Medical reports recommending work restrictions at a teaching hospital in Brazil

Abstract: this non-experimental and cross-sectional descriptive study aimed to evaluate medical reports recommending work restrictions for workers at a teaching hospital in brazil. a form was used for data collection, characterizing the medical reports and work restrictions, with its content previously validated. a total of 176 medical reports was analyzed, containing 337 work restrictions, recommended from january 2001 to january 2004. the results demonstrated a high rate of work restrictions in the study hospital. the most restrictions frequent were ergonomic (78.6%), and the most often prescribed were: no weight lifting or carrying and no repetitive movements. there was an average of two restrictions per medical report, 79.5% of which were definitive and the mean restriction period was 13 months.

Abstract: the aim of this cross-sectional study was to evaluate musculoskeletal symptoms among female members of the nursing personnel. the sample consisted of 105 female nursing aides and technicians who were working at a university hospital with highly dependent patients. the questionnaire was composed of items on demographic and job-factor information and the symptom section was a modification of the "nordic musculoskeletal questionnaire". in this study, 93% of the health workers reported at least one musculoskeletal symptom in the previous 12 months. the highest prevalence of musculoskeletal disorders was in the following anatomical areas: low back (59%), shoulders (40%), knees (33.3%) and neck (28.6%). as a result of musculoskeletal pain, 29.5% of the respondents reported missing work and 47.6% reported having had an appointment to see a physician in the previous 12 months. limited experience on the job and in the present unit were also observed among those who had more frequent complaints of pain in the knees (p=0.0272) and low back pain (p=0.0332), respectively. however, hand/wrist pain occurred more often among the participants with the higher numbers of weekly worked hours (or=3.72:1.26

Abstract: Este trabalho teve por objetivo avaliar sintomas músculo-esqueléticos em trabalhadoras de enfermagem. Os sujeitos compreenderam 105 auxiliares e técnicos de enfermagem que trabalhavam em unidades de interna o de um hospital que atende pacientes com alto grau de dependência física. Utilizou-se um questionário contendo dados demográficos e ocupacionais e queixas osteomusculares, dentro de uma abordagem ergon mica, adaptado do Nordic Musculoskeletal Questionnaire. Das participantes, 93% referiram algum tipo de sintoma osteomuscular nos últimos 12 meses. As mais elevadas prevalências desses sintomas, segundo as áreas anat micas, foram: regi o lombar (59%), ombros (40%), joelhos (33,3%) e regi o cervical (28,6%). Verificou-se também que, das respondentes, 29,5% faltaram ao trabalho e 47,6% consultaram um médico nos últimos 12 meses devido aos mesmos sintomas. Observou-se também menor tempo de trabalho na área de enfermagem e na unidade atual entre as trabalhadoras com ocorrência maior de dor nos joelhos (p=0,0272) e dor lombar (p=0,0332), respectivamente. Analysis of Perceptions and Prenatal Medical Records of Black Teens Girls in a São Paulo Hospital  [PDF]

Abstract: This paper studied pre-natal black teenager girls in a public hospital in Santos, state of Sao Paulo, Brazil. We discussed the strategies of the professional staff members, and the conceptions of reproductive and sexual health for teenagers. We noticed, among other things that medical records had not been completely filled, the item color had subdivisions, total lack of familiarity with public policies directed toward the Black population, and some discrete signs of institutional racism. We considered that specific attention to black teens was not given during work as part of a public policy; the lack of medical prevention leads to a referral hospital that does not articulate, on triage, the integral health policy for the Black population, the institutional care and equity.

Abstract: the objective of this paper is to report some clinical and therapeutic aspects of inhaled nitric oxide in pediatrics. some references were obtained from medline? using the keywords: inhaled nitric oxide and pediatrics, and critical care. other sources were the university library and personal files. along the last decade, clinical trials with inhaled nitric oxide demonstrated only a few specific areas of proven efficacy and a variety of possible adverse events. toxicity related to inhaled nitric oxide included metahemoglobinemia, cytotoxic pulmonary effects, excess production of nitrogen dioxide and peroxynitrite, and injury to the pulmonary surfactant system. the administration of inhaled nitric oxide to patients with severe left ventricular dysfunction and pulmonary hypertension should be cautious, since vasodilatation may increase pulmonary blood flow and lead to excessive preload. some studies showed the clinical effects related to abrupt nitric oxide withdrawal, including rebound pulmonary hypertension. current literature supports the therapeutic use of inhaled nitric oxide in persistent pulmonary hypertension of the newborn (gestational age > 34 weeks) to improve oxygenation and avoid extracorporeal oxygenation; and in congenital cardiopathy accompanied by pulmonary hypertension, especially in the immediate postoperative period. to date, research in pediatrics and multicentre trials in adults with inhaled nitric oxide therapy have failed to show mortality reduction or decrease the amount of time under mechanical ventilation for acute respiratory distress syndrome and acute lung injury. this indication needs further investigations. persistent pulmonary hypertension is the most important indication for inhaled nitric oxide. the food and drug administration has not approved inhaled nitric oxide in acute respiratory distress syndrome for adults and children.

