Abstract: Crouzon is a congenital syndrome described as one of the varieties of craniofacial dysostosis caused by premature obliteration and ossification of two or more sutures, most often coronal and sagittal. Oxycephaly, scaphocephaly, wedge skull, and oblique head are prominent presentations. Pediatric patients with Crouzon’s syndrome frequently present for multiple ophthalmic surgeries requiring general endotracheal anesthesia. In our study we reported our anesthetic experience with the laryngeal mask airway in a 29 years-old female patient with Crouzon’s syndrome. We used sevo urane 1-8 % and fentanyl 1 mcg/kg to induce anesthesia, and maintained anesthesia with 40% nitrous oxide, oxygen, and sevo orane 1-3 %. Both induction and maintenance were smooth. Emergence from anesthesia was also smooth.
Evaluation of Sexual Dysfunction in Females With Rheumatoid Arthritis

Abstract: Ob-jec-ti-ve: To investigate the influence of rheumatoid arthritis (RA) on sexual functioning in female patients and to compare the results with data from the general population. Metarials and Methods: One hundred women with RA and 45 age- and body mass index-matched healthy women were included in the study. Clinical parameters, health status of the patients during activities of daily living, functional classifications of patients, activity of disease, scores of articular index, hand grip values, laboratory activity parameters and radiological damage at the hands, wrists, ankles, knees, and hips were evaluated. The Female Sexual Function Index (FSFI) and the Beck Depression Index (BDI) were used in both groups. Correlations between the disease parameters and the total score and individual scores of 6 domain structure of FSFI (desire, subjective arousal, lubrication, orgasm, satisfaction, and pain) were analysed. Results: The mean age of the patients was 44.8±8.2 (21-60) years and controls was 44.7±8.5 (26-59) years (p>0.05). Total and subunit FSFI scores were significantly lower in RA group than in controls (p<0.001, p<0.001 to p<0.01, respectively). BDI scores were also significantly higher in RA group (p<0.001). It was found that all parameters, which affecting the progress of disease, also had negative effects on sexual function in RA group. Regression analysis showed that the correlations between the sexual dysfunction and age (elderly), the duration of disease, high functional class, high health status score and high BDI score were more significant than the other parameters. Conclusion: In this study, sexual dysfunction was found to be more frequent among RA patients than in the normal population. Turk J Phys Med Re-hab 2012;58:36-46.
Glioblastoma located in the third ventricle: case report

Abstract: --------------------------------------------------------------------------------Glioblastoma is the most aggressive and common tumor of the CNS. It locates rarely at third ventricle. We report a case of primary glioblastoma located in the third ventricle. The lesion was found in a 27-year-old man who suffered from headache, nausea and vomiting. CT showed a heterogeneous intraventricular tumor caused hydrocephalus. The histological examination of the tumor defined the diagnosis of glioblastoma. Tumor resection was indicated but the patient refused the treatment. Post-operative radiotherapy was done. The patient died 6 months afterwards. In conclusion, in the differential diagnosis of third ventricular lesions the most malign tumor of the central nervous system, glioblastoma should be considered. Stereotaxis can be used successfully with a diagnosis purpose in deeply located lesions like third ventricle.

This paper investigates the effect of the Van-Ercis, Turkey, (Mw: 7.2)earthquake occurring on 23rd of October, 2011 on the transportation networks in the region. The basic incentivefor this research is to conceptualise the reliability and performance of the networks after the earthquake through the operational and topological analysis of the system. The demand and composition of the traffic along with the behaviour of the pedestrians were taken into account to evaluate the performance of the networks. In addition, the general structure of the cities and towns, as far as planning is concerned, is also paid attention and regarded as one of the main elements for the appraisal. The outcomes obtained are thought very important to be guidance for the expected Istanbul earthquake in the near future.

