Aplicaciones clínicas del estudio objetivo de los ruidos respiratorios en pediatría

Abstract: breath sounds research has been very active over the past few years, with many groups working in the field. the objective of this paper was to review the nonemclature, equipment used and the clinical applications of this methodology. along with normal lung and traqueal sounds, a review of wheeze, crackles, snoring and stridor characteristics are presented. the main clinical applications are: evaluation of the upper airway anatomy and lung function studies, obstructive slep apnoea screening, evaluation of recurrent wheezy infants and broncho-provocation challenge testing in preschool children, regional ventilation studies and distance breath sound transmission for diagnostic and teaching purposes. in the future, smaller and cheaper instruments will be developed, that together with spirometric lung testing will give valuable information in childhood pulmonary studies

Abstract: this article states that the incorporation of ethical issues such as bioethics and principles of good practice, facilitates the academic management in a faculty of medicine. it presents some interesting aspects necessary to carry out a quality academic management that serves the university mission and its educational and research project. it underlines good practices regarding not only professional relations but also organizational aspects. it includes an analysis of academic management and its association with quality and leadership, it reflects on the ethical values of quality work and on the theory of functional or interactional leadership, a key element to boost the development of a university health institution.

Abstract: an important challenge for medical schools is to recognize and stimulate the academic career of its clinical faculty. these professionals are mainly dedicated to teaching activities and play a role in the modeling of its students. therefore, they contribute to the development of medical education. the aim of this paper is to analyze the main features of this particular kind of academics and to stimulate physicians to embrace the teaching career. the development channels of teaching academic activity are direct teaching, curricular innovation and administration, counseling, clinical investigation, education administration, leadership and assessment of the learning process. we define each of these activities, giving examples of the activities that can be carried out and how to assess them.

Abstract: El estudio de los ruidos respiratorios ha concitado el interés de numerosos grupos de trabajo en los últimos a os. El objetivo de esta revisión fue actualizar la nomenclatura, los equipos utilizados para la realización del estudio, y especialmente evaluar lo que se ha publicado en relación a la utilidad clínica de esta metodología. Junto con la descripción de los ruidos pulmonares y traqueales normales, se revisan las características de las sibilancias, crepitaciones, ronquido y estridor laríngeo. Dentro de las aplicaciones más importantes, se pueden describir: la evaluación anatómica de la vía aérea superior, diagnóstico de apnea obstructiva, apoyo en el diagnóstico diferencial de la obstrucción bronquial en lactantes y en preescolares durante la prueba de provocación bronquial, el estudio de la ventilación pulmonar regional y la transmisión de se ales acústicas a distancia que pueden ser muy útiles desde el punto de vista asistencial y docente. En el futuro probablemente la tecnología desarrollará nuevos instrumentos, con menor costo, adaptados a las pruebas clásicas de función pulmonar Breath sounds research has been very active over the past few years, with many groups working in the field.
