Prevalencia de diabetes en una población adulta de Madrid (Espa?a): studio MADRIC (MADrid RIesgo Cardiovascular)

Abstract: objective: to estimate the prevalence of diabetes mellitus and impaired fasting glycemia in an adult population living in madrid (spain). methods: in this cross-sectional, population-based survey, we studied 1,344 adults aged 31-70 years, randomly selected from the population living in 14 primary care districts of the madrid region. all participants underwent a clinical evaluation that included a clinical interview, physical examination and fasting blood analysis (glycemia, cholesterol and triglyceride levels). the participants were considered to be diabetic if they had been previously diagnosed with diabetes by their general physician or had a fasting glycemia > 126mg/dl without a previous diabetes diagnosis. impaired fasting glycemia was defined as fasting glycemia between 100mg/dl and 125mg/dl in non-diabetic participants. results: the sex- and age-adjusted prevalence figures for diabetes and impaired fasting glucose were 6.6% (95% ci: 5.9-8.7) and 14.1% (95% ci: 12.1-15.8), respectively. a substantial proportion of diabetic patients [17.2% (95% ci: 10.9-23.5)] had not been previously diagnosed. the variables independently associated with diabetes were age, male gender, abdominal obesity and hypertension. conclusions: our prevalence figure for diabetes is similar to those reported in other spanish regions. the high frequency of impaired fasting glucose is worrisome, particularly when combined with obesity, as this association confers a high risk for developing diabetes mellitus.

Abstract:

Recent research suggests that both tropical ocean warming and stratospheric temperature anomalies due to ozone depletion have led to a poleward displacement of the midand high-latitude circulation of the Southern Hemisphere over the past century. In this study, we attempt to distinguish the influences of ocean warming and stratospheric cooling trends on seasonal changes of both the zonally symmetric and asymmetric components of the southern hemisphere circulation. Our analysis makes use of three data sets-the ERA40 reanalysis and results from two different runs of the GFDL global atmosphere and land model (AM2.1) for the period 1870 to 2004. A regression analysis was applied to two variables in each of the three data sets-the zonal component of the surface wind U(10 m) and the height at 300 hPato determine their correlation with zonally averaged polar stratospheric temperatures (T_polarat 150 hPa, averaged over a band from 70S - 80S) and low-level equatorial temperatures (T_equatorat 850 hPa averaged over a band at 5S - 5N). La transformation des relations entre état et associations au centre d’une nouvelle dynamique institutionnelle : le cas de la politique de la ville en France

Abstract: Les associations uvrant pour la politique de la ville constituent le principal mode d’intervention de l’état dans cette politique publique. Il serait pourtant erroné de ne considérer ces associations que comme de simples auxiliaires de la puissance publique. On assiste en fait à une dynamique de plus grande ampleur qui tout en inscrivant les associations dans une nouvelle configuration institutionnelle participe aussi de l’émergence d’un état médiateur . Celui-ci emprunte à l’idée associative son positionnement au sein de la société caractérisé par la valorisation de la proximité et de la cohésion sociale qui en fait une sorte de corps intermédiaire entre l’état et la société civile. Réciproquement, l’état médiateur intègre à lui, par des dispositifs partenariaux, certaines associations, celles notamment qui de petite taille, peuvent justifier de la plus grande proximité avec les populations des quartiers de la politique de la ville. L’état médiateur ne désigne donc en aucune manière une phase de retrait de l’état mais un redéploiement de celui-ci, marqué par un fort interventionnisme sur la société en même temps que l’adoption d’un discours et de pratiques qui furent longtemps l’expression exclusive des associations.

Abstract: this article analyzes the círio [candle-lit procession] and the feast of our lady of nazaré as a great collective rite that combines formal and devotional aspects with the profane informality of a brazilian festival. as a grass roots manifestation of catholicism and a typical celebration of the state of pará, the círio of nazaré and attending festivities embrace native elements to express a regional identity, a particular temporality, and a perception of order and differing ways to contrast it.

Abstract: the paper describes two new endoscopic signs which can be useful in the use of diagnostic bronchoscopy. this is an essay on particular anatomic and functional circumstances suggesting the presence of a pathology which cannot be seen by the bronchoscopist. these two signs are the bubble sign and the black hole sign. the bubble sign is observed when the airflow is absent in a subsegmental bronchus leading to a peripheral pulmonary mass. this can be shown by the lack of mobile bubbles when a subsegmental bronchus is flooded with saline solution. the black hole sign consists in the circular penumbra observed at the end of a bronchus which communicates with a cavity. the paper describes the presentation and interpretation of these signs and offers an explanation about their physiopathology.

Abstract: the author starts by asking himself whether the family is relevant nowadays. to answer this question he analyzes the couple's relationship with each other and with their children, placing these relationships historically. it is suggested that the "familial problems" come to being when a certain pattern of family becomes standard. those families which do not follow this pattern are then called "problematic". for that reason, the author prefers to make reference to different "familial situations". these are thought from a double perspective: the invariance which also refers to a sort of evolution, or the novelty which, as a contrast, relates to rupture and re-composition. three areas of familial situations are then distinguished: those which come from their familial relationships, from the social world and from each subject (as the variations from his/her internal world affect his/her familial bonds). at last, the author asks himself if the new familial forms really answer to the production of novelty and if this novelty relates to its shape or structure. he concludes that the family is not new nor old, but that it does something new or not. the analysis of the family should include an analysis of the making in its bounds and of the consequences of this making.

Abstract: O autor inicia por perguntar-se sobre a relevancia da família na atualidade. Para responder esta quest o, examina os vínculos de casal e de rela o entre pais e filhos, situando-os historicamente. Sugere que os "problemas familiares" instauram-se a partir do momento em que se institui uma forma de família como oficial, sendo "problemas" as que n o seguem esse modelo. Por isto, prefere referir-se a diferentes "situa es familiares", que passam a ser pensadas a partir da crítica de dois critérios: o de invariancia, evolu o de formas anteriores, ou de novidade, ruptura e recomposi o. Distingue três áreas das situa es familiares que podem ser examinadas de acordo com estes critérios: aquelas provenientes das rela es familiares, do mundo social e de cada sujeito, das varia es de seu mundo interno que produzem efeitos nos vínculos familiares. Por fim, o autor pergunta-se se as novas formas familiares respondem realmente à produ o de uma novidade e se esta novidade é de forma ou de estrutura. Conclui que a família n o é nova ou velha, mas faz algo novo ou n o. A análise da família deve incluir uma análise do fazer em seus vínculos e das conseqüências desse fazer.

Abstract: Not available Se indica cuando nacen los institutos del área de Ciencia y Tecnología de Materiales y se expone cómo se evalúan los resultados de la labor que realizan. Se describe esta labor comenzando con la que llevan a cabo los cuatro institutos sectoriales en los ámbitos de las Ciencias de la Construcción, la Ciencia y la Tecnología de Polímeros, la Cerámica y el Vidrio, y la Metalurgia. A continuación se consigna la que realizan los cuatro institutos de Ciencias de Materiales en Aragón, Barcelona, Madrid y Sevilla; y la Unidad de Física de Materiales, en Bilbao. Al pasar revista a la situación actual estos centros se hace referencia a sus líneas de trabajo y proyectos en curso, y se mencionan en algún caso sus horizontes de futuro. También se indican en sendos anexos datos sobre recursos humanos y presupuestarios.
