Metastasis to Paranasal Sinuses as the First Presenting Sign of Renal Cell Carcinoma  [PDF]

Abstract: The clear cell carcinoma is a relatively rare tumor, represented approximately 85% by renal cell carcinoma (RCC), but has a characteristic of early metastatic, which sometimes spread to unusual places. Metastatic tumors in the paranasal sinuses are very rare. This case report a 65 years-old patient who presented sinusitis and epitaxis as a first sign of renal cell carcinoma with metastasis to the right maxillary sinus, which was opereted at Lagoa Hospital—RJ and strengthen the inclusion of this disease in the differential diagnosis of sinus disease. The renal cell carcinoma is a relatively rare tumor, representing approximately 3% of malignant tumors. About a third of patients with renal cell carcinoma have metastasis at diagnosis. The sinuses are a rare location of metastasis. The maxillary sinuses and the paranasal sinuses are most commonly affected by metastatic tumors to the sinuses region. The most frequent primary tumor to metastasis to paranasal sinuses is the renal cell carcinoma. This report highlights the rarity of the case and the importance in the differential diagnosis of sinus disease.
Serum lipid levels in pregnancies complicated by preeclampsia

Abstract: context and objective: pre-eclampsia is a disorder that occurs only during pregnancy. postpartum changes relating to lipid metabolism may contribute towards the endothelial lesions observed in preeclampsia. thus, the aim of the present study was to evaluate the lipid profile among patients who present preeclampsia and correlate these parameters with 24-hour proteinuria. design and setting: cross-sectional analytical study including 77 pregnant patients seen at hospital dório silva. methods: this study involved 42 women with preeclampsia and 35 healthy pregnant women in the third trimester of pregnancy as controls. blood samples were obtained from all the patients, and the serum levels of triglycerides, total cholesterol, low-density lipoproteins (ldl), high-density lipoproteins (hdl) and very low density lipoproteins (vldl) were determined. cases and controls were matched for maternal age, gestational week and body mass index. results: the vldl and triglyceride values from the women with preeclampsia were significantly higher than those of the healthy women. there was a positive correlation between increased proteinuria and higher vldl and triglyceride levels in patients with preeclampsia. conclusion: among the patients with preeclampsia, higher vldl and triglyceride levels were positively correlated with proteinuria. these observations indicate that the pregnant women who presented elevated lipid levels were more susceptible to cardiovascular disorders and, consequently, pre-eclampsia.

Abstract: evaluation of health planning activities adapted to the local context is an essential part of daily work in the family health strategy (fhs). this study aimed to evaluate the planning activities conducted by family health teams in espírito santo state, brazil. this was an ecological study based on secondary data from 241 health teams in 46 municipalities (counties) in the state that use the evaluation for quality improvement in the family health strategy (amq-esf) form. criteria were set for evaluating the responses by health professionals to the quality standards in the amq-esf forms, and health planning indicators were listed. the responses were stratified according to the municipality's population size and time since implementation of the fhs and population coverage. the study mostly showed low adequacy rates in the teams' work processes in relation to the amq quality standards, and stratification of the variables had no influence on the local planning practices. specific measures are needed to upgrade the work processes in the fhs.

Abstract: this paper aims to analyze the content validity of self-assessment tools that make up the evaluation for quality improvement (eqi) of the family health strategy (fhs). it is a retrospective ecological study on secondary data available from 46 out 64 municipalities participating in the eqi program until december 2007 in espírito santo state, brazil, where 241 family health teams were identified. the average percentage of "yes" responses was calculated from data provided by health teams for each quality standard component of the eqi. there was a non-linearity between the implementation complexity of work processes suggested by the eqi from the list of quality standards on each quality level and self-assessment of the health teams of municipalities surveyed. the non-linear relation between the quality levels of eqi and complexity of implementation of work processes recommended by the standards of quality shows the need to rearrange the terms of self-assessment, establish new quality standards and to find a more consistent correlation between the difficulty of implementation of work processes in fhp and quality levels.

Abstract: objectives: to investigate the prevalence of post partum depression and its correlation with thyroid dysfunction in mothers. methods: a descriptive, cross-cutting observational study carried out among 292 women between the 31st and 180th days post partum, attending public health units in the metropolitan region of vitória, in the state of espírito santo, brazil. the sample was analyzed for socio-demographic and clinical-obstetric characteristics. a depressive disorder was defined by a score greater than or equal to twelve on the edinburgh post-partum depression scale (epds). the thyroid was assessed using serum dosages of thyroid stimulating hormone, (tsh), free thyroxin (t4 free) and anti-peroxidase antibodies (tpo). evaluation of the results was carried out using descriptive analysis and the χ2 test, with a level of significance of 5%. results: 115 women (39.4%) scored higher than 12 on the epds and were thereby deemed to be depressed; 117 (60.6%) scoring lower than 12 were considered not to be depressed. the prevalence of post-partum depression in the group with thyroid dysfunction was 36% and 40% in the group without thyroid dysfunction. there was no statistically significant difference in the frequency of depression between patients with or without thyroid dysfunction (χ2=0.131;p=0.717). conclusions: the frequency of ppd was high but no association was observed between post-partum depression and thyroid dysfunction.