Abstract: O objetivo dos autores é relatar alguns aspectos da aplica o clínica e terapêutica do óxido nítrico inalatório em pediatria. As referências foram obtidas através do Medline utilizando as palavras-chave: óxido nítrico inalatório e pediatria, e cuidados intensivos. Outras bases de pesquisa utilizadas foram obtidas na biblioteca da universidade e em arquivos pessoais. Na última década, os estudos clínicos com o óxido nítrico inalatório comprovaram sua eficácia somente em situa es específicas e associa o a uma variedade de efeitos adversos. A toxicidade do óxido nítrico inalatório inclui metahemoglobinemia, efeitos citotóxicos pulmonares, produ o aumentada de dióxido de nitrogênio e peroxinitrito e altera es no sistema surfactante pulmonar. A administra o do óxido nítrico inalatório a pacientes com disfun o ventricular esquerda grave e hipertens o pulmonar deve ser extremamente cautelosa, pois a vasodilata o determina eleva o do fluxo sanguíneo pulmonar, podendo aumentar muito a pré-carga. Alguns estudos observaram efeitos clínicos relacionados com a retirada abrupta do óxido nítrico inalatório, como a hipertens o pulmonar rebote. Atualmente, de acordo com a literatura médica, o uso do óxido nítrico inalatório está aprovado na hipertens o pulmonar persistente do neonato (idade gestacional igual a ou acima de 34 semanas) para melhorar a oxigena o e evitar a necessidade de oxigena o por membrana extracorpórea e nas cardiopatias congênitas acompanhadas de hipertens o pulmonar, principalmente durante o pós-operatório imediato. Até hoje algumas pesquisas pediátricas e estudos multicêntricos em adultos n o conseguiram demonstrar redu o na mortalidade e no tempo de ventila o pulmonar mecanica com o uso do óxido nítrico inalatório na síndrome do desconforto respiratório agudo e na les o pulmonar aguda. Investiga es futuras s o necessárias para esse uso. A hipertens o pulmonar persistente é a indica o mais importante para o uso do óxido nítrico inalatório. A Food and Drug Administration n o aprova o uso do óxido nítrico inalatório para síndrome do desconforto respiratório agudo em crian as e adultos.
Use of Asymmetric Models to Adjust the Vitamin Intake Distribution Data for Older People  [PDF]

Abstract: One of the main interests in the nutrition field is to estimate the distribution of usual nutrient in-take. Data from vitamin intake generally present high asymmetry mainly to the presence of outliers. This can occur due to the variability of the diet and, in this case, robust estimation to get the distribution of the data can be required. Then, the aim of paper is to propose an alternative approach for estimating usual intake through asymmetric distributions with random effects applied to data set 10 vitamins obtained from a dietetic survey for 368 older people from Botucatu city, S?o Paulo, Brazil. In general, these asymmetric distributions include parameters related to mean, median, dispersion measures and such parameters provide good estimates for the intake distribution. In order to make some comparisons, a model fitted by National Cancer Institute (NCI) method with only for amount of nutrient intake was established using Akaike Information Criteria (AIC). NCI method is based on a Box-Cox transformation coupled with normal distribution but in case of asymmetric data, this transformation can be not useful. It was observed that, in the presence of outliers, the asymmetric models provided a better fit than the NCI method in the major of the cases. Then, these models can be an alternative method to estimate the distribution of nutrient intake mainly because a transformation for the data is no necessary and all the information can be obtained directly from the parameters.

Abstract: Sendo os sujeitos da educa o o professor e o aluno, observa-se a rela o de poder e saber que se estabelecem entre ambos no processo de ensino tendo como foco os alunos portadores de altas habilidades. Posto que o aluno historicamente ocupa o lugar do aluno, compreende-se que a fonte do saber e do conhecimento aparece histórica e culturalmente na figura do professor. Os alunos com altas habilidades e superdota o muitas vezes p e em prova o conhecimento e autoridade do professor estabelecendo uma rela o de conflito entre o poder disciplinar e o saber, assim utiliza-se Foucault para esclarecer esta rela o entre o saber e o poder que historicamente permeiam e rege nossa vida social, moral, ética e educacional.
