Abstract: a literature review of the most important elements of acute hepatic failure syndrome was made. the present state of the terminology and definition of the syndrome as well as some historical antecedents were examined. the principles supporting the clinical diagnoses, with emphasis on the hepatic encephalopathy as main syndrome were summarized. reference was made to the pathogenic mechanisms explaining the occurrence of hepatic failure and their characteristic clinical elements, the complications and the support measures, which are the basis of the treatment. likewise, criteria for liver transplantation, other therapeutical choices and the elements related with the mostly fatal prognosis of this disease were provided.

Abstract: Se realizó una revisión bibliográfica de los elementos más importantes del síndrome de insuficiencia hepática aguda. Se examinó el estado actual de la terminología y la definición del síndrome, así como algunos aspectos de los antecedentes históricos. Se efectuó una síntesis de los principios en que se sustenta el diagnóstico clínico, con énfasis en la encefalopatía hepática como síndrome fundamental. Se hizo referencia a los mecanismos patogénicos que explican la instauración de la insuficiencia hepática y los elementos clínicos que la caracterizan. Se exponen las complicaciones y las medidas de sostén que constituyen la base de su tratamiento. Se muestran los criterios para la indicación del trasplante hepático, así como otras opciones terapéuticas y se brindan los elementos que tienen que ver con el pronóstico casi siempre fatal de esta enfermedad. Abstract: Se realizó una revisión sobre los antecedentes históricos de la hepatitis viral aguda vinculados a los distintos conflictos bélicos en los que ha estado involucrada la humanidad. Se hizo una síntesis actualizada de los aspectos más importantes acerca de los virus, la clínica, la epidemiología y el diagnóstico de la hepatitis viral aguda. Se mostraron los elementos más relevantes referentes al comportamiento y manejo de la entidad para la medicina militar contemporánea. Abstract: this necroinflammatory liver disease of unknown origin was reported for the first time in 1950. it was mainly described in young women. this nosological entity has had several denominations through history. at present, it is known as autoimmune hepatitis. it appears in individuals with certain genetic predisposition when they are exposed to some environmental agent that has not been identified yet, giving rise to an autoimmune mechanism directed to the liver, where a necroinflammatoy process takes place leading to fibrosis and, finally, to cirrhosis. there are 3 types of autoimmune hepatitis: type1 is characterized by the presence of antinuclear antibodies, smooth antimuscle and antiactin; type 2, by the existance of anti- liver/kidney chromosome antibodies; and type 3, by the appearance of antibodies against the soluble antigen. the diagnosis is based on the histological study of the liver and on the presence of the above mentioned antibodies. monoclonal hypergammaglobulinemia is a laboratory alteration that characterizes the disease. other lab findings were the elevation of aminotranferases, alkaline phosphatase and bilirubin. the response to the treatment with glucocorticoids is common in this affection. azathioprine associated with corticosteroids is used in the maintenance therapy in order to prescribe low doses and to avoid their side effects. azathioprine may be also used alone as a maintenance therapy.
