Pancreatite autoimune e diagnóstico diferencial com a neoplasia do pancreas: A propósito de um caso clínico

Abstract: the autoimmune pancreatitis (aip) is a type of chronic pancreatitis characterized histologically by a lymphoplasmocytic infiltration and periductal fibrosis. the clinical presentation may mimic a carcinoma of the head of the pancreas, especially in cases of cephalic disease, characterized by obstructive jaundice, abdominal pain and weight loss. this clinical entity may be associated with other autoimmune diseases and the diagnosis is based on the diagnostic criteria suggested by mayo clinic and the japanese society of the pancreas, which associate the histological and radiological features to the increase in igg4 and response to treatment with corticosteroids. we present a case report of a patient who underwent a cephalic pancreatoduodenectomy, due to a preoperative diagnosis (clinical and radiological) of carcinoma of the head of the pancreas. the pathological study revealed that it was a lymphoplasmocytic pancreatitis and immunological study later confirmed the diagnosis of auto immune pancreatitis. this article aims to raise awareness for the differential diagnosis with carcinoma of the head of the pancreas, to avoid performing a surgery with relevant morbidity and mortality rates in a disease sensitive to treatment with corticosteroids

Abstract: A Pancreatite Auto-imune (PAI) é um tipo de pancreatite crónica caracterizada, histológicamente, por um infiltrado linfoplasmocitário e fibrose periductal. O quadro clínico pode mimetizar um carcinoma da cabe a do pancreas, principalmente nos casos de doen a cefálica, sendo caracterizado por icterícia obstructiva, dor abdominal e perda ponderal. Esta entidade clínica pode estar associada a outras doen as auto-imunes e o diagnóstico tem por base os critérios diagnósticos sugeridos pela Clínica Mayo e pela Sociedade Japonesa do Pancreas, em que associam as características histológicas e radiológicas à eleva o da IgG4 e à resposta ao tratamento com corticóides. Apresentamos um caso clínico de uma doente submetida a Duodenopancreatectomia cefálica, devido a um diagnóstico pré-operatório (clínico e radiológico) de carcinoma da cabe a do pancreas. O estudo anatomo-patológico revelou tratar-se de uma pancreatite linfoplasmocitária e o estudo imunológico posterior corroborou o diagnóstico de Pancreatite Auto Imune. A espiritualidade nas pessoas idosas: influência da hospitaliza??o

Abstract: spirituality is the personal search for meaning and purpose in life which assumes particular relevance in old age. hospitalization in old age is a period of high anxiety, and spirituality has shown to be a relevant coping strategy. this study compares old people's spirituality at hospital admission and discharge, analysing the influence of socio-demographic variables, pathology and period of hospitalization. the sample comprises 250 participants (≥ 65 years old), 50.4% female. the scale of spirituality, a 5-point likert scale comprising five items, was administered by interview at admission and discharge. main findings suggest that: i) 57% of participants show similar mean of spirituality when comparing admission and discharge, 22.5% maintain low spirituality and 22.5% maintain high spirituality; ii) 43% reveal alterations in their spirituality, 21.9% show decreasing of spirituality between admission and discharge and 20.8% show increase. those participants living in couple tend to show increase in their spirituality. hospitalization may have different impacts in the spirituality of elderly, what should be considered by practitioners when delivering care.

Abstract: this study aims at investigating the process of deinstitutionalization of madness, as seen by professionals involved in a project called "morada s?o pedro", as well as at constructing a theoretical/practical reference on therapeutic homes. it is a qualitative research in which semi-structured questionnaires were filled out by six professionals in mental health. results point out psychiatric reform as a way to include people in society and respect individual rights. this should be done with sensitive care, the need to invest in the person's potential and possibilities concerning his/her subjectivity and uniqueness. this fact may lead to new visibility, in the sense that the people may become actual citizens. i conclude that it is necessary to construct some theoretical reference based on practices that take place in therapeutic homes. thus, workers in mental health would reflect and construct concepts to consolidate advances in mental health in the context of psychiatric reform.

Abstract: Este estudio tiene como objetivo investigar junto a los profesionales vinculados al Proyecto Morada de S o Pedro el proceso de desinstitucionalización de la locura, con miras a la construcción de un referencial teórico-práctico sobre las residencias terapéuticas. Se trata de una investigacion cualitativa, realizada a través de cuestionario semiestructurado, con seis profesionales de la salud mental. Los resultados apuntan para una reforma psiquiátrica como una forma de incluir a las personas en la sociedad, de respetar los derechos individuales, con un cuidado sensible; la necesidad de invertir en las potencialidades y las posibilidades del individuo ejercer su subjetividad y singularidad, posibilitando una nueva visibilidad, en el sentido de que los usuarios se asuman como ciudadanos en la sociedad. Se concluye que es necesario construir un referencial teórico basado en las prácticas realizadas en las residencias terapéuticas, permitiendo reflexiones, construyendo conceptos para que los trabajadores de la salud mental consolides avances en los nuevos dispositivos en la salud mental en el contexto da reforma psiquiátrica.

