Vision-Related Quality of Life and Self-Rated Satisfaction Outcomes of Rhegmatogenous Retinal Detachment Surgery: Three-Year Prospective Study

Abstract: Background Subjective functional outcomes measurements, such as vision health-related quality of life (VRQoL) and self-rated satisfaction measures can provide helpful multidimensional vision health information that is more comprehensive than traditional objective measures, such as best corrected visual acuity (BCVA). The purpose of this study is to demonstrate 3-year longitudinal postoperative VRQoL and self-rated satisfaction changes after rhegmatogenous retinal detachment (RRD) surgery. Methodology/Principal Findings A prospective case series report was conducted in 92 RRD patients who underwent surgery during January 2004 through December 2006. Preoperative, 3-month, 1-year and 3-year postoperative patient VRQoL and self-rated satisfaction were assessed by face-to-face interviews. The importance of objective variables for predicting three dependent variables: CLVQOL composite scores change, 3-year postoperative CVLQOL composite score and self-rated satisfaction degree scores were calculated by stepwise multivariate linear or logistic regression analysis methods. Results The total CLVQOL composite scores change ranged between -48 and 90 (mean±standard deviation: 19.48±31.34), including positive changes in 62 patients. The self-rated satisfaction degree scores ultimately improved in 86 patients as compared with preoperative degrees. Statistically significant increases occurred only in the composite scores of subscale mobility and self-rated satisfaction degrees in the first 3 months, while the composite scores of the remaining subscales, and the total CLVQOL, BCVA in the RRD eye and weighted average BCVA, increased steadily throughout the first postoperative year. A better 3-year postoperative weighted average BCVA was associated with all of the 3 dependent outcome variables. Conclusions VRQoL of RRD patients improved substantially after surgery and they were satisfied with their postoperative vision. The BCVA, VRQoL and self-rated satisfactory degree scores recovered in different patterns, and supplemented each other in the RRD surgery outcomes evaluated. Surgeons are advised to pay closer attention to binocular vision in RRD patients, and make efforts to explain the results of surgery.
Implementation and first-year screening results of an ocular telehealth system for diabetic retinopathy in China

Abstract: BCDRT implementation was based on the acquisition of adequate digital retinographs, secure digital transmission, storage and retrieval of participants' data and reader-generated medical reports. Local diabetic residents meeting inclusion criteria were enrolled into the BCDRT system beginning in 2009. Participants recommended for further in-person examination with ophthalmologists were followed, and the consistencies in diagnoses between BCDRT and ophthalmologists for DR or macular edema were calculated.A total of 471 diabetic residents participated in BCDRT screening in 2009. The proportions of total DR, proliferative DR, and diabetic macular edema were 24.42% (115 patients), 2.12% (10 patients) and 6.47% (24 patients), respectively: 56 patients consulted ophthalmologists for further in-person retinal examination with funduscopy after pupil dilation. High rates of consistency between BCDRT screening and ophthalmologists were observed for macular edema (Kappa = 0.81), moderate or severe non-proliferative DR grade (Kappa = 0.92), and other DR grades (Kappa = 1). A total of 456 (96.82%) patients were willing to participate in the next BCDRT screening.BCDRT was a reliable and valid system for DR screening, and offers the potential to increase DR annual screening rates in local residents.It is well recognized that final visual loss in diabetic retinopathy (DR) generally occurs after early stages that are chronic and asymptomatic. Therefore, 20 years ago, the World Health Organization recommended annual eye fundus examinations for all diabetic patients [1]. Until recently, however, the proportion of diabetic patients undergoing annual eye examinations was still low in developed countries; for example, 40%-50% in the U.S [2]. Abstract: Controlling the ferroelectricity and magnetism in multiferroic materials has been an important research topic. We report the formation of a highly polarized state in multiferroic DyMnO3 single crystals which develops well above the magnetic transition temperatures, and we attribute it the thermally stimulated depolarization current effect of excess holes forming Mn4+ ions in the system. We also show that this high temperature polarized state intimately correlates with the lower temperature ferroelectric state that is induced by the incommensurate spiral magnetic order of Mn spins. This study demonstrate an efficient approach to tune the multiferroicity in the manganite system.
Assessing the severity of conjunctivochalasis in a senile population: a community-based epidemiology study in Shanghai, China