Brachial Neuritis Presenting with Isolated Long Thoracic Nerve Involvement

Abstract: Brachial neuritis is a sudden onset disorder characterized by severe pain and weakness of the shoulder and upper extremities. Pain is followed by motor weakness, sensory and reflex impairments. Electroneuromyography is the most valuable method for the diagnosis of brachial neuritis. Early and definite diagnosis is important to prevent unnecessary procedures including surgical interventions. Here we report a patient admitted to our outpatient clinics with a 3-4 day history of sudden onset of severe pain and motor weakness of shoulder muscles. Due to his scapular winging and specific ENMG findings, he was diagnosed as brachial neuritis presenting with isolated nervus thoracicus longus neuropathy. Turk J Phys Med Rehab 2009;55:83-6.
Superparamagnetic Iron Oxide Enhanced Magnetic Resonance Imaging in the Detection of Malignant Liver Lesions

Abstract: Objective: To determine the value of superparamagnetic iron oxide (SPIO)-enhanced MR imaging in malignant liver lesions.Materials and Methods: Twenty patients with various hepatic masses or metastases were investigated with 1 tesla MR. T1-weighted FLASH and double- echo TSE T2-weighted sequences were applied before and after contrast injection. “Defining statistics” were prepared according to pre and postcontrast signal intensity measurements. Results: In this study there were 11 patients with metastases, 4 patients with hemangioma, 4 patients with HCC and one patient with a hemangioma and a metastasis at the same time. In cases wheret we found only hemangiomas, in all sequences, S/N ratios (p=0.043) and T2-weighted double-echo TSE with long TE SI change (p=0.043) were significant. While in the hemangiomas in T2-weighted double-echo TSE with long TE there is an decrease in the C/N ratios; in cases with metastasis and HCC, compared to other sequences significant increase in C/N ratio in T2-weighted double-echo TSE with short TE sequences was detected.Conclusion: SPIO-enhanced MR imaging can be used as a non-invasive radiologic technique for the detection focal liver lesions, to determine the number of lesions and also has a value in differentiating the benign lesions (especially hemangiomas) from primary tumors or metastatic malignant lesions.

It is a fact that performing endoscopy using conventional methods requires substantial time and development of alternative diagnostic modalities. Replacement of rigid endoscopes with flexible and digital devices in time, faster performance of the procedures and reduced time for them to turn back to work brought about the expectancy. It was possible that easier and more reliable methods could exist. Idea of capsule endoscopy was born following examination of the gastrointestinal tract with an endoscope with shape of a capsule. Although idea of capsule endoscopy was suggested long years ago, it fell behind the advances in conventional step of the endoscopy, especially developmental speed of the flexible devices. Furthermore, three-dimensional (3D) organ views can be processed digitally by means of sophisticated software in the fields of computerized tomography and magnetic resonance imaging. Thus, virtual cystoscopy of high resolution and specificity is possible by means of both methods. Wireless capsule endoscopy is still in the experimental stage. “Steerable independent intracorporeal endoscope” with feature of consecutive instillation in addition to its diagnostic utility is not a dream.

Power System Analysis of an Aero-Engine  [PDF]

The aim of this study is analyzed in detail for better understanding of energy and power of an aero-engine. In this regard, this study presents energy equations were applied to the turbofan engine components. The engine has a thrust range of 82 to 109 kN. It consists of fan, axial low pressure compressor (LPC), axial high pressure compressor (HPC), an annular combustion chamber, high-pressure turbine (HPT) and low pressure turbine (LPT). The results show that power of the engine flow approaches a maximum value to be 82.85 MW in the combustor outlet, while minimum power is observed at LPC inlet with the value of 1.37 MW. Furthermore, important parameters of the engine are also analyzed from reverse-engineering method. It is expected that results of this study will be beneficial of power, cogeneration and aero-propulsive generation systems in similar environment.

An Application of Kalman Filtering and Artificial Neural Network with K-NN Position Detection Technique  [PDF]

Abstract: RFID technology is one of the important technologies to determine the object locations. Distances are calculated with respect to calibration curves of RSSI amplitudes. The aim of this study is to determine the 2D position of mobile objects in the indoor environment. The importance of the work is to show that localization by using Artificial Neural Network plus Kalman Filtering is more accurate than using classical KNN method. An indoor wireless sensing network is established with strategically stationed RFID transmitter nodes and a mobile object with a RFID receiver node. A fingerprint map is generated and K-Nearest Neighbourhood algorithm (KNN) is deployed to calculate the object locations. Fingerprint coordinates and RSS values received at these coordinates are deployed to set up an Artificial Neural Network (ANN). This network is used to determine the unknown object locations by using RSS values received at these locations. The accuracy of object localization is found to be better with ANN technique than KNN technique. Object coordinates, determined with ANN technique, are subjected to Kalman filtering. The results show that localization accuracies are improved and localization error distances are reduced by 46% with the deployment of ANN + Kalman Filtering.