Abstract: the management performance of educational organizations is studied, by simultaneously considering the criteria of efficiency, effectiveness, responsiveness and relevance. such performance is analyzed through a performance frontier built from a model of data envelopment analysis, based on quinn and rohrbaugh's spatial model, as well as on sander′s multidimensional paradigm. the model for evaluation of educational organizations management performance proposed joins efficiency, effectiveness, responsiveness and relevance indicators in a single performance score. it has been applied to the evaluation of the management performance of high schools in the state of santa catarina, brazil. the model has provided performance scores and optimal management performance goals for the evaluated schools. it has also identified the reference schools and correspondent management performance empiric frontier for the set of schools. the results illustrate the validity of the model as an instrument for the evaluation of management performance aiming at the quality improvement of educational organizations.

Abstract: functionality has primordial importance in old people's independence; situation we wish to be as long as possible in the course of one's life. functionality is a multidimensional construct measured through personal and instrumental activities of the life (adl and iadl). the aim of this study is to validate a scale of daily life activities for the portuguese population, middle-aged and elderly (n = 1322). results of this study show us the existence of three factors, with suitable psychometric characteristics, which are: mobility, instrumental activities of daily life and personal activities of daily life, with an explained variation of 64.75%. it has a specific interest for the population living in the community.

Abstract: this study analyses information flow in the clinical practice of resident physicians. to this end it considers the precepts of evidence based medicine] (ebm), and focuses on resident physicians at ufsc university hospital medical clinics. the relevance of this research project is underpinned by several reasons: the recognition of ebm in clinical practice as an alternative that can support clinical decisions, through the selection of information sources of greater evidence available in medical literature; verification that there is a lack of studies on resident physicians in the national literature - the majority of studies have focused on the manner that physicians and professors in health fields use the information systems and services; the conviction that the use of information selected according to criteria encourages more efficient clinical practice. this study identifies and analyses the processes involved in information flow in clinical practice by physicians from the perspectives of selection, access, retrieval and use of information sources. it presents an ideal model of information flow from the perspective of ebm.
O uso da informa??o e a prática clínica de médicos residentes

Abstract: this paper describes the study of information requirements and use of information by resident physicians in clinical practice. it contextualizes the study undertaken at ufsc university hospital of resident physicians and outlines the information requirements in clinical practice by the resident physicians in the hospital. it then maps the process of choosing the information sources to be used and shows those most widely used. the results suggest that: in the course of their work resident physicians are faced with issues classified as foreground issues which are more connected with the clinical aspects of patient treatment, to the detriment of issues connected with basic aspects such as epidemiology and etiology; for their main sources of information they use periodicals, textbooks and handbooks, guidelines and other manuals in an almost equal way; the internet was the most used method for information searches; within those information systems the most used were: uptodate, the capes journal portal and pubmed/medline. these results suggest that the resident physicians are aware of the importance of information use for more efficient clinical practice.
Caracteriza??o dos fumadores e factores que influenciam a motiva??o para a cessa??o tabágica

Abstract: background: smoking is currently a leading public health priority. profiling smokers allows for adequate intervention strategies and it is important to assess smokers? motivation. aims: to calculate the rate of smokers in 4 files from 3 health centres in coimbra and characterise smokers in terms of demographics, consumption pattern, motivation for smoking cessation and co-morbidities. investigate the relationship between motivation to stop smoking and age, gender, consumption and age at starting smoking and cardiovascular, respiratory and psychiatric co-morbidities. methods: descriptive study with analytical component. accessible population: 15-65 year old patients from 4 files from 3 health centres in coimbra seen july - august 2007. data treatment: spss 17. results: 224 randomly interviewed patients, 64.3% women; mean age 44.9 years old. rate of smokers was 17% (52.63% female). smokers? mean age was 39.4 years old. the mean age at starting smoking was 17.2 years old (16.4 in men). mean cigarette consumption was 17.5/day (13.3 in women). 47.4% was poorly motivated, 52.6% moderate/highly motivated. 50% of the smokers had co-morbidities. there was no association between any of these factors and smoking cessation motivation. discussion/conclusions: the results are similar to other national studies. there were more younger female smokers than male, but females smoked fewer cigarettes/day. in this study the most frequent co-morbidities were cardiovascular and psychiatric. only half of the smokers were motivated to stop smoking.
