Perspectives on access to in vitro fertilization in Portugal

Abstract: objective: to analyze users' reasons for choosing in vitro fertilization treatment in public or private services and to identify their suggestions for improving fertility treatment. methods: a qualitative study using an interpretative approach was conducted. fifteen semi-structured interviews were conducted with patients undergoing in vitro fertilization treatment (nine women, one man and five couples) at home or at their workplace in the districts of viana do castelo, braga, porto and lisbon, portugal, between july 2005 and february 2006. results: users evaluated access to in vitro fertilization treatment in public and private services based mainly on their individual experiences and called for more access to less costly, faster and friendlier care with suitable facilities, appropriate time management and caring medical providers. these perceptions were also associated with views on the need for fighting stigmatization of infertility, protecting children's rights and guaranteeing sustainability of health care system. interviewees sought to balance reduced waiting time and more attentive care with costs involved. the choice of services depended on the users' purchase power and place of residence and availability of attentive care. conclusions: current national policies on in vitro fertilization treatment meet user's demands of promoting access to, and quality, availability and affordability of in vitro fertilization treatment. however, their focus on legal regulation and technical-scientific aspects contrasts with the users' emphasis on reimbursement, insurance coverage and focus on emotional aspects of the treatment. the study showed these policies should ensure insurance coverage, participation of user representatives in the national council for assisted reproductive technology, promotion of infertility research and certification of fertility laboratories.

Abstract: in times of deep changes in the education field, the proposition of this study is to contribute to the reflection on the nursing teaching process by approaching one of the present-day pedagogical branches based on the constructivism: the pedagogy of the competences. thus, it was done a bibliographic study using the references of contemporaneous experts who defend the constitution of competences to teach, according to what was discussed by educators in the "3o international education seminar". it describes the changes of paradigms between the traditional education and its overcome, it defines and presents the competences to teach and it points out the proposition to work with projects to achieve the constitution of competences.
Agriculture and short run macroeconomics

Abstract: a short run macro-model is formulated to derive the interactions between farm and nonfarm sector in response to stabilization policies. exagenous variables are changes in fiscal and monetary policies, exchange rate, and international prices. endogenous variables explicitly analyzed are farm and nonfarm real incomes and nominal and relative prices. main results of the model are: (a) relative prices tend to change when exogenous variables change; (b) farm output and relative price tend to be reduced by expansive fiscal and monetary policies even if income elasticity of demand for farm products is zero; (c) although the inflationary effect of expansive monetary or fiscal policy is higher the lower the elasticity of supply of farm products, farm nominal prices tend to increase at most a much as nonfarm nominal prices. the effects of several different assumptions regarding supply and demand elasticities upon the model results are derived.

Abstract: the experience of the curricular training in a mental health work attendance to out-patients, caps, lead to this study development in the attempt to understand and characterize interdisciplinary team in this institution, as well as to understand the admittance of a nursing student in this team. the analysis of the replies disclosed that in the reports is found the concept of interdiscipline as well as of the multidiscipline (work in compartments). the conception with has of the model of assistence and of the admittance of the project in it is compatible with the conceptions that establish the description of the work: flexibility, the projects inter-relation the enlarged practice and the psychosocial rehabilitation. the fact that the service has a program of lecturing- assistence integration, "naturalizes"and validates the participation of a nursing student in the projects of assistance or sociability.

Abstract: the objetive of this study is to offer a space for the fourth-year students of the nursing school to express their experiences. a space was put in operation by the use of an operative group technique, and the subjects of the study,. centered in the task, exchange experiences and evaluate their participation in the groups. to understand the students' experiencé, a qualitative study with the research-action method was produced. the problematic of the study involved the concept that learning is a socialization process which demands changes and mobilize the novices' feelings. the operative group worked as a continent for the students' experiences in which they could explain their feelings. such feelings, mobilized on their graduation process, were impossible to be ignored the nursing teacher.

Abstract: this article puts in context and evaluates the historical and political process of the brazilian psychiatric reform. the goal is to provide a brief retrospective and analysis of the management mechanisms that have permitted the psychiatric reform to advance. this historic process is divided into three periods: the implementation of deinstitutionalization, the expansion of the psychosocial support network, and the consolidation of the reformist hegemony. it is verified that the brazilian psychiatric reform advances as management mechanisms are created to increase the service network. nonetheless, consolidating the territorial support network and increasing the resources of the annual mental health care budget of the national health system remain a challenge.

Abstract: this qualitative study had the purpose to increase the discussions about the constitution of competences in the education of nurses so that they can work in the mental healthcare area. goals: to analyze the representations of the research subjects about mental healthcare competences. method: qualitative research. setting: the nursing department in a public university in the state of s?o paulo. subjects: professors and healthcare nurses working in the same area. the mobilization of testimonies happened in focus groups, followed by discourse analysis. the building of competences and promotion of complex situations in the students' learning process were discussed, and the discourse analysis yielded the following empirical categories: the concept of competence, what is a complex situation, which knowledge is necessary to manage complex situations in psychiatric nursing and mental healthcare, competence: knowing how to manage a complex situation.
Problematizando o processo ensino-aprendizagem em enfermagem em saúde mental

Abstract: the article shows a research project which discusses the graduation program within mental health field, specially the nursing course. we admit that it is necessary to improve studies concerning to new pedagogical tendencies, aiming to overcome the traditional education model which privileges the knowledge transference as a purpose itself, the little flexibility of curriculums and the professors and students stiff performance, factors that prevent the critical-reflexive development of professionals. before this problem, we searched on competencies pedagogy, a theoretical support for necessary changes in the sense to improve and update the education-learning process. we suggest a dialogue between practice and assistance education on psychiatric and mental health nursing, on which teachers and nurses may indicate a way to identify and build necessary capacities to enable nurses to act with efficacy in real situations present in mental health.
Screening South African potato, tomato and wheat cultivars for five carotenoids

Abstract: In South Africa malnutrition is of great concern. Vitamin A deficiency is one of the leading causes of infections as a result of micronutrient malnutrition. Although supplementation and food fortification programmes exist, these either are not available or are unaffordable to communities in remote rural areas. The selection of crops that are naturally rich in provitamin A (β-carotene) and other carotenoids that can be recommended to small-scale farmers for breeding and for food production, could be an effective way to address vitamin A deficiencies and associated diseases. The aim of this study was to profile two cultivars each of potato, tomato, bread wheat and durum wheat, which are highly consumed crops in South Africa, for their carotenoid content using high-performance liquid chromatography. To this effect, reliable extraction and quantification of five carotenoids – lutein, zeaxanthin, canthaxanthin, β-carotene and lycopene – were performed for these crops. Lutein and zeaxanthin were found to be the major carotenoids in potato, whilst lycopene was the major carotenoid in tomato. In durum wheat, only lutein and zeaxanthin were identified whilst bread wheat contained lutein, zeaxanthin and β-carotene. The methodology used proved to be robust and suitable to screen a large number of potato, tomato and wheat cultivars for their carotenoid content.
