Experience with Early Postoperative Feeding after Abdominal Aortic Surgery.

Abstract: Background: Abdominal aortic surgery is a form of major vascular surgery, which traditionallyinvolves long hospital stays and significant postoperative morbidity.Experiences with transit ileus are often encountered after the aortic surgery.Thus traditional postoperative care involves delayed oral feeding until thepatients regain their normal bowel activities. This report examines the feasibilityof early postoperative feeding after abdominal aortic aneurysm (AAA)open-repair.Methods: From May 2002 through May 2003, 10 consecutive patients with infrarenalAAA who underwent elective surgical open-repair by the same surgeon inour department were reviewed. All of them had been operated upon andcared for according to the early feeding postoperative care protocol, whichcomprised of adjuvant epidural anesthesia, postoperative patient controlledanalgesia, early postoperative feeding and early rehabilitation. The postoperativerecovery and length of hospital stay were reviewed and analyzed.Results: All patients were able to sip water within 1 day postoperatively without trouble(Average; 12.4 hours postoperatively). All but one patient was put onregular diet within 3 days postoperatively (Average; 2.2 days postoperatively).The average postoperative length of stay in hospital was 5.8 days. Nopatient died or had major morbidity.Conclusions: Early postoperative feeding after open repair of abdominal aorta is safe andfeasible. The postoperative recovery could be improved and the length ofstay reduced by simply using adjuvant epidural anesthesia during surgery,postoperative epidural patient-controlled analgesia, early feeding, earlyambulation, and early rehabilitation. The initial success of our postoperativerecovery program of aortic repair was demonstrated.
Successful Treatment of Tracheal Mucoepidermoid Carcinoma Using Rigid Bronchoscopy Combined with Conventional Surgical Resection

Abstract: Mucoepidermoid carcinoma of the trachea is a rare tumor. This investigation reports ona case of a 67-year-old male with mucoepidermoid carcinoma in the lower 1/3 of the trachea.The patient presented with intermittent coughing and hemoptysis lasting for 1 month.The preoperative investigation revealed an intraluminal polypoid mass in the posterolateraltrachea with 75% stenosis of the tracheal lumen. A rigid bronchoscope was used to evaluatethe airway before surgery, and the tumor was mechanically resected using the tip of the rigidbronchoscope just before intubation. Finally, the tumor was completely removed, and theairway was successfully reconstructed. At 6 months after surgery with no adjuvant chemoradiotherapy,the patient was free of disease.
Mechanical properties and microstructure of 3D-printed high Co–Ni secondary hardening steel fabricated by laser melting deposition

Abstract: The mechanical properties and microstructure of the 3D-printed high Co–Ni secondary hardening steel fabricated by the laser melting deposition technique was investigated using a material testing machine and electron microscopy. A microstructure investigation revealed that the samples consist of martensite laths, fine dispersed precipitates, and reverted austenite films at the martensite lath boundaries. The precipitates are enriched with Co and Mo. Because the sample tempered at 486°C has smaller precipitates and a higher number of precipitates per unit area, it exhibits better mechanical properties than the sample tempered at 498°C. Although the 3D-printed samples have the same phase constituents as AerMet 100 steel, the mechanical properties are slightly worse than those of the commercial wrought AerMet 100 steel because of the presence of voids.
Extracting TM Image Information Using Texture Analysis

Abstract: The maximum likelihood classification (MLC) is one of the most popular methods in remote sensing imag classification. Because the maximum likelihood classification is based on spectrum of objects, it cannot correctly distinguish objects that have same spectrum and cannot reach the accuracy requirement. In this paper, we take an area of Fengtai District of Beijing as an example and discuss the method of combining texture of high-resolution image with spectrum to improve the accuracy of TM image information extraction. Firstly, analysis of the textures of the in high-resolution imags is made by using texture analysis of Gray Level Coocurrence Matrices and selecting statistic index. Then threshold is selected and the optimal threshold is obtained according to entropy. Objects that have same spectrums such as vegetation land and cultivated land are distinguished using image segmentation in virtue of the optimal threshold. Finally, the find result is obtained through combining image segmentation with original classification. The finat result is compared with the classification results based on spectrum only or texture only. The result indicates that the objects with same spectrum are distinguished by using texture analysis in image classification, and the combination improves more than spectrum only or texture only in classification accuracy.
Quantum Thermal Transport through Extremely Cold Dielectric Chains

Abstract: In the framework of Green's function theory out of equilibrium, a Landauer--Buttiker (LB) formula for thermal conductance is derived. A simplified model for describing extremely cold dielectric chains is proposed for the first time. Further we apply the present LB formula for studying thermal conductance at low-lying modes, emerging in dielectric atom chains. We find that quantum thermal conductance undergoes an anomalous transition due to new quasiparticle excitations, resulting from nonlinear atom--atom interactions. This theoretical prediction is in excellent agreement with a high-accuracy measurement to thermal conductance quantum.

