Violencia contra la mujer en la pareja: determinantes y respuestas sociosanitarias

Abstract: the present study aims to review the problem of intimate partner violence, as well as its causes and consequences. it will also specifically analyze the role of health professionals. in opposition to the classical epidemiological view of risk factors, heise proposes an ?ecological framework? to study violence against women. this framework analyzes the interplay among the personal, situational and sociocultural factors that combine to cause abuse. regarding the frequency of intimate partner violence in spain, in january 2003 there were 2.519 formal complaints and 69 women died between january and november 2003. no geographical patterns in mortality or the incidence of formal complaints of intimate partner violence or among the provinces with the highest incidence of formal complains and those with highest mortality were observed. the only national survey published in spain was performed by the women's institute in 1999, which reported a prevalence of domestic violence of 9.2%. a frequency of 22.8% was found in a primary health care center in granada. health services can play a key role in helping victims of domestic violence, since most women contact the health services at some time in their lives. professionals in administrative or managerial positions can contribute to raising awareness of this health problem, which is one of the main causes of poor health and disability. evidently, beyond consciousness-raising and early detection campaigns, public health strategies should be designed to prevent this serious health problem the causes of which can be changed.
La violencia contra la mujer en la pareja: un estudio en el ámbito de la atención primaria

Abstract: objectives: to determine the prevalence of intimate partner violence (ipv) in a sample of women attending primary care centers for any reason and to analyze the characteristics of this violence. methods: a cross sectional study was performed in 23 primary care centers in three regions of spain (andalusia, madrid and valencia). the sample included 1,402 randomly selected adult women aged between 18 and 65 years attending these centers. among the variables collected through a self-administered questionnaire were the existence of physical, emotional or sexual abuse, and its timing, duration, and frequency. results: overall, 32% had experienced some type of ipv. by type, 7% had experienced both physical and emotional ipv, 14% had experienced emotional ipv, 3% had experienced emotional and sexual ipv, and 6% had suffered all 3 types of ipv. among the latter, 52% had experienced abuse for more than 5 years. women who experienced abuse in a previous relationship had a greater probability of being abused by a new intimate partner (or = 3.30; 95% ci, 2.10-5.18). conclusions: these results show the scale of ipv in spain. health professionals play an important role in ipv, as they are in a privileged position to tackle this serious public health problem.

Abstract: parkinson′s disease is the second most common neurodegenerative disorder after alzheimer′s disease. current therapies are symptomatic and, although these therapies are efficacious during the early stages of the disease, they present important side effects when they are used for a long time. the ideal therapy would be the one that would slow down or stop the progression of the disease. this can be achieved, for instance, with neuroprotective and neurorestorative therapies. among them, cell therapy and therapy with trophic factors such as glial cell line derived neurotrophic factor (gdnf) are the most challenging and promising ones for the scientific community. although the use of gdnf as a treatment for parkinson′s disease was proposed several years ago, it is necessary to develop alternative strategies to deliver gdnf appropriately to concrete areas of the brain. here, the use of microspheres as the most suitable tool for the administration of this neurotrophic factor is discussed.

Abstract: Since the discovery of the Vascular Endothelial Growth Factor (VEGF) and its leading role in the angiogenic process, this has been seen as a promising molecule for promoting neovascularization in the infarcted heart. However, even though several clinical trials were initiated, no therapeutic effects were observed, due in part to the short half life of this factor when administered directly to the tissue. In this context, drug delivery systems appear to offer a promising strategy to overcome limitations in clinical trials of VEGF. The aim of this paper is to review the principal drug delivery systems that have been developed to administer VEGF in cardiovascular disease. Studies published in the last 5 years are reviewed and the main features of these systems are explained. The tissue engineering concept is introduced as a therapeutic alternative that holds promise for the near future.

