Comparison of the Results of Early and Delayed Inpatient Stroke Rehabilitation

Abstract: Objective: This study was designed to investigate the best time to start rehabilitation and to identify the predictors of functional outcomes after rehabilitation in patients with stroke after their first cerebrovascular accident.Materials and Methods: A total of 138 stroke patients who had their first stroke were divided into 5 groups according to the time elapsed from the cerebrovascular accident to the onset of rehabilitation (first 20 days, 21-40, 41-60, 61-80, and 81-100 days). Motor status of the patients was evaluated with the Brunnstrom Recovery Scale (BRS) and their functional status was assessed using the Functional Independence Measure (FIM) at admission and discharge. There was no statistically significant difference among the groups in terms of age, gender, localization of the lesion, etiology, and motor and functional status at baseline as well as additional systemic diseases. According to the results of rehabilitation, efficiency (average increase in FIM per day) and effectiveness (proportion of potential improvement achieved during rehabilitation of the groups) were calculated.Results: There was no statistically significant difference among the groups in terms of efficiency and effectiveness. Correlation analysis revealed that efficiency showed negative correlations with shoulder subluxation while efficiency showed a positive correlation with BRS scores of the lower extremities and effectiveness. However, effectiveness showed positive correlations with efficiency, baseline FIM scores, and BRS scores of the hands, arms, and the lower extremities while it was inversely correlated with shoulder subluxation and bladder incontinence.Conclusion: We concluded that starting stroke rehabilitation at any time within the first 100 days following the first stroke did not affect the results of rehabilitation. Therefore, we assume that starting stroke rehabilitation even after a delayed period also seems to be as efficacious as early rehabilitation. Turk J Phys Med Rehab 2013;59:7-12.
Coefficient bounds for new subclasses of bi-univalent functions

Abstract: In the present investigation, we consider two new subclasses N_{{\Sigma}}^{{\mu}}({\alpha},{\lambda}) and N_{{\Sigma}}^{{\mu}}({\beta},{\lambda}) of bi-univalent functions defined in the open unit disk U={z:|z|<1}. Besides, we find upper bounds for the second and third coefficients for functions in these new subclasses.

Abstract: Aim: To determine the success rates of lacrimal massage and probing according to age groups for congenital nasolacrimal duct obstruction (CNDO) and emphasize the outcomes of these treatments at early and late ages. Material and Method: Between April 2000 and March 2009, two hundred and ninety-four eyes of 203 patients diagnosed with congenital nasolacrimal duct obstruction were retrospectively examined. According to symptoms and age, patients were treated with lacrimal massage and antibiotic therapy or probing and irrigation under general anesthesia. The cases were divided into two groups: Group 1 (lacrimal massage and antibiotic treatment) and Group 2 (probing and irrigation treatment). Results: In group 1, the cases were 1-42 months of age (mean 16.6±10.4 months) and were followed-up 2-32 months (mean 9.5±8.4 months). Proportions of eyes treated successfully were 53.7% overall, 96% for the 25 eyes in children 0 to 6 months old, 51.3% for the 37 eyes in children 7 to 12 months, 53.3% for the 30 eyes in children 13 to 18 months, and 39.7% for the 68 eyes in children 19 to 36 months. In group 2, the cases were 9-84 months of age (mean 24.5±13.1 months) and were followed-up 2-38 months (mean 11.1±10.6 months). Proportions of eyes treated successfully were 86.5% overall, 94.1% for the 17 eyes in children 9 to 12 months old, 93.1% for the 44 eyes in children 13 to 18 months, 93.7% for the 32 eyes in children 19 to 24 months, 84% for the 25 eyes in children 25 to 36 months, 71.4% for the 7 eyes in children 37 to 48 months, and 66.6% for the 9 eyes in children 49 to 84 months. Conclusions: In the 0-1 age group, lacrimal massage and antibiotic treatment are the first therapeutic option. Early probing before 12 months of age should be refrained from unless symptoms such as dacryocystitis or severe blepharitis are presented. We consider that delay in probing is a viable option with mild symptoms up to the age of 18 months, or later if the parents wish it. Probing is successful in the older age group and can be performed up to the age of 5 years, but the success rates decrease with increasing ages of cases. (Turk Arch Ped 2010; 45: 359-65)
A rare cause of diarrhea: Eosinophilic gastroenteritis