Pulmonary Lymphangioleiomyomatosis: A Clinicopathological Analysis of Ten Cases.

Abstract: Background: Lymphangioleiomyomatosis (LAM) of the lung is a very rare disease ofunknown etiology which occurs particularly in women of childbearing age.So far, there have been very few reports of LAM in Taiwan.Methods: Data on all patients with a clinical or pathological diagnosis of LAM inChang Gung Memorial Hospital from 1990 to 2001 were collected from thepathology files or by consultation with thoracic surgeons, chest physicians,and radiologists of this institution. Cases were confirmed by lung biopsy andhigh-resolution computed tomographic scanning. Clinical data were obtainedfrom patients' charts. Additional immunostaining for HMB-45 antigen, estrogenreceptor, and progesterone receptor was carried out for cases with availableparaffin blocks.Results: In total, 10 patients confirmed with a diagnosis of LAM and adequate clinicalfollow-up information were found from 1991 to 2001. All were femalesof fertility age. The follow-up periods ranged from 18 to 167 months. Sixpatients received hormonal therapy. According to the pulmonary functiontests and clinical symptoms, 6 patients were in stable condition and 4 showedslow progression of disease by the time of the last follow-up. Two patientsdied after lung transplantation and abdominal surgery for retroperitonealLAM, respectively, at another medical center.Conclusions: The clinicopathological features of our series are similar to those reportedfrom other countries. The clinical course was slowly progressive with no significantresponse to hormonal therapy. Lung biopsies are important for adiagnosis, but the size and location of the biopsy specimens greatly affect thediagnostic reliability.
Minimally Invasive Surgery for Coronary Artery Disease with Associated Lung Cancer

Abstract: Simultaneous surgical management of patients with co-existing ischemic heart diseaseand lung carcinoma remains controversial. Traditionally, these operations have been staged,with coronary artery revascularization first, followed by pulmonary resection at a later date.Under this procedure, tumor resection is delayed, and these staged procedures may increasemorbidity and cost. Our experience with minimally invasive surgery in the cardiac and thoracicfields suggests that both diseases can be resolved within the same operation. Thisreport presents a 65-year-old man with coronary artery disease (CAD), and left lung cancer.He received concomitant coronary artery bypass grafting for CAD and pulmonary resectionfor lung cancer via a left parasternal minithoracotomy. No complaints or clinical signs wereobserved during a 6 month follow-up. Therefore, major cardiac and thoracic procedures thatare performed via a minimally invasive approach using conventional instruments, can resultin a safer, quicker, and more economical procedure. The results of the a minimally invasiveapproach demonstrated the technical feasibility of treating a triple vessel cardiac diseasewith lung cancer.
Correlation of Anaplastic Lymphoma Kinase Overexpression and the EML4-ALK Fusion Gene in Non-Small Cell Lung Cancer by Immunohistochemical Study

Abstract: Background: Recently the echinoderm microtubule-associated protein-like 4-anaplastic lymphoma kinase (EML4-ALK) fusion gene with transforming activity was identified in non-small cell lung cancer (NSCLC). In addition, NSCLC patients with the EML4-ALK fusion gene had a dramatic response and longer progression free survival after ALK inhibitor treatment than those without this fusion gene. However, the incidence and clinical and molecular characteristics of the EML4-ALK fusion gene in NSCLC patients of Taiwan are still unclear.Methods:Sixty-four fresh frozen tumor specimens were obtained from the tissue bank of Chang Gung Memorial Hospital for RNA extraction and EML4-ALK fusion gene detection. Paraffin sections of lung tumors from all of these patients were available and were analyzed for ALK protein expression by immunohistochemical (IHC) study. The results were correlated with clinical and molecular biomarkers.Results: Three of the 64 tumors (4.7%) had the EML4-ALK fusion gene. Two were adenocarcinomas, and one was adenosquamous carcinoma. Twenty patients with non-squamous cell carcinomas had epidermal growth factor receptor (EGFR) mutations, so the EML4-ALK fusion gene was found in 14.3% of EGFR wild type non-squamous cell carcinomas. Two tumors were variant 3 (3a+3b with 3b predominant) and had strong staining (3+) for ALK by IHC stains. One tumor was variant 1 and had moderate staining (2+) for ALK. None of the ALK wild type tumors had strong staining for ALK. When compared with other clinical and molecular features, only the IHC stain for ALK was significantly correlated with the EML4-ALK fusion gene (p = 0.0002).Conclusions: ALK overexpression detected by IHC study could be a promising detection method for the EML4-ALK fusion gene and is worth further confirmation with more samples.