Abstract: introduction: psychiatric symptoms are frequent in the postpartum period, a moment marked by hormonal alterations and changes in social character, family organization and women's identity. the edinburgh postnatal depression scale (epds) is a self-reporting instrument to track depression after pregnancy, unfortunately not always properly supported by health care professionals. this study aimed at verifying the prevalence of postpartum depression in women receiving care at basic health units. methods: cross-sectional study including 292 women in the postpartum period (from day 31 to 180) who answered the epds questionnaire. cut-off point < 12 for epds depression was used. results: a total of 115 women (39.4%) had scores < 12 in epds, classified as depressive; 177 (60.6%) had scores < 12 and were not considered depressive. women with lower education, higher number of pregnancies, higher parity, higher number of live children and shortest relationship time had more depression. conclusion: high frequency of postpartum depression is associated with social factors, which shows the importance of health care professionals in early detection of depression, with the aid of instruments such as epds, due to its efficacy and practicability.

Abstract: the immature stages of neotropical ticks are poorly known and in many cases have not been described. this work presents a morphological description of amblyomma brasiliense larvae (f1) and a redescription of nymphs (f1). a. brasiliense is reported as one of the most aggressive ticks to humans in brazil. immature ticks obtained from a laboratory colony initiated from adult specimens collected in the parque estadual intervales (24o 18' s and 48o 24' w), s?o paulo, brazil, were analyzed under scanning electron microscope, and also under light and stereoscopic microscopes. a. brasiliense larvae present basis capituli rectangular; short palpi; rounded idiosoma; coxa i with two spurs, the external one being longer than the internal one; and coxae ii and iii each with one short spur. nymphs present basis capituli rectangular with a sharp pointed cornua; oval idiosoma with scutum reaching coxa iii; coxa i with two evident spurs, the external one being longer than the internal one; coxae ii-iii each with one short spur; and coxa iv with a very short spur and chitinous tubercles on internal surface of posterior border of idiosoma. these morphological features, in association with chaetotaxy and porotaxy, should make possible the identification of immature amblyomma ticks.

Abstract: objective: to investigate growth asymmetry between the left and right hemimandibles (hms) during the 2nd and early 3rd trimester of pregnancy. methods: sixty eight hemimandibles (34 mandibles) of fetuses were used-20 female and 14 male-preserved in 10% formalin solution, and the following measurements were performed: condyle-coronoid process (co-cp), gonion-coronoid process (go-cp), gonion-gnathion (go-gn), condyle-gnathion (co-gn), symphyseal height (sh), mandibular angle (ma). the data were collected, tabulated and analyzed with the aid of spss software, version 11.0, 2005. one-way anova test was performed to compare the mean values of anatomical measurements of the right and left hms. gestational ages were divided into second trimester (period 1: 13-18 weeks and period 2: 18-24 weeks), and early third trimester (period 3: 24-30 weeks) of pregnancy. results: we noted a slight growth rate asymmetry in go-gn, co-cp, co-gn, go-cp and sh, comparing the left and right mandibular halves, between the 2nd and early 3rd trimester of pregnancy, although not statistically significant (p > 0.05). it was also found that the mandibular angle decreased and showed a slight-though statistically significant (p < 0.05)-asymmetry in the same prenatal period. conclusion: the authors concluded that there was a slight asymmetry in the growth rate of measurements go-gn, co-cp, co-gn, go-cp and sh, comparing the left with the right hemimandible between the 2nd and early 3rd trimester of gestation.
INTERNATIONAL CRIMINAL COURT AND THE ROME STATUTE SOME NOTES ON THE PRINCIPLE OF COMPLEMENTARITY: A READING OF THE BRAZILIAN LAW

Abstract: The Second World War, despite of the atrocities committed, is a mark in the history of human rights achievements. The Charter of Human Rights and the establishment of the United Nations recognized the international community as one of the most important allies of the effectiveness of human rights. The response of international community to all violations of human rights could be seen in the establishment of international criminal courts. Despite of all efforts to create a permanent international criminal jurisdiction after Nuremberg, only in the 1990`s, after the cold war, the world could see a real mobilization to accomplish the aim of establishing such jurisdiction. The Rome Statute, which created the permanent international criminal court and its rules and proceedings, has had a tremendous support from the international community, and also from Brazil. But, despite of the advance that the Rome Statute represents, there is a long way to be followed, especially because all states that make part of the treaty must understand that the ICC has its basis on the complementary principle, and it is not a violation of sovereignty, but a tool to strengthen national sovereignty. This is the issue that will be examined on this article.