Abstract: Eosinophilic gastroenteritis (EG) is a rare disease that is characterized by an eosinophil-driven inflammation of the digestive tract, presenting with gastrointestinal symptoms. In this report, a case of EG in an elderly man who presented with diarrhea was presented.A 68-year-old man admitted to hospital due to diarrhea, nausea, abdominal pain, and weight loss started two weeks ago. Hypereosinophilia were determined. Serum immunoglobulin E level was high. Stool examinations for intestinal parasites were negative. Biopsies were taken from esophagus, stomach, duodenum during upper and lower gastrointestinal endoscopies. Histopathological assessement showed an intense edema, eosinophilic and increased lymphoplasmasitic infiltration in lamina propria. Increased eosinophil count seen in bone-marrow biopsy, atypical cell was not seen. Eosinophilic gastroenteritis was considered and, ketotifen 2 mg/day and methylprednisolone 1 mg/kg/day were started. His symptoms were improved.Gastrointestinal symptoms of EG are nonspecific. The increase of eosinophilic cells in the circulating blood can suggest the possibility of EG. Definite diagnosis is made by histopathological assessment. Glucucorticoid therapy is the mainstay and is effective in the treatment of EG. However, the antihistamines are also used solely or combined with glucocorticoids.

Abstract: e10-5a3 is a dhfr-ts- leishmania major double knockout auxotrophic shown previously to induce substantial protection against virulent l. major infection in both genetically susceptible and resistant mice. we investigated the capacity of dhfr-ts- to protect against heterologous infection by l. amazonensis. the degree of protection was evaluated by immunization of balb/c or c57bl/6 mice with e10-5a3, followed by l. amazonensis challenge. whether immunized by subcutaneous (sc) or intravenous (iv) inoculation, susceptible and resistant mice displayed a partial degree of protection against challenge with virulent l. amazonensis. sc-immunized balb/c mice developed lesions 40 to 65% smaller than non immunized mice, while iv immunization led to protection ranging from 40 to 75% in four out of six experiments compared to non immunized animals. the resistant c57bl/6 mice displayed comparable degrees of protection, 57% by sc and 49% by iv immunization. results are encouraging as it has been previously difficult to obtain protection by sc vaccination against leishmania, the preferred route for human immunization.

Abstract: It is possible to expose the structure of Ottoman society through the tereke registers which constitute one of the main sources of Ottoman socio-economic history. Analyzing tereke registers is the way to conceptualize the past and to find though static some data respecting the time of their registry. In addition, these registers also provide some information regarding the taste and life styles of people in different levels of Ottoman society. Particularly the preferences for the taste of clothing reflect the statutes and economic power of the people in the society.The purpose of this study is to find out what is the relations between the taste of clothing and the basic necessities of people by analyzing the tereke register of a merchant in Bal kesir in the 19th century. Since the nineteenth century is a period of transformation in Ottoman Empire as is the case all over the world, another aim is to check whether these changes were visible in Bal kesir and to what extent. The answers will be given upon the registry of that Merchant, Hac Halil. Any change in the taste of clothing between the earlier centuries and the 19th century will be noted according to the registry. Because of this registry we can find out the trade connections, transactions and payment relations among the merchants of Bal kesir. Using this individual’s registry allows us to look at the socio-economic structure of Ottoman society closely.
MENTORSHIP IN THE PROFESSIONAL DEVELOPMENT OF PROVINCIAL EDUCATIONAL SUPERVISORS

Abstract: Supervision can be defined as the leading process of novice teachers usually by experienced ones who have the qualities in order to conduct the profession better. It can be said that mentorship can be beneficial to train educational supervisors. This study purposes to determine benefits of mentorship process in the process of training vice-supervisors. This study was conducted with a qualitative research method and 41 maximum diversity supervisors working in nine different provinces were interviewed in 2012. The data were collected with semi-structured interview form and analyzed with descriptive analysis technique. Results reveal that training the supervisors and vice-supervisors by means of mentor supervising on the job is not sufficient. However, it was understood that supervisors need mentorship, and they believe that this system will help them develop professionally; it will increase their motivation, productivity, institutional learning and knowledge sharing process. As a result of this study, a model was developed to improve the mentorship process in supervision system.

Abstract: J rg Nowak führt die Erkenntnisse der marxistischen und feministischen Staatstheorie zusammen und entwickelt ein analytisches Begriffsinstrumentarium, womit sich die geschlechterpolitische Ausrichtung der Familienpolitik zwischen 2002 und 2007 analysieren l sst. Er vertritt die These, dass die hegemoniale Einbindung des liberalen Feminismus in die staatliche Politik ein klassenselektives Ern hrerregime hervorgebracht hat. Das Buch ist trotz einer eher kursorischen Diskussion der Familienpolitik zu empfehlen. J rg Nowak brings together insights from Marxist and feminist theories of government and thus develops an analytical set of concepts with which to analyze the gender-political bias of family politics between the years 2002 and 2007. He postulates that the hegemonic inclusion of liberal feminism in government politics brought about a class-selective regime that favors breadwinners. The reviewer recommends the book, despite its rather cursory discussion of family politics.
