Intravesical protrusion of the prostate as a predictive method of bladder outlet obstruction

Abstract: objective: pressure-flow study is the gold standard for diagnosis of bladder outlet obstruction (boo). a prospective study was carried out to compare urodynamic evaluation and measurement of intravesical protrusion of the prostate for diagnosing boo. materials and methods: patients presenting with lower urinary tract symptoms (luts) associated with benign prostatic hyperplasia and suspected boo were prospectively evaluated through conventional urodynamics and classified according to the bladder outlet obstruction index (booi). they also underwent abdominal ultrasound measurement of the intravesical prostatic protrusion (ipp) and prostatic volume. the ipp was classified into three stages: grade i under 5 mm; grade ii, between 5 and 10 mm; and grade iii over 10 mm. results: forty-two patients, mean age 64.8 ± 8.5 years were enrolled. transabdominal ultrasound determined a mean prostatic volume of 45 ± 3.2 ml. achieved ipp's values were the following: grade i - 12 (28.5%), grade ii - 5 - (12%) and grade iii - 25 (59.5%). the results of prostate volume differed significantly between obstructed and non-obstructed men (p = 0.033) and for ipp among obstructed, inconclusive and non-obstructed men (p = 0.016). for ipp, the area under roc curve was 0.758 (95% confidence interval - 0.601 to 0.876), and the cutoff point to indicate boo was 5 mm with 95 % sensitivity (75.1 - 99.2) and 50 % specificity (28.2 - 71.8). conclusion: ipp and prostatic volume measured through abdominal ultrasound are noninvasive and accessible methods that significantly correlate to urinary boo, and are useful in the diagnosis of male urinary obstructive problems.
A Novel Intraurethral Device Diagnostic Index to Classify Bladder Outlet Obstruction in Men with Lower Urinary Tract Symptoms

Abstract: Objectives. Using a urethral device at the fossa navicularis, bladder pressure during voiding can be estimated by a minimal invasive technique. This study purposes a new diagnostic index for patients with lower urinary tract symptoms (LUTSs). Methods. Fifty one patients presenting with LUTSs were submitted to a conventional urodynamic and a minimal invasive study. The results obtained through the urethral device and invasive classic urodynamics were compared. The existing bladder outlet obstruction index (BOOI) equation that classifies men with LUTSs was modified to allow minimal invasive measurement of isovolumetric bladder pressure in place of detrusor pressure at maximum urine flow. Accuracy of the new equation for classifying obstruction was then tested in this group of men. Results. The modified equation identified men with obstruction with a positive predictive value of 68% and a negative predictive value of 70%, with an overall accuracy of 70%. Conclusions. The proposed equation can accurately classify over 70% of men without resorting to invasive pressure flow studies. We must now evaluate the usefulness of this classification for the surgical treatment of men with LUTSs. 1. Introduction Invasive pressure flow studies (PFSs) in urodynamics are still the gold standard method for objective classification of bladder outlet obstruction (BOO) in men with lower urinary tract symptoms (LUTSs). It is able to delineate patients for successful surgical approach [1], however, it is still costly, time-consuming, and associated with significant morbidity. The risks of complications after conventional urodynamic study in men with BOO are greater, and acute urinary retention, macroscopic hematuria, urinary tract infection, and/or fever can occur in over 19% of the cases [2]. During the past 14 years, many experts have raised minimal invasive possibilities as substitutes [3, 4]. Griffiths et al. have previously described and validated a minimal invasive technique based on controlled inflation of a penile cuff during voiding [5]. Others had attempted other types of penile cuffs and condoms with insufficient results [3, 6–9]. Trying to overcome these limitations, we developed, in association with the University’s Biomedical Engineering Department, a urethral device capable of extracting from the voiding patient measurements comparable to those achieved from invasive PFS [10]. Abstract: the mixture of semi-synthetic derivatives (-)-3-o-acetyl-cassine hydrochloride and (-)-3-o-acetyl-spectaline hydrochloride, prepared from the mixture of natural alkaloids (-)-cassine and (-)-spectaline (4:1) isolated from senna spectabilis, has been shown to be a potent acetylcholinesterase (ache) inhibitor, thereby prompting further molecular studies. in this sense, docking and dynamic molecular studies were carried out in this work, aiming to acquire a deeper understanding about all the structural aspects of molecules (-)-3-o-acetyl-cassine and (-)-3-o-acetyl-spectaline hydrochlorides, which differ with respect to their ache inhibitory potentials. both molecules establish important interactions with the peripheral anionic site within the catalytic gorge of torpedo californica ache. however, only the major compound (-)-3-o-acetyl-cassine hydrochloride significantly interacts with the catalytic triad. explicit-solvent molecular dynamic simulations were conducted in order to gain better understanding about the hypothetical interactions taking place between the semi-synthetic alkaloid molecules (-)-3-o-acetyl-cassine and (-)-3-o-acetyl-spectaline hydrochlorides and ache. the data obtained in this study indicated that (-)-3-o-acetyl-cassine hydrochloride is the most potent inhibitor of ache possibly due to the favorable interactions of this molecule with the target protein, with lower desolvation cost. these results suggested that the size of the side chain has an effect on the inhibitory potential of the evaluated molecules and may represent the starting point for the development of new derivatives of (-)-3-o-acetyl-cassine hydrochloride, with a view to the discovery of new effective ache inhibitors.

