Dabigatran etexilate for prevention of cardioembolic complications in nonvalvular atrial fibrillation: how to do the intervention more effective and safer

Abstract: Application of dabigatran etexilate for prevention of cardioembolic complications in nonvalvular atrial fibrillation is considered. Results of randomized clinical trials on efficacy and safety of dabigatran in various doses are discussed. Besides, a comparison of dabigatran and varfarin therapies is presented taking into account pharmacoeconomic data. Recommendations on dabigatran dosing are considered to do intervention more effective and safer.

Abstract: Aim. To analyze the structure of expenses associated with warfarin dose adjustment and international normalized ratio (INR) control for patients with atrial fibrillation (AF) per 1 year of treatment.Material and methods. AF prevalence rate was calculated according to the large population studies data. The study was performed based on methodology for Cost of Illness evaluation. Expenses associated with INR monitoring within the obligatory medical insurance program were calculated based on the general tariff agreement for 2011 year for one of the territorial subject of Russia. Expenses associated with INR monitoring in outpatient departments of university clinics and commercial laboratories were calculated based on price list of respective institutes. Expenses associated with INR monitoring in case of self-monitoring were calculated based on expenses for portable coagulometer and test strips purchasing.Results. Around 539 000 people in Russia are needed warfarin therapy and associated continuous INR monitoring. In the obligatory medical insurance program the cost of warfarin is less than 1% of total costs and the direct costs of treatment and control of nearly 4 058.12 RUR/person per year, in university clinics, respectively, 0.03%, and 13 019.05 RUR, in case of self-monitoring - 0.01-0.05% and from 40 405.00 (in the first year) to7 404.98 RUR (in the consequent years), respectively.Conclusion. The cost of warfarin, regardless of the INR monitoring approach is less than 1% of the total cost of the treatment with therapy efficacy and safety control.

Abstract: According to recent guidelines, oral dabigatran etexilate is indicated for stroke and systemic embolism prevention in patients with atrial fibrillation (AF).Aim. Based on the RE-LY study to evaluate the cost-effectiveness of dabigatran etexilate versus warfarin prescribed in “real-world” settings from a Russian payer perspective.Material and methods. Markov model simulated AF patients at moderate to high risk of stroke while tracking clinical events and resulting functional disability. Acute event costs and resulting long-term follow-up costs incurred by disabled stroke survivors were calculated using general tariff agreement of Russian obligatory health insurance system and official national statistics. Clinical events, summarized as events per 100 patient-years, expected life years, total costs, and incremental cost effectiveness ratios (ICER) were calculated.Results. Over a lifetime, dabigatran etexilate treated patients experienced fewer intracranial haemorrhages and fewer ischaemic strokes. ICER of dabigatran etexilate was 461,602 roubles per one additional life year versus “real-world” warfarin.Conclusion. This study demonstrates that dabigatran etexilate is a cost-effective alternative to current care for the prevention of stroke and systemic embolism among Russian patients with AF.
Public health importance of lassa fever epidemiology, clinical features and current management review of literature

Abstract: The public health importance of Lassa fever can not be over emphasized if one considers the high infectivity and mortality rates associated with the disease. This study dealt extensively on the epidemiology, clinical features and current management of Lassa fever through literature review. The aim of this study is to sensitise the public on what it needs to know on Lassa fever as well as updating the knowledge of health workers on current management of the disease and important precautive measures to take when handling a patient with Lassa fever. Strict barrier nursing, isolation, use of protective devices are important preventive measures when managing a patient with Lassa fever infection. As Lassa fever may have a long incubation period (Up to 20 days), it is possible that travellers from endemic areas may be incubating the disease. However, one case of Lassa fever entering a non-endemic area should not cause fear of an epidemic as long as correct infection control procedures are followed. (Af. Abstract: In the article the results of tests of loams by static, dynamic and cyclic loads are given.Simple shift (cutting) is typical for soil deformation in natural conditions. At an increase of tangential stresses up to a limiting value the simple shift is completed by exhausting their strengths with break or without break of material uniformity. Shear strength is the basic strength parameter of soils in general and clay soils in particular. The estimation of shear strength of clay soils by tests in conditions of simple shift by static, dynamic and cyclic loads was considered. The diagrams of limiting state confirming the influence of low-frequency vibration on strength characteristics of cohesive soils were obtained.

