Self-expanding metal stents versus antrectomy for the palliative treatment of obstructive adenocarcinoma of the gastric antrum

Abstract: background: gastric cancer patients are first diagnosed with an unresectable tumor in up to 40% of cases. gastric outlet obstruction causes nausea, vomiting, dehydration and malnutrition. the aim of the study was to compare self-expanding metal stents to antrectomy and roux-en y gastrojejunostomy for palliation of obstructive adenocarcinoma of the gastric antrum. methods: retrospective study in patients with obstructing cancer of the gastric antrum. patients were divided into two groups: group a, underwent endoscopic placement of self-expanding metal stents and group b underwent surgical treatment with antrectomy and roux-en y gastrojejunostomy. collected data included: age, gender, performance status (karnofsky's score), body mass index, histopathology, clinical stage (tnm classification), technical and clinical success of the procedure, time to oral intake, in-hospital stay, re-intervention rate, and complications related to the treatment and survival. results: a total of 39 patients with gastric adenocarcinoma were included, 21 male and 18 female. nineteen patients were assigned to group a and 20 patients to group b. there were no statistically significant differences between groups in regards to age, body mass index, karnofsky's score and clinical stage. the technical and clinical success was similar for both groups. there was a statistically significant difference between groups favoring self-expanding metal stent in time to oral intake (1 ± 0 vs. 4.9 ± 0.6 days, p = 0.0001) and in-hospital stay (0.94 ± 1.18 vs. 7.8 ± 7.7 days, p = 0.0005). we did not find statistically significant differences with regards to long-term survival. conclusions: in patients with malignant gastric outlet obstruction due to gastric cancer, endoscopic palliation with self-expanding metal stents provide a shorter interval to oral intake, shorter in-hospital stay and lower rate of complications.
Safety and Efficacy of Radiofrequency Ablation in the Management of Unresectable Bile Duct and Pancreatic Cancer: A Novel Palliation Technique

Abstract: Objectives. Radiofrequency ablation (RFA) has replaced photodynamic therapy for premalignant and malignant lesions of the esophagus. However, there is limited experience in the bile duct. The objective of this pilot study was to assess the safety and efficacy of RFA in malignant biliary strictures. Methods: Twenty patients with unresectable malignant biliary strictures underwent RFA with stenting between June 2010 and July 2012. Diameters of the stricture before and after RFA, immediate and 30 day complications and stent patency were recorded prospectively. Results. A total of 25 strictures were treated. Mean stricture length treated was 15.2?mm (SD = 8.7?mm, Range = 3.5–33?mm). Mean stricture diameter before RFA was 1.7?mm (SD = 0.9?mm, Range = 0.5–3.4?mm) while the mean diameter after RFA was 5.2?mm (SD = 2?mm, Range = 2.6–9?mm). There was a significant increase of 3.5?mm (t = 10.8, DF = 24, P value = <.0001) in the bile duct diameter post RFA. Five patients presented with pain after the procedure, but only one developed mild post-ERCP pancreatitis and cholecystitis. Conclusions: Radiofrequency ablation can be a safe palliation option for unresectable malignant biliary strictures. A multicenter randomized controlled trial is required to confirm the long term benefits of RFA and stenting compared to stenting alone. 1. Introduction Self-expanding metal stents (SEMS) have become the mainstay palliative treatment for malignant biliary obstruction in patients with a life expectancy greater than 3 months [1, 2]. Their use has improved bile duct patency beyond what was achieved with plastic stents; however, long-term patency continues to be an unresolved issue. SEMS can occlude from tissue ingrowth or overgrowth, benign epithelial hyperplasia or secondary to biofilm, and sludge formation within the lumen of the stent [3]. Up to 50% of patients will have stent occlusion in the first 6 to 8 months [4, 5]. Different design alternatives have been explored in an attempt to improve stent patency. Covered SEMS were designed to prevent tissue ingrowth; however, they are contra-indicated for hilar drainage, have higher migration rates, and might be associated with increased risks of pancreatitis and cholecystitis [6–11]. Another treatment strategy to prolong stent patency and eventual survival is photodynamic therapy (PDT). PDT showed promising results; however, it carries a high complication rate including cholangitis and photosensitivity requiring the patient to avoid direct exposure to light for 4–6 weeks [12–14]. Abstract: the relationship between asymptomatic shedding of bacterial enteropathogens and the hygiene habits of families who have had a child with typhoid fever (tf) are investigated. the sample was made up of 80 families: 40 families in which one child had had tf (group a) and 40 in which no children or either of the parents had had a history of tf (group b). in each group 20 families belonged to a low socieconomic status (ses) and 20 to a high ses. a structured interview was used to evaluate the ses and the hygiene habits of the child; observations were made to measure the hygiene habits of the family (toilet, kitchen and food preparation) and bacteriological studies (fecal samples and hand markers). results show that carriers were more frequent in group a than in group b. the bacterial species found were significantly more numerous in group a than in group b (fecal samples: e. coli, the classic serotypes, shigella ssp, and hand markers: e. coli). families of group a had higher carriage rates than those of group b. finally there exists a significantly higher association between inadequate hygiene habits and carrier families. these results show the need to teach specific habits of proper hygiene to the entire population, because the fact of belonging to the high ses does not in itself preclude inadequate hygiene habits.
Evaluation of the Properties of Healing of the Extract of Kombucha in Sheep in Growth with Malnutrition, Parasitocis and Respiratory Problems  [PDF]