Abstract: the morphology of the brain of coati, nasua nasua, was studied, to compare the findings with other species described. the brains of five coatis were used, proceeding from the scientific breeding school (cecrimpas), unifeob. the animals were sacrificed in accordance with the legislation (cobea). with a needle, the common carotid artery and the external jugular vein was cannulated to cranial direction, injected latex solution stained with colored red barium respectively. afterwards the animals were fixed in 10% formaldehyde solution. brain has its dependent nutrition of four arteries, the internal carotid arteries and the right and left vertebral arteries. these vessels had composed the basal and carotid circuits that anastomose through the cerebral arteries volumes. they run in the base of the brain under piamater meninges.
NLO predictions for the growth of $F_2$ at small $x$ and comparison with experimental data

Abstract: We present parametrizations for the proton structure function $F_2$ in the next to leading order in perturbative QCD. The calculations show that the dominant term to $F_2(x,Q^2)$ should grow as $x^{-\ls}$ for small $x$ values, with the exponent $\ls$ being essentially independent of $Q^2$. Comparisons with the most recent H1 and ZEUS data confirm the value $\ls \sim 0.35$ obtained previously from fits to low energy data.
Inferring long memory processes in the climate network via ordinal pattern analysis

Abstract: We use ordinal patterns and symbolic analysis to construct global climate networks and uncover long and short term memory processes. The data analyzed is the monthly averaged surface air temperature (SAT field) and the results suggest that the time variability of the SAT field is determined by patterns of oscillatory behavior that repeat from time to time, with a periodicity related to intraseasonal oscillations and to El Ni\~{n}o on seasonal-to-interannual time scales.

Abstract: introduction: plastic surgery consultation is commonly sought for the treatment of pelviperineal injuries in general hospitals. the objective of this study was to present the experience acquired in the treatment of perineal, sacral, and hip injuries with the use of perforator flaps. methods: patients referred to the plastic surgery division of the clinical hospital of medicine college of universidade de s?o paulo for evaluation of pelviperineal and hip wounds from february to may 2009 were retrospectively evaluated. a total of 22 patients underwent reconstruction with skin and fasciocutaneous flaps based on the perforator vessels, according to the inclusion criteria. the average follow-up period was 6 months. results: pelviperineal injuries consisted of pressure ulcers in 20 cases (91%), deep infection in 1 case (45%), and perineal hidradenitis in 1 case (4.5%). the choice of flap for reconstruction was dependent on the local wound: 15 cases (68.2%) of sacral ulcers were repaired with a superior gluteal artery perforator flap; 3 cases (13.6%) of ischial ulcers were repaired with an inferior gluteal artery perforator flap; and 2 cases (9.1%) of trochanteric ulcers were repaired using a tensor fascia lata perforator flap. a fasciocutaneous gluteofemoral flap was selected for reconstruction of post fournier' syndrome in 1 patient and was used after resection of perineal hidradenitis in 1 patient. a new suture for late primary closure was necessary in 3 (13.6%) cases in which the suture line dehiscence was < 10% of the injury perimeter during the first 15 post-operative days. there were no cases of > 3% necrosis of the flap surface. these results were maintained during the follow-up evaluation period. conclusions: the results of the study were satisfactory, and the utility of surgical flaps without the incorporation of muscle for pelviperineal reconstruction was demonstrated. this treatment alternative decreases donor site morbidity and preserves the muscular tissue for

Abstract: The company Vale has inaugurated in Pará, on 26/06/2012 Biopalma, a factory dedicated to the extraction of palm-tree fat. The objective of Vale is to produce biodiesel from palm-tree fat to supply its own fleet of vehicles and machines using B20 blend. With this initiative the company aims to go further into more clean energetic arrays. The company innovates the market of biofuels adopting palm-tree as a raw material, as well as using B20, creating a new tendency in the biodiesel market. The company also highlights the importance of the oil-chemistry in the internal market, mainly to generate a sustainable process.