Abstract: Experiment has been supplied on 20 white rats-males in mass of 140-160 g to whom under a skin the tumoral strain of a Guerin’s carcinoma has been transplanted. The purpose of the real research was studying distant tumour influences on histotopography structural components of a thyroid gland. Distant influence of a tumour on a thyroid gland calls essential rearrangements of a stroma and a parenchima of an organ which are accompanied bydepression of functional activity thyro-cyties and reduction of quantity of haemocapillars of a stroma of an organ.

Abstract: The experimental research has been supplied on 70 white rats - males in mass of 140-160 to whom under a skin the tumoral strain of a carcinoma of Guerin has been transplanted. The operation purpose was learning of influence exogenous homologous tumoral RNA on a morfo-functional status of adrenals at animals with tumor. Tumour growth causes a strain of compensator mechanisms of adrenals at cellular level and is accompanied by response from a microcirculatory channel. Introduction exogenous homologous tumoral RNA an animal locks development of reactive changes in a parenchima and an organ stroma.
Proliferative activity of Guerin’s tumour cells under the action of homologic tumour rhibonucleic acids

Abstract: Ability of exogenous rhibonucleic acids (RNAs) to penetrate into cells and to cause the phenotypic transformations, changing the character of metabolic process, was the reason for application of RNAs with the purpose of determined action on metabolism of tumour cells. The aim of our research was to study the influence of homologic tumour RNAs on proliferative activity of Guerin’s carcinoma cell. We used the transplantable tumor culture of Guerin’s carcinoma. RNAs was extracted by the method of thermal phenol fractioning. Radiolabeled nuclei index decreased for 40% after exogenous homological tumour RNAs treatment. It was accompanied by the decrease of packing tissues index for 15%. Thus, on the basis of analysis of experimental material we can conclude that exogenous homological tumour RNAs causes the decrease of DNA-producing cells number with the increase of atipism degree of Guerin’s carcinoma cells.
Assessment of Medical Waste Incinerator Performance Based on Physical Characteristics of Ashes  [PDF]

Abstract: The aim of this study was to assess the performance of the combustion process during medical waste incineration by studying physical properties of the ashes produced. Combustion characteristics data including maximum temperatures, total weight of waste loaded, weight of ashes, weight reduction, sieve analysis and particle size distribution were determined experimentally. The test runs were conducted in a newly installed incinerator at Temeke district hospital. The average maximum temperatures achieved in the primary chamber was 397.8℃and 839℃ for secondary chamber with average incineration cycle time of 99 minutes. These temperatures were lower compared to the design temperatures of 650℃ and 950℃ as a result of loading wet waste. The ash samples were collected under the incinerator grate by randomly sampling the ashes for each run after weighing the total ash. The particle size distribution of ashes observed was not uniform due to presence of non-combustible materials in the sharps waste. However, the fineness modulus ranged between 2.0 and 4.0, which is in the acceptable range. From the above results it was concluded that, the incinerator performance was high in terms of the parameters assessed. To improve the incinerator performance further, it was recommended that the medical waste should be stored in a dry place away from rain.

Abstract: anodic electrooxidation of iodide in 0.1 mol l-1 tbap/ ch2c12 was studied electrochemically via cyclic voltammetry, convolution transforms voltammetry, chronoamperometry and chronopotentiometry techniques at a glassy carbon electrode (gce) at various temperatures ranging from -20 °c to 21 °c. it was found that at less positive potential triiodide (i-3) is formed followed by a moderate fast chemical process, while at more positive potential, the triiodide is oxidized to iodine (i2) followed by another chemical process, i.e ec1ec2 scheme. the effect of lowering the temperature on the heterogeneous electron transfer rate constant and the diffusion coefficient was discussed. the relevant chemical and electrochemical parameters of the electrode reaction were determined. the accurate test of the parameters evaluated experimentally was verified by comparing the experimental voltammograms with the simulated one.