Abstract: This community-based epidemiologic study was conducted to evaluate prevalence rates and related factors of conjunctivochalasis among people over 60 years old in the CaoYangXinCun community, Shanghai, China. Cluster sampling was used in randomly selected local residents aged no less than 60 years. A cross-sectional study using a slit-lamp ophthalmoscope for examination was carried out from September 2008 to October 2008. A modified grading system based on the well-accepted Meller and Tseng's system was used. SPSS10.0 software was used for data and statistical analysis.A total of 2110 residents took part in this study, with a response rate of 94.85%. Among these, 930 cases were confirmed as conjunctivochalasis, with a prevalence rate of 44.08%. The prevalence rate increased with age (X2 = 10.44, P < 0.01). A total of 1762 eyes were confirmed as conjunctivochalasis eyes. Of these eyes, 943 were classified as Grade I (53.52%), 647 as Grade II (36.72%), 162 as Grade III (9.19%), and the remaining 10 eyes as Grade IV (0.57%). The conjunctiva usually accumulated on the nasal and temporal areas of the conjunctival sac (944 eyes, 53.58%).Although the rate estimated in the present study is not as high as reported in the former hospital-based study, conjunctivochalasis is considered a common age-related eye disease, deserving more attention to its early diagnosis.The original term conjunctivochalasis was coined by Hughes in 1942, describing a redundant, loose, nonedematous inferior bulbar conjunctiva interposed between the globe and the lower eyelid[1]. In 1986, Liu emphasized frequent tearing (epiphora) as a symptom of conjunctivochalsis after a thorough study in 15 patients. Abstract: 针对海西区优越的生态环境质量、重要的生态服务功能和强烈的发展愿景,结合重点产业发展显著的经济、社会效益和强烈的生态影响,通过规划期区域生态状况评价及其动态变化分析,进行海西区重点产业发展生态风险评价.结果表明:2008年海西区生态状况总体较好,但是空间差异显著.区域生态状况的Ⅰ级、Ⅱ级分区主要分布于武夷山区和中部大山带,重点产业近远期发展生态风险以"无风险"和"轻风险"为主,对海西区生态环境保护和生态安全保障具有重要意义;区域生态状况的Ⅲ级、Ⅳ级分区集中于沿海地带,规划期重点产业发展生态风险多属"重风险"等级.不同土地产出率、发展方案下,海西区重点产业发展生态风险及其空间分布呈显著差异.由于各地对重点产业发展的强烈愿景,地方愿景规划方案下海西区重点产业发展生态风险相对较强;但是土地产出率的提高有助于海西区减轻、规避重点产业发展生态风险;敖江流域、闽东诸河等地重点产业发展生态风险相对较轻,可以适当增加重点产业及相关产业布局.

Abstract:

Identification results of water quality model parameter directly affect the accuracy of water quality numerical simulation. To overcome the difficulty of parameter identification caused by the measurement’s uncertainty, a new method which is the coupling of Finite Difference Method and Markov Chain Monte Carlo is developed to identify the parameters of water quality model in this paper. Taking a certain long distance open channel as an example, the effects to the results of parameters identification with different noise are discussed under steady and un-steady non-uniform flow scenarios. And also this proposed method is compared with finite difference method and Nelder Mead Simplex. The results show that it can give better results by the new method. It has good noise resistance and provides a new way to identify water quality model parameters.

Abstract: A tracking algorithm based on improved Camshift and UKF is proposed in this paper to deal with the problems which exist in traditional Camshift algorithm, such as artificial orientation and tracking failure under color interference as well as object’s changed illumination occlusion. Meanwhile, in order to solve the sheltered problem, the UKF is combined with improved Camshift algorithm to predict the position of the target effectively. Experiment results show that the proposed algorithm can avoid the interference of the background color and solve the sheltered problem of the object, so that achieving a precise and timely tracking of moving objects. Also it has better robustness to color noises and occlusion when the object’s scale changes and deformation occurs.
