Validation of two informant-based screening instruments for personality disorders in a psychiatric outpatient population

Abstract: Purpose: The predictive validity of two informant-based screening instruments for personality disorders (PDs), the Standardized Assessment of Personality (SAP) and a short eight-item version (SAPAS-INF), were studied in 103 Dutch psychiatric outpatients, using the SCID-II as the ‘gold standard’. Methods: All patients and their informants were interviewed separately and independently by different interviewers who were unaware of the results in the other conditions. Results: According to the SCID-II, 66 patients had at least one personality disorder (PD). The SAP correctly classified 72% of all participants in the category PD present/absent. The sensitivity and specificity were 69% and 76%, respectively. The positive and negative predictive values were 84% and 58%. The SAPAS-INF, using a cut-off score of 3, correctly classified 70%; the sensitivity and specificity were 76% and 58%, respectively. The positive and the negative predictive values were 77% and 57%. Conclusion: These results show that the informant-based SAP as well as the shorter informant-based SAPAS-INF are adequate; though rather moderate screening instruments for identifying PD. The SAP and the SAPAS-INF, however, both performed worse than the SAPAS-SR, which is based on the patient’s self-report. Therefore, it is concluded that the SAP or the SAPAS-INF can be used as a satisfactory screening instruments for the presence/absence of PD in those cases where patients themselves are unable to provide the required information.
Screening for Personality Disorders: A Comparison of the Dimensional NEO-FFI with the Categorical SAPAS-SR  [PDF]

In psychiatric outpatients, the usefulness of the dimensional NEO-FFI as a screening instrument for personality disorders (PDs) was compared with the categorical screening instrument SAPAS-SR using the SCID-II as the gold standard. Major research questions are: 1) is the NEO-FFI a useful screening instrument for PDs? 2) does the NEO-FFI outperform a categorical screening instrument (SAPAS-SR)? 3) does combining both instruments improve the screening results? Extreme raising on Big Five personality trait domains (NEO-FFI) domain scores were examined in relation to the presence and the number of PDs as diagnosed, with to the SCID-II. Additionally, the NEO-FFI, in conjunction with a short self-report screening instrument (SAPAS-SR), was analysed with respect to sensitivity and specificity for screening of PDs. According to the SCID II, 97 patients (50%) were suffering from a PD. The majority of them had no (35.9%) or only one (40%) extreme score on one of the Big Five personality domains. There were no significant relationships between separate extreme traits on PD or five factor profiles, as proposed in the literature, and the presence of a SCID-II PD. Comparisons of the NEO-FFI with the SAPAS-SR showed no significant relationships. Using both screeners in conjunction resulted in an increase in specificity and the number of correctly classified cases at the expense, however, of the sensitivity. Correlation and regression analyses showed that personality traits are statistically significant predictors for each of the12 PDs. However, the associations between NEO-FFI scores and the DSM-VI-TR PD criteria were rather modest. Support could not be obtained for the view that separate extreme scores on basic personality traits or combinations of such scores in five-factor profiles will provide adequate screening possibilities for PDs. The SAPAS-SR has better screening potential than the NEO-FFI or the SAPAS-SR and the NEO-FFI together.

Abstract: We report the realization of quantum microwave circuits using hybrid superconductor-semiconductor Josephson elements comprised of InAs nanowires contacted by NbTiN. Capacitively-shunted single elements behave as transmon qubits with electrically tunable transition frequencies. Two-element circuits also exhibit transmon-like behavior near zero applied flux, but behave as flux qubits at half the flux quantum, where non-sinusoidal current-phase relations in the elements produce a double-well Josephson potential. These hybrid Josephson elements are promising for applications requiring microwave superconducting circuits operating in magnetic field.
Assessment of the Antiviral Properties of Recombinant Porcine SP-D against Various Influenza A Viruses In Vitro

Abstract: The emergence of influenza viruses resistant to existing classes of antiviral drugs raises concern and there is a need for novel antiviral agents that could be used therapeutically or prophylacticaly. Surfactant protein D (SP-D) belongs to the family of C-type lectins which are important effector molecules of the innate immune system with activity against bacteria and viruses, including influenza viruses. In the present study we evaluated the potential of recombinant porcine SP-D as an antiviral agent against influenza A viruses (IAVs) in vitro. To determine the range of antiviral activity, thirty IAVs of the subtypes H1N1, H3N2 and H5N1 that originated from birds, pigs and humans were selected and tested for their sensitivity to recombinant SP-D. Using these viruses it was shown by hemagglutination inhibition assay, that recombinant porcine SP-D was more potent than recombinant human SP-D and that especially higher order oligomeric forms of SP-D had the strongest antiviral activity. Porcine SP-D was active against a broad range of IAV strains and neutralized a variety of H1N1 and H3N2 IAVs, including 2009 pandemic H1N1 viruses. Using tissue sections of ferret and human trachea, we demonstrated that recombinant porcine SP-D prevented attachment of human seasonal H1N1 and H3N2 virus to receptors on epithelial cells of the upper respiratory tract. It was concluded that recombinant porcine SP-D holds promise as a novel antiviral agent against influenza and further development and evaluation in vivo seems warranted.

