DEMOGRAPHIC AND CLINICAL ASPECTS OF TUBERCULOSIS: A CASE SERIES OF PATIENTS FROM IN-PATIENT DEPARTMENT OF A HOSPITAL

Abstract: Controlling tuberculosis in India is a tremendous challenge. The disease is a major barrier to social and economic development. Based on the current scenario, the present observational prospective study was designed for 63 patients with tuberculosis admitted in in-patient department of a government general hospital in four months. Case sheets and laboratory reports of patients were collected and noted for evaluation. The study included patients who were all having active disease and absence of HIV infection. The follow-up of the patients was done every day. The case sheets of the patients revealed their age distribution, gender, body weight distribution, history of illness and previous illness, social habits, common and uncommon signs and symptoms, diagnostic tests performed, treatment therapy provided and adverse effects. Early identification of TB cases and the effective use of the first line drugs were the primary goals of the department. The common clinical features observed in the patients as well as the therapy provided by the physicians was typical and as per the Revised National Tuberculosis Control Programme of Government of India. Non-adherence, drug resistance, extra-pulmonary disease and severe contaminant disease states were not observed.
Vehicle Accident Automatic Detection and Remote Alarm Device

Abstract: The Rapid growth of technology and infrastructure has made our lives more easy . The advent of technology has also increased the traffic hazards and the road accident take place frequently which causes huge loss of life and property because of the poor emergency facilities. Our project will provide an optimum solution to this draw back. An accelerometer can be used in a car alarm application so that dangerous driving can be detected . It can be used as a crash or rollover detector of the vehicle during and after a crash. With signals from an accelerometer, a severe accident can be recognized. According to this project when a vehicle meets with an accident immediately Vibration sensor will detect the signal or if a car rolls over, an Micro electro mechanical system(MEMS) sensor will detects the signal and sends it to ARM controller. Microcontroller sends the alert message through the GSM MODEM including the location to police control room or a rescue team. So the police can immediately trace the location through the GPS MODEM, after receiving the information. Then after conforming the location necessary action will be taken. If the person meets with a small accident or if there is no serious threat to anyone`s life, then the alert message can be terminated by the driver by a switch provided in order to avoid wasting the valuable time of the medical rescue team. This paper is useful in detecting the accident precisely by means of both vibration sensor and Micro electro Mechanical system(MEMS) or accelerometer. As there is a scope for improvement and as a future implementation we can add a wireless webcam for capturing the images which will help in providing driver`s assistance. 10. Tramadol analgesia in labour

Abstract: The aim of the present study was to assess the efficacy and safety of intramuscular (i.m) tramadol hydrochloride as analgesia for labour. The study was conducted on 100 primigravidae with 37 to 41 weeks of pregnancy, with vertex presentation in active phase of labour, without any maternal or foetal complications. 100 mg of tramadol hydrochloride was given by i.m route during active labour. The following points were noted: (i) time of onset of pain relief, (ii) degree of pain relief scores (0 = no pain relief, 1 = mild relief, 2 = moderate relief, 3 = good relief), (iii) maternal adverse effects and (iv) foetal adverse effects. Good pain relief was noted in 58% of the parturients, moderate relief in 30% and mild relief in 12%. The average duration of pain relief was 4 h and the average onset of pain relief was 15 min. Majority of the parturients did not suffer from any adverse effects and only 16 % had minor adverse effects. Apgar score in the new born was > 7 in 96% at 1 min and 100% at 5 min. From the study it was concluded that tramadol hydrochloride i.m injection is safe and effective analgesia for labour.
Design and Implementation of Simultaneous Shield And Repeater Insertion for On-chip Interconnects

Abstract: A Resource based optimization is a new approach for high performance integrated circuits. The method is applied to simultaneous shield and repeater insertion, resulting in minimum coupling noise under power, delay, and area constraints Repeater insertion is a well known design technique to reduce the delay required to propagate a signal along a line. Shielding inserts an additional line between a victim line and an aggressor line. Finally placing a shield beside and inserting repeaters along a victim line and are chosen to exemplify the resource based optimization process. In the active shielding architecture shield driving circuits as 4:1 multiplexer, full adder, multipliers are inserted. The power consumption of active shielding architecture is observed to be approximately 20% less compare to passive shielding architecture. The main aim of this is minimize the coupling noise under power, delay, and area constraints