Abstract: objectives: currently, there is not agreement about the adequate tension for each patient with female stress urinary incontinence treated with urethral slings. the adjustable tension sling remeex (external mechanic regulation) allows adjustment to ideal tension tr ying to avoid or minimize possible reoperations. the objective of these paper is to describe the components of the remeex system, its indications, and the surgical technique to implant and adjust it. methods: indications: the remeex system is indicated female urinary incontinence in cases of urethral hypermobility, fixed urethra, primary sphincteric dysfunction, failure of other incontinence repaired techniques, and urinary incontinence in bladder hyperreflexia. study protocol: the diagnosis is made with appropriate history and physical examination and completed with voiding cystourethrogram and urodynamic study, urinar y tract ultrasound and, optionally, urethrocystoscopy. remeex prosthesis characteristics: the system has three elements: polypropylene mesh, pressure tensor, and disconnection tool. technique: 1. anesthesia: it maybe performed under general or spinal anesthesia. 2. preparation and patient position. 3. surgical technique step-by-step: -abdominal access: 4-6 cm suprapubic incision and development of a supra-aponeurotic space to place the pressure tensor. -vaginal access: longitudinal incision 1 cm from the urethra meatus, dissection of the vesicovaginal plane, and development of the space to place the polypropylene mesh. -combined abdominal-vaginal access: bilateral punction with a 20 cm suture-passing needle from the abdomen to the vagina and passage of the mesh from the paraurethral espace threading its sutures in the pressure tensor system,and closure of the incisions.-cystoscopy.-tension adjustment and postoperative control. conclusions: 1. it is an easy to implant system and a reproducible operation. 2. this technique avoids the morbidity of abdominal operations. 3. it allows the readjus

An empirical analysis of retirement behaviour in Spain: partial versus full retirement

Abstract: In several developed countries, the ageing process of the population may pose fiscal risks to the PAYG systems of public pensions. This paper studies the determinants of two forms of accessing retirement in Spain, either partial or full retirement. Our goal is to identify if social security legislation influences the choice between these two alternative paths. Using a newly released data set we estimate a multinomial logit duration model including different measures capturing the economic incentives embedded in the social security system. Our results show that social security incentives determine individual retirement decisions. Besides, partial retirement legislation modifies the selection of retirement routes and affects the age of retirement moving it to an earlier date.

Abstract: The presentation of scalp metastases from leiomyosarcoma of the vena cava is an extremely infrequent event. There are no other publications that describe such finding and very few of leiomyosarcoma in vessels. About this event we have reviewed the English literature describing studies on scalp metastases and skin metastases in general: their incidence, origin, clinical appearance, meaning, and diagnosis. The case we describe would be the second one presented worldwide because, as far as we know, it has been only one more published in 2005.
Historia de la vegetación en relación con la evolución geomorfológica de las llanuras costeras del este de la provincia de Buenos Aires durante el Holoceno

Abstract: changes in vegetation communities of the eastern coastal plains of buenos aires province during the holocene were reconstructed based on the comparison of palynological records from two radiocarbon dated cores located north (t9-1, 36° 45' s; 56° 37' w) and south (la lagunita, 37° 43' s; 57° 21' w) of villa gesell headland. between 5,400 and 3,300 14c yr b.p. pollen records suggest the development of similar mosaics of halophytic-psammophytic communities at both sites, on tidal plains related to littoral barriers prograding simultaneously to the north and south of the villa gesell headland during the regressive event when sea level was stabilized. the poaceae/chenopodiaceae index (pci) fluctuations suggest environmental instability as a consequence of barriers changes. from ca . 3,300 14c yr b.p. vegetation communities changed and differentiated at both sides of villa gesell headland simultaneously with the stabilization of the environments during the sea level regression, as it is reflected by a change in pci with similar values and sign. to the north, at ca . 3,300 14c yr b.p. the mosaic of halophytic-psammophytic communities from environments associated to the barrier was replaced by a community placed on stabilized sand dunes associated with the advance toward the north of beach ridges of the barrier. to the south, at ca . 2,500 14c yr b.p. halophytic communities dominated by chenopodiaceae were established reaching a higher development in a sheltered salt marsh and coastal lagoon environment, delimited by the rapid southwest progradation of the barrier.