The Care Pathway Concept: concepts and theories: an introduction

Abstract: This article addresses first the definition of a (care) pathway, and then follows a description of theories since the fifties of the last century. It ends with a discussion of theoretical advantages and disadvantages of care pathways for patients and professionals. The objective of this paper is to provide a theoretical base for empirical studies on care pathways. The knowledge for this chapter is based on several books on pathways, which we found by searching in the digital encyclopedia Wikipedia. Although this is not usual in scientific publications, this method was used because books are not searchable by databases as Pubmed. . From 2005, we performed a literature search on Pubmed and other literature databases, and with the keywords integrated care pathway, clinical pathway, critical pathway, theory, research, and evaluation. One of the inspirational sources was the website of the European Pathway Association (EPA) and its journal International Journal of Care Pathways. The authors visited several sites for this paper. These are mentioned as illustration of a concept or theory. Most of them have English websites with more information. The URL's of these websites are not mentioned in this paper as a reference, because the content of them changes fast, sometimes every day.
MVA-Based H5N1 Vaccine Affords Cross-Clade Protection in Mice against Influenza A/H5N1 Viruses at Low Doses and after Single Immunization

Abstract: Human infections with highly pathogenic avian influenza viruses of the H5N1 subtype, frequently reported since 2003, result in high morbidity and mortality. It is feared that these viruses become pandemic, therefore the development of safe and effective vaccines is desirable. MVA-based H5N1 vaccines already proved to be effective when two immunizations with high doses were used. Dose-sparing strategies would increase the number of people that can be vaccinated when the amount of vaccine preparations that can be produced is limited. Furthermore, protective immunity is induced ideally after a single immunization. Therefore the minimal requirements for induction of protective immunity with a MVA-based H5N1 vaccine were assessed in mice. To this end, mice were vaccinated once or twice with descending doses of a recombinant MVA expressing the HA gene of influenza virus A/Vietnam/1194/04. The protective efficacy was determined after challenge infection with the homologous clade 1 virus and a heterologous virus derived from clade 2.1, A/Indonesia/5/05 by assessing weight loss, virus replication and histopathological changes. It was concluded that MVA-based vaccines allowed significant dose-sparing and afford cross-clade protection, also after a single immunization, which are favorable properties for an H5N1 vaccine candidate.
A Recombinant Influenza A Virus Expressing Domain III of West Nile Virus Induces Protective Immune Responses against Influenza and West Nile Virus

Abstract: West Nile virus (WNV) continues to circulate in the USA and forms a threat to the rest of the Western hemisphere. Since methods for the treatment of WNV infections are not available, there is a need for the development of safe and effective vaccines. Here, we describe the construction of a recombinant influenza virus expressing domain III of the WNV glycoprotein E (Flu-NA-DIII) and its evaluation as a WNV vaccine candidate in a mouse model. FLU-NA-DIII-vaccinated mice were protected from severe body weight loss and mortality caused by WNV infection, whereas control mice succumbed to the infection. In addition, it was shown that one subcutaneous immunization with 105 TCID50 Flu-NA-DIII provided 100% protection against challenge. Adoptive transfer experiments demonstrated that protection was mediated by antibodies and CD4+T cells. Furthermore, mice vaccinated with FLU-NA-DIII developed protective influenza virus-specific antibody titers. It was concluded that this vector system might be an attractive platform for the development of bivalent WNV-influenza vaccines.
Plasminogen Controls Inflammation and Pathogenesis of Influenza Virus Infections via Fibrinolysis

Abstract: Detrimental inflammation of the lungs is a hallmark of severe influenza virus infections. Endothelial cells are the source of cytokine amplification, although mechanisms underlying this process are unknown. Here, using combined pharmacological and gene-deletion approaches, we show that plasminogen controls lung inflammation and pathogenesis of infections with influenza A/PR/8/34, highly pathogenic H5N1 and 2009 pandemic H1N1 viruses. Reduction of virus replication was not responsible for the observed effect. However, pharmacological depletion of fibrinogen, the main target of plasminogen reversed disease resistance of plasminogen-deficient mice or mice treated with an inhibitor of plasminogen-mediated fibrinolysis. Therefore, plasminogen contributes to the deleterious inflammation of the lungs and local fibrin clot formation may be implicated in host defense against influenza virus infections. Our studies suggest that the hemostatic system might be explored for novel treatments against influenza.