Are we eliminating cures with antibiotic abuse? A study among dentists

Abstract: Context: The theme of “World Health Day 2011” is “combat drug resistance- No action today, No cure tomorrow” which is very pertinent. The present study emphatically demonstrates the current issues related to the overwhelming concerns regarding indiscriminate use of antibiotics, leading to a bleak tomorrow where cures may be few. Aim: To know the prescription pattern of antibiotics for various dental procedures by dental practitioners. Materials and Methods: A pretested questionnaire was used which contained two sections pertaining to prescription of antibiotics for healthy and medically compromised patients during various dental procedures, with therapeutic and prophylactic considerations. Results: Questionnaire response rate of 66.6% was observed. Amoxicillin emerged as the most preferred antibiotic for dental procedures both as a therapeutic and a prophylactic drug. 50% of the endodontists and 40% of the general dentists opted to prescribe antibiotics during root canal therapy where ideally operative intervention would have sufficed. Overuse of antibiotics for routine scaling and extraction was observed. Conclusion: The dental profession as a whole needs to acquire a deeper understanding of the global effects of superfluous antibiotic prescription. Antibiotics when judiciously used are precise life-saving drugs.
PERFORMANCE EVALUATION OF MIMO OFDM USING WATER FILLING ALGORITHM

Abstract: In this paper the water filling algorithm has been implemented for allocating the power to the MIMO channels for enhancing the capacity of the MIMO network. The water filling algorithm provides solution with the help of channel stateinformation. The singular value decomposition and water filling algorithm have been employed to measure the performance of MIMO OFDM integrated system. The capacity and SNR performance for SISO and MIMO system without and with water filling algorithm are also been demonstrated for comparison. For adaptive modulation the capacity and BER performance of different QAM has been compared. The simulation study has been carried out to reflect the effectiveness of the proposed work
STATIC ANALYSIS OF LEAF SPRING

Abstract: Leaf springs are special kind of springs used in automobile suspension systems. The advantage of leaf spring over helical spring is that the ends of the spring may be guided along a definite path as it deflects to act as a structural member in addition to energy absorbing device. The main function of leaf spring is not only tosupport vertical load but also to isolate road induced vibrations. It is subjected to millions of load cycles leading to fatigue failure. Static analysis determines the safe stress and corresponding pay load of the leaf spring and also to study the behavior of structures under practical conditions. The present work attempts to analyze the safeload of the leaf spring, which will indicate the speed at which a comfortable speed and safe drive is possible. A typical leaf spring configuration of TATA-407 light commercial vehicle is chosen for study. Finite element analysis has been carried out to determine the safe stresses and pay loads.
ANALYSIS OF PERFORMANCE CHARACTERISTICS OF LASER BEAM WELDING

Abstract: Laser Beam Welding (LBW) is a widely used welding technique in Automotive, Aerospace, andElectronic and Heavy manufacturing industries to join a variety of metals and alloys because of its high speed,fine welding seam quality, low heat input per unit volume, deep penetration, and narrow heat affected zone, andreduced tendency to cracking. LBW process is so complex in nature hat the selection of appropriate input parameters is not possible by the trial-and-error method. The selection of welding parameters in any machining process ignificantly affects the weld quality and the production cost. In this work, the effects of welding input arameters, viz., pulse duration, pulse frequency, welding speed and pulse energy on Bead geometrical parameters, viz., bead penetration, bead width and bead volume are analyzed. It is based on the empirical models developed by response surface methodology.
SERUM UREA, CREATININE IN RELATION TO FASTING PLASMA GLUCOSE LEVELS IN TYPE 2 DIABETIC PATIENTS

Abstract: India as a developing country has more prevelance of diabetes and now has more people with type 2 diabetes (more than 50 million) than any other nation. Diabetes mellitus is a chronic metabolic disorder that can lead to cardiovascular, renal, neurologic and retinal complications. Type 2 diabetes has quickly become a global health problem due to rapidly increasing population growth, aging, urbanization and increasing prevalence of obesity and physical inactivity. A total of 40 diabetic patients of both sexs aged between 35 to 75 years attending medicine OPD were included in the study.After obtaining informed consent from the study group 5 ml of fasting venous blood sample was collected. Plasma glucose was estimated by GOD – POD method. Estimation of plasma creatinine was done by the modified Jaffe’s method. Serum urea was estimated by Urease-Berthelot’s method. There was significant increase in levels of serum urea, creatinine and FPG (p<0.001) in diabetic patients compared to healthy controls. On applying Pearson’s correlation serum urea correlated positively with creatinine (p<0.001, r = 0.910) in cases and also in contorls (p<0.001, r = 0.868). Blood urea and creatinine is widely accepted to assess the renal functions. Good control of blood glucose level is absolute requirement to prevent progressive renal impairment.
