Síndrome metabólico en familiares de primer grado de pacientes con diabetes mellitus tipo 2 Metabolic syndrome in first degree relatives of patients with diabetes mellitus 2

Abstract: Se realizó un estudio descriptivo y transversal de 340 personas divididas en 2 grupos, con y sin antecedentes patológicos familiares de diabetes mellitus tipo 2 (APF de DM2), con el objetivo de detectar si las que presentaban APF de DM2 tenían más hiperinsulinemia, presencia de síndrome metabólico, factores de riesgo cardiovascular asociados y lesión de órganos diana, expresados en hipertrofia del ventrículo izquierdo de los sujetos que no tenían dicho antecedente. Se obtuvo como resultado que la hiperinsulinemia estuvo presente en 96 pacientes con APF de DM2 (56,5 %) y una significación de p = 0,001. El síndrome metabólico fue más frecuente en las personas con APF DM2 (90) para un 52,9 % y una significación de p = 0,000. La hipertrigliceridemia fue el marcador por excelencia del síndrome metabólico y el factor de riesgo que con más significación estadística se asoció a los sujetos con APF de DM2 (96: 52,9 % y una significación de p = 0,000). La hipertrofia ventricular izquierda (HVI) se asoció como expresión de lesión de órganos diana a los sujetos con APF de DM2 (36: 21,2 % y una significación de p = 0,043). Concluimos que los sujetos con APF de DM2 son más propensos a tener hiperinsulinemia, al síndrome metabólico, a factores de riesgo cardiovascular y a lesión de órgano diana en comparación con los que no tienen dicho antecedente. Abstract: The pulmonary artery aneurysm is an anomalous dilation of the trunk of this artery or one ofits main branches. The presence of pulmonary hypertension and processes that affect thevascular wall are the phisiopathological premises for their development. It is not a very frequentillness, of multiple aetiology, that can established without symptoms, with unspecificsymptoms or just start with complications; as rupture of the aneurysm, compression of nearstructures or thrombus formation. Their natural history is not known and it is thought that itsrelated to the causes that give them origin. In this paper we present two patients with pulmonaryartery aneurysm, secondary to severe pulmonary hypertension in the course of aninterauricular communication, complicated with thrombus adhered to the walls of the pulmonaryartery. The diagnosis was carried out by transthoracic and transaesophagic echocardiography.

Abstract: authors present an infrequent case of a female patient with abdominal tumor a great superficial collateral circulation type cava-cava, the abdominal ultrasound, the transthoracic and transesophageal echocardiogram and the computerized axial tomography (cat) identified a tumor of the vena cava inferior. the fine-needle aspiration cytology of lesion confirmed the presence of primary fusopleomorphous sarcoma of high degree of malignancy of vena cava inferior.

Abstract: by taking statistics as an object of study (rather than a means of analysis) and taking a historical perspective from within the theoretical framework of the sociology of statistics, topics and sources are put forward for possible historical research. the topics and sources suggested here are a development of the approach adopted in the four-volume collection história das estatísticas brasileiras: 1822-2002 brought out by the instituto brasileiro de geografia e estatística. ultimately, the aim is to write a history of brazil from the perspective of the purpose and practice of statistics, or in other words, their use in the practice of government.

Abstract: recreated in the early days of the brazilian republic (jan./1890) as a central agency, the general directory of statistics (dge) faced difficulties in consolidating the brazilian statistical activity. a new time only was achieved when josé luiz say?o de bulh?es carvalho (1866-1940), a medical doctor specialist in public health dedicated to demographic studies and researches, took its direction (in two times) for almost 17 years. his performance applied to facilitate the interaction of dge with its similar agencies in the federation. he idealized agreements with provincial statistical agencies; imagined counsels (or committees) for making collective decisions; imagined a national statistical conference based on the experience he has accumulated in two conferences of the international statistical institute (1s1); applied attention to the formation of a statistical professional community; encouraged and sponsored the translation of books, among other measures. the census of1920, he had made, the only major census at the early brazilian republic, was guided by friendly relations with the federal and provincial statistical organizations and with the society (catholic church, media, unions, clubs etc.). 1t was, we can say, a modern census, using processing machines, and concluded in a timely fashion. the printed volumes of results were illustrated by pictorial graphics, also used at the pavilion of statistics, called (by the press) the pavilion of precisely science, which fixed the presence of dge in the 1nternational exhibit of the brazilian 1ndependence centennial (1822-1922). with his experience, anticipated the basis of brazilian 1nstitute of geography and statistics (1bge), created in 1936 as the new brazilian statistical central agency (still working with growing success), that can be see, in sum, as a measure of his success in a long term vision.

Abstract: focusing on the statistical information, this article aims at remarking the polemics that grow around its social-political dimension which defines the demand, and technical-scientifical dimension which defines the suplly, through a historical evolution. it aims at showing that the definition and harmony between both, the demand and the suplly of statistical information, are not fixed by the market free game, natural locus of the spontaneous economical coordination, so, being absolutely necessary to have a specialized, proper and adequately institutionalized coordination in some official government level (because it comes to an output wanted as public). therefore this institutional coordination should be up to some public agent which would act to the limits of its attributions, cosequently practicing a careful regulation of the whole social and cognitive process, based on the principles of a policy of information. in this way, the policy of information is related to the settling of polemics that may obstruct or make it difficult to turn public a certain output (in this case the statistical information), relating it in the context of some power agency (in this case a specialized coordination), establishing a six-pillar set (utility, properly, validity, visibility, continuity, integrity), able to sustain and support it.

Abstract: this text analyzes the need to control or administer information so that it can best serve the public and the public sector. achieving this will require a clearly defined policy and governing regime. to ensure the harmonization and synchronization of supply and demand with regard to statistics, the author calls for the creation of a coordinating institution, the precise nature of which would be defined by information policy makers.
