Abstract: the purpose of this study was to identify and analyze the tendencies and types of studies published in brazil and abroad, involving elders aged >80 years, living in the community. a systematic review of national literature was performed using the lilacs and scielo databases, and pubmed and embase for international literature, covering publications of the last two decades. twelve national and 162 international references were selected. biological sciences were the prevalent area both at the national (50%) and international (74.1%) levels. all national studies were observational, 91.7% of which were cross-sectional. of the international studies, 93.3% were observational, 48.1% of which were cross-sectional and 37.6% were cohort studies. the united states were the country responsible for 41.4% of all international publications. brazil and china were the only developing countries with international publications. despite the significant number of international scientific publications as of 2005, this fact has not been observed at the national level.

Abstract: this study aimed to describe and evaluate the national elderly care policy in brazil and how it relates to nursing. a bibliographic review identified that this policy is published in the form of laws, decrees, texts, and articles in periodicals and books. the social actors involved in the national elderly care policy are the elderly, their families, the community, and the health team, especially the nursing team. we conclude, based on the literature analysis, that this policy has provided the social actors involved an awareness of social justice and full guarantee of elderly rights. it was also concluded that the insertion of nursing in the national elderly care policy involves the care process within the life cycle and in several levels of health care, including the elderly health, and participation in the elaboration and implementation of this policy as well.

Abstract: A finalidade deste estudo foi identificar a dependência funcional de idosos e a sobrecarga do cuidador. Trata-se de estudo epidemiológico e transversal realizado em 2009 com 574 idosos e 124 cuidadores em Ribeir o Preto-SP, por meio dos instrumentos MIF e Escala de Sobrecarga de Zarit. Entre os idosos, a maioria era do sexo feminino (67,8%), com média de 76,6 anos, baixa escolaridade (54,7%) e renda individual mensal de R$ 942,20. Apenas 15,7% foram identificados como dependentes. Dos cuidadores, 85,6% era do sexo feminino, com média de 56,5 anos e 90,3% eram familiares (filhas ou esposas). A média de sobrecarga dos cuidadores foi de 27,8 (±17,5). A dependência do idoso foi fator de risco para sobrecarga do cuidador (p<0,05). A abordagem preventiva e a interven o precoce devem ser prioritárias para essa popula o. S o necessárias a vigilancia de uma equipe multidisciplinar de saúde, a aplica o de instrumentos de avalia o do comprometimento da funcionalidade e a interven o para preven o da sobrecarga dos cuidadores. Se objetivó identificar la dependencia funcional de ancianos y la sobrecarga del cuidador. Estudio epidemiológico, transversal, realizado en 2009 con 574 ancianos y 124 cuidadores en Ribeir o Preto-SP, mediante los instrumentos MIF y Escala de Sobrecarga de Zarit. Entre los ancianos, hubo mayoría femenina (67,8%), media etaria de 76,6 a os y baja escolarización (54,7%), renta mensual individual promedio de R$942,20. Sólo 15,7% fueron identificados como dependientes. Entre los cuidadores, 85,6% era de sexo femenino, media etaria de 56,5 a os, 90,3% eran familiares (hijas o esposas). El promedio de sobrecarga fue de 27,8 (±17,5). La dependencia del anciano constituyó factor de riesgo para sobrecarga del cuidador (p<0,05). El abordaje preventivo y la intervención precoz deben priorizarse en este segmento. Son necesarios el apoyo de un equipo de salud multidisciplinario, la aplicación de instrumentos de evaluación del compromiso de la funcionalidad e intervención para prevenir la sobrecarga de los cuidadores. This study investigated the functional dependency of aged individuals and caregiver burden. Abstract: objective: to identify demographic and socioeconomic differentials associated with the health status of oldest-old individuals living in two cities of different brazilian regions. methods: a comparative and cross-sectional epidemiological study was conducted with the oldest-old (> 80 years), living in the cities of ribeir?o preto (rp, southeastern brazil) and caxias do sul (cs, southern). the probabilistic sample included 117 individuals in cs and 155 in rp, and data were collected between 2007 and 2008. the instrument included demographic and socioeconomic data, mini-mental state examination, functional independence measure, number of self-reported comorbidities and geriatric depression scale. results: mean age was similar, with predominance of women (~70%) and widowed individuals (~60%) in both cities. mean level of education did no differ statistically, although mean income was higher in rp than in cs (p = 0.05). rp showed a higher concentration of individuals in the extreme levels of education and income than that of cs. mean score of the mini-mental state examination was similar in both groups and higher among men, individuals aged between 80 and 84 years, married and with a higher level of education. better functional performance was observed in elderly individuals aged between 80 and 84 years in both cities, in those with higher level of education in rp; and in males and married individuals in cs. elderly individuals in cs showed higher number of comorbidities than those in rp (p < 0.001). male elderly individuals, married and with -higher income level showed fewer depressive symptoms in both groups; and those in rp showed higher geriatric depression scale score than the others in cs (p < 0.001). conclusions: although the oldest old in cs showed lower socioeconomic inequality and fewer depressive symptoms, they also had a higher mean number of comorbidities and lower level of functional independence, when compared to those in rp.

Abstract: the aging context of the elder elderly needs to be taken into account, due to the redefinition of family as a social institution. this is an epidemiological, cross-sectional study that characterized the household arrangements of 147 elder elderly living in ribeir?o preto, sp, through home interviews. the age range between 80 and 84 years predominated. the majority receives a retirement benefit ranging from one to three minimum wages, and owns their house. on the average, they have 4.08 children and live with 2.8 people. most men live with their partners, while women live by themselves. the elderly head a large part of households. the most frequent arrangements are families with two and three generations. regarding the household arrangement, there was equivalence in the results between the ones that moved into the house of the elderly and cases when the elderly moved into the family’s house, reinforcing the family as a protector and caregiver of the elderly.

Piecewise linear regularized solution paths

Abstract: We consider the generic regularized optimization problem $\hat{\mathsf{\beta}}(\lambda)=\arg \min_{\beta}L({\sf{y}},X{\sf{\beta}})+\lambda J({\sf{\beta}})$. Efron, Hastie, Johnstone and Tibshirani [Ann. Statist. 32 (2004) 407--499] have shown that for the LASSO--that is, if $L$ is squared error loss and $J(\beta)=\|\beta\|_1$ is the $\ell_1$ norm of $\beta$--the optimal coefficient path is piecewise linear, that is, $\partial \hat{\beta}(\lambda)/\partial \lambda$ is piecewise constant. We derive a general characterization of the properties of (loss $L$, penalty $J$) pairs which give piecewise linear coefficient paths. Such pairs allow for efficient generation of the full regularized coefficient paths. We investigate the nature of efficient path following algorithms which arise. We use our results to suggest robust versions of the LASSO for regression and classification, and to develop new, efficient algorithms for existing problems in the literature, including Mammen and van de Geer's locally adaptive regression splines.