With the objective to evaluate the properties, curatives of kombucha extract as an alternative treatment were used; 20 sheep creoles weaned with stunting, respiratory diseases, parasitic and malnutrition were divided into control and experiment n = 10 groups; the first one was administered by ivermectin 1% (2 mg/kg) subcutaneous, with a reinforcement of closantel 5% (0.5 mg/kg) by oral route and enrofloxacin at 5% (3 mg/kg) intramuscularly for three consecutive days and the second group by oral route 20 ml of extract kombucha per animal each three days for about 4 months, was weighed at the beginning of each month; clinical tests were conducted and statistical data indicated no significant differences in weight gain 30.370 kg/p control, 31.290, 3.503 kg/p experimentally, some showed small differences in blood samples and in stool tests for example, control ended with Eimeria 1.00 ± 0.00, creatinine, 0.17 ± 0.04 mg/dl, uric acid 23.20 5.37 ± mg/dl, 46.12 ± 4.70 MCV, 7.21 ± 0.25 g/dl total protein, 38.20 10.06 ± 1.40 and segmented neutrophils ±1.07, and on the other hand the experimental in erythrocytes present 9783000.00 ± 1036072.17 million, in hemoglobin 13.43 ± 0.51 g/dl, hematocrit 38.87 ± 2.13, and platelet 422320.000 ± 52442.578 thousands. Both groups did not exceed normal parameters, however, physically the experimental animals were better and had no parasites or respiratory problems as opposed to controls.

Abstract: In order to assess the impact of the early and late tissue repair of Kombucha extract on incised wounds in animal models, 24 Wistar male rats were used and divided into three groups: one of the groups received a topical cure on the wound with Brosin, and the other received the Kombucha extract. The third group did not receive any treatment, working as a control group. The objective was to evaluate and compare the Kombucha extract with a commercial healing product. Each one of the animals took a 1 cm wound in depth and length at a femur level on a side of the left thigh. The healing process was evaluated on an early and late phase by performing a morphometric and morphological analysis under the assumption of a faster recovery with the use of Kombucha. The results showed that there was a poor recovery in the control group. On the other hand, the other two groups, Brosin and Kombucha, were similar, with little inflammation and a high cell proliferation and migration along with basal array pulls of elastin and collagen which served for angiogenesis, repair and renovation of the new tissue.

Abstract: the internal structure of cities has been examined through various disciplines such as economic spatial models, ecological structures and land use. this paper seeks to study centrality in urban structure models for medium-sized mexican cities through the spatial analysis of population density, population age structure and housing in 32 mexican cities. quantitative urban structure models use distance to the central business district as a basic aspect for determining the behavior of certain phenomena. this study shows that the historical period to which urban space belongs enables one to identify consistent spatial patterns in the structure of intermediate mexican cities.

Abstract: Este trabajo exploratorio pretende aplicar y estudiar esquemas espaciales monocéntricos de ciudades a partir del análisis espacial sobre el gradiente de densidad, estructura de edad de la población y tenencia de la vivienda en 32 ciudades intermedias mexicanas. Los modelos cuantitativos de estructura urbana toman como referencia la distancia al centro de negocios de la ciudad como un elemento, determinante en el comportamiento de ciertos fenómenos. En el presente estudio vemos cómo el factor del período histórico al que pertenece el espacio urbano permite encontrar patrones espaciales consistentes en la estructura de las ciudades intermedias mexicanas.
