Seguimiento de recién nacidos de término con encefalopatía hipóxica isquémica Follow-up of term infants with hypoxic-ischaemic encephalopathy (HIE)

Abstract: La encefalopatía hipóxica isquémica (EHI) continúa siendo una patología neonatal frecuente, sin embargo, no existen a nivel nacional publicaciones acerca de las secuelas neurológicas. Objetivo: determinar el tipo e incidencia de estas secuelas. Material y método: Se controlaron prospectivamente 100 RN de término con diagnóstico de EHI mediante evaluación neurológica y del desarrollo psicomotor. Resultados: Fueron catalogados como EHI leve 43 RN, lográndose seguimiento a 2 a os en 29 ni os (67%), en los cuales no se pesquisó secuelas neurológicas. Presentaron EHI moderada 52 RN, se controlan 45 ni os (87%) a dos a os y 40 a tres a os, siendo el porcentaje de secuelas 33%. Con EHI grave evolucionan 5 RN, 4 fallecen y uno sobrevive con secuelas severas. Conclusiones: No se pesquisó secuelas en los pacientes con EHI leve, un 33% de los pacientes con EHI moderada evolucionaron con secuelas y los casos con EHI severa fueron pocos y con alta mortalidad HIE continues to be a frequent neonatal pathology, however there does not exist at a national level publications about the neurological sequelae of HIE. Materials and method: using a prospective protocol 100 term infants with HIE were followed up with a psychomotor and neurological evaluation. Results: 43 newborns were classified as mild HIE, 29 were followed up for 2 years all were free from handicap. 52 were classified as moderate HIE, 45 completed 2 years of follow-up, 40 for 3 years, 33% had sequelae. 5 had severe HIE, 4 died and the other is severely handicapped. Conclusions: infants with mild HIE did not have sequelae, 33% of those with moderate HIE had sequelae and the cases with severe HIE were few and had a high mortality

Abstract: La asfixia neonatal es una causa importante de mortalidad y secuelas neurológicas. El objetivo de nuestro estudio es identificar los factores de riesgo maternos, perinatales y neonatales asociado a un Puntaje de Apgar (PA) bajo. Pacientes y Método: Se analizan los datos de 57 241 recién nacidos (RN), entre los a os 1997 y 2004; los antecedentes fueron ingresados en forma prospectiva a una base de datos computacional. Se realizó un análisis multivariado con un modelo de regresión logística para identificar los factores de riesgo. Resultados: 1 167 presentaron un PA £ 3 al minuto (2,1%) y 291 presentaron además un puntaje £ 5 a los 5 minutos (0,5%). Los factores de riesgo de PA bajo estadísticamente significativos fueron: RN de muy bajo peso, malformaciones congénitas mayores, RN peque o para la edad gestacional, RN pretérmino, embarazo de 42 semanas, expulsivo prolongado, fórceps, desprendimiento placentario y líquido amniótico con meconio. Conclusiones: En este trabajo se identifican los factores de riesgo de PA bajo en nuestra población, los cuales deben ser considerados para una adecuado manejo perinatal Background: Neonatal asphyxia is an important cause of mortality and neurological sequelaes. Conclusions: Risk factors of low AS are identified in our population, essential for an adequate perinatal care

Abstract: chile has achieved an important improvement in the indices of perinatal health, which has increased the impact of congenital malformations in perinatal and infant mortality. in order to maintain this reduction it is necessary to develop strategies to improve the mortality associated to congenital malformations. in answer to this new situation, it was created the "centro de referencia perinatal oriente" (cerpo). a multidisciplinary team of professionals. our objective is to work coordinated in order to improve the quality of attention and perinatal results of pregnant women of eastern santiago and regions, whose fetuses present a congenital malformation that will require of neonatal attention in high complexity pediatrics centers. in order to achieve the objectives, a model of integral, multidisciplinary attention was settled, with the purpose of satisfy their necessities. it is described in detail the model of attention "cerpo" and the activities made during this first year of work
