and of patients with DHF severe thrombocytopenia (< 50.000 platelets × mm3), long PT and PPT in 70.9%, 23.0% and 42.3%,

Abstract: Teniendo en cuenta la necesidad de un currículo que garantice una formación universitaria universal, contextual, flexible y comprometida con la realidad social y profesional, en el presente trabajo se dise ó el perfil profesional que caracterizará al egresado de la Licenciatura en Ciencias Farmacéuticas de la Universidad de Oriente, Cuba. Se utilizó para ello el Modelo de Actuación Profesional; se precisó el problema profesional, el objeto de la profesión y el objetivo profesional, y sobre la base del sentido articulador entre estas configuraciones se definieron los campos, esferas y modos de actuación, dejando explícito, además, los objetivos educativos e instructivos que se aspiran alcanzar. Taking into account the need of a curriculum assuring universal, contextual and flexible university formation that is committed to the social and professional realities of the country, the present paper designed the professional profile of the Pharmaceutical Sciences graduate in the University of Oriente, Cuba. To this end, the Professional Performance Model was used; the professional problem, the object of the profession and the professional objectives were defined. On the basis of the articulation of these configurations, the various fields, spheres and modes of performance were established, thus clearly stating the educational and learning objectives that are expected to be attained.

Abstract: often, users get to know the information management system (sgi) while using it. this can cause problems in information retrieval, especially if there is a discrepancy between the needs and expectations of the user and the representation of the task to be accomplished in the interface. this paper illustrates the design of the sgi interface from its inception. it takes intoaccount the user, the task (mental model), and the completion of the task (activity), with the support of cognitive ergonomics, the emotional design, usability, and an evaluation of the interface project. the techniques applied to this end were the analysis of the task and activity, systematic observation, paper prototyping, the target group, the questionnaire, the interview, and card management. the use of these techniques was based on the logic employed by the users and the logic of analysts and information professionals. this can facilitate the development and implementation of suggestions that would add value to the understanding and the analysis of the issues involved in the design of the sgi interface and it can also encourage increased user satisfaction.

Abstract: En este ensayo la profesora Ivette Fred discute la utilización del lenguaje científico en las discusiones sobre el cambio climático en la tierra. Absteniéndose a cuestionar la validez de los planteamientos científicos, se limita a analizar la forma en la cual los científicos transmiten estas ideas. Considera que el lenguaje utilizado para referirse a los cambios climáticos es similar al que se emplea en los discursos religiosos de corte apocalíptico. Compara ambos discursos, como describen el fin del mundo en un lenguaje muy similar. Termina por plantear las siguientes preguntas: Por qué los científicos recurren al lenguaje apocalíptico? Cómo los seres se pueden unir como humanidad? Cómo se podría describir una catástrofe sin sonar catastrófico?
