Cáncer de seno en la Clínica San Pedro Claver de Bogotá, 2004

Abstract: objective: reviewing new cases of breast cancer attending clinica san pedro claver, bogotá, colombia, during 2004. material and methods: patients were recruited from the meeting offered by the service which all new breast cancer patients attend. hospitalised patients, outpatients, clinical meetings, consulting rooms, operating room schedules and pathology reports were consulted to ensure that a true cross-section was included. the information was recorded in an excel file and analysed using epiinfo software. results: breast cancer was diagnosed in 220 patients. average age was 59; 18 % were aged les than 40. infiltrative ductal cancer (84,1 %) was the most frequently found histological type. distribution by stage showed that most patients presented stage 0 to stage ii (62,7 %). tumours measured 2 or more centimetres in 78 % of the patients. 83 % of the patients had surgery and 60 % chemotherapy. however, these scores were lower in patients aged more than 69 (58,3 % for surgery and just 13,9 % for chemotherapy, p<0.05). hormonotherapy was prescribed for 40,9 % of the patients. conclutions: breast cancer is the more frequent found carcinoma. clinica san pedro claver's cancer records revealed the personal, clinical and therapeutic characteristics of new female breast cancer patients attending this hospital during 2004. most patients were in early breast cancer stages; however, detecting a greater number of non-palpable lesions should still be stressed. treating elderly patients (those older than 69 regarding that found for younger patients) and the quality of procedures, hormonal receptor reports and therapeutic decision should also be evaluated in an attempt to explain the low percentage of hormone treatment currently being used in the hospital.

Abstract: summary objective: analysing current evidence regarding proposals for vaccinating the colombian population against human papilloma virus (hpv). methodology: assessing hpv vaccine phase iii studies (future i, ii and patricia study groups). comments are made regarding generalising such vaccination amongst the general population, also taking factors limiting their use into consideration. results: methodological limitations were found when assessing these studies thereby challenging the importance of the efficacy found in such studies such as the choice of inadequate substitutes, analysing results involving heterogeneous primary compounds, analysis by protocol and not by intention to treat, inclusion criteria which did not underpin a general vaccination programme for girls aged younger than 15 and factors which could affect the validity of the studies such as conflicts of interests and sources of sponsorship. several complications have been reported regarding how the vaccine has been applied, providing better information about this prevention method’s real behaviour (i.e. when taken beyond its experimental design stage). conclusions: there was not sufficient evidence to recommend introducing hpv vaccination within colombian immunisation schedules when this review was made.
Diagnóstico de instituciones prestadoras de salud con habilitación de servicios oncológicos en Bogotá, Colombia

Abstract: the objective of this study was to obtain information on the performance of the oncological health service providing institutions (ips per their initials in spanish). the method used was a descriptive study based on surveys. we found that of every 73 forms sent to ips with enabled oncology services and to level ii state social enterprises (ese per their initials in spanish), 42 were returned (57.5%). we evaluated outpatient activities and calculated the theoretical supply and the availability of human resources. the facilities that have inpatient service for cancer patients were described and their cancer diagnostic procedures, treatment and monitoring aspects were assessed. the results of the study showed that cancer services in bogotá are mostly private, that there is a lack of integration between the specialties and that the range of services currently available should be sufficient for present needs, except for gastrointestinal cancer surgery service.

Abstract: Objetivo: analizar el estado actual de la evidencia de la vacunación para el virus de papiloma humano (VPH) a nivel poblacional teniendo como referente su posible aplicación en Colombia. Metodología: evaluación de estudios fase III sobre las vacunas contra el VPH: estudios de los grupos FUTURE I, II y PATRICIA. Se reflexiona sobre su generalización a nivel poblacional y se analizan factores que limitan su uso. Resultados: la evaluación de estos estudios encuentra limitaciones metodológicas que controvierten la eficacia presentada en las publicaciones de los estudios como son la elección de variables sustitutas inadecuadas, análisis de resultados con variables primarias compuestas que son heterogéneas, análisis por protocolo y no por intención de tratamiento, criterios de inclusión que no sustentan la aplicación poblacional de la vacuna en menores de 15 a os y factores que pueden afectar la validez de los estudios como conflictos de intereses y origen del patrocinio. De otra parte, se presentan reportes de complicaciones con la aplicación de la vacuna que brindan una mejor información sobre el verdadero comportamiento de este método de prevención por fuera del dise o experimental. Conclusiones: para el momento de esta revisión no se tiene la suficiente evidencia que sustente la inclusión de la vacuna contra el VPH como programa de vacunación poblacional en Colombia. Leiomioma uterino en paciente con síndrome de Mayer-Rokitansky-Küster-Hauser: reporte de caso

Abstract: objective: mayer-rokitansky-küster-hauser (mrkh) syndrome is a congenital malformation of the uterus and the upper part of the vagina in females having normal secondary sexual characteristics and karyotype 46 xx. complete uterine apl1asia could happen in the presence of two rudimentary horns or symmetric or asymmetric uterine hypoplasia accompanied by aplasia of one or two horns. the appearance of tumours in uterine rudiments is not often described. this case is presented following a review of the literature regarding an association between uterine leiomyoma and mrkh syndrome. materials and methods: a case has been presented and a search was made through medline on the pubmed page using the terms "leiomyoma" and "mayer-rokitansky-küster-hauser syndrome". 8 references were obtained; 7 of them were identified as being pertinent and included in the review. a manual search was also made of publications related to the base pathology. conclusion: finding a leiomyoma in an uterine remnant in mrkh syndrome is very rare; there are very few publications about it in the pertinent medical literature. it was preferred to carry out laparotomy given the size of the tumour in this patient who had a diagnostic impression of uterine leiomyoma, the pathology being reported by biopsy freezing and the definitive as leiomyoma.

Abstract: Objetivo: el síndrome de Mayer-Rokitansky-Küster-Hauser (MRKH) es una malformación congénita del útero y la parte superior de la vagina en las mujeres que muestran características sexuales secundarias normales y cariotipo 46 XX. Puede ocurrir aplasia completa del útero en presencia de dos cuernos rudimentarios o hipoplasia uterina simétrica o asimétrica acompa ada por aplasia de uno de los dos cuernos. Es poco frecuente la aparición de tumores en los rudimentos uterinos. Este caso se presenta con el objetivo de hacer una revisión de la literatura respecto a la asociación de leiomioma uterino y síndrome de MRKH. Materiales y métodos: se presenta el caso y se realiza búsqueda a través de MEDLINE en la página de PubMed con los términos "leiomyoma" y "Mayer-Rokitansky-Küster-Hauser syndrome". Se obtuvieron 8 referencias, de las cuales 7 se identificaron como pertinentes y se incluyeron en la revisión. Además, se hace una búsqueda manual de publicaciones relacionadas con la patología de base. Conclusión: el hallazgo de un leiomioma en el remanente uterino en el síndrome de MRKH es muy poco frecuente, con apenas unas cuantas publicaciones en la literatura médica. Abstract: the indoleacetic acid (iaa) is a natural auxin and one of the main growth regulators in higher plants. the extracellular indoleacetic acid (iaa) amount produced by the green microalga scenedesmus obliquus culture was detected by high performance liquid chromatography (hplc). it was evaluated the effect of three process variables in the extracellular iaa production in these cultures using 23 factorial design for screening methodology. the experimental design variables were agitation, illuminance (intensity of luminous flux) and carbon source (co2/air v/v (%) mixture) at three different levels. the iaa concentration and the specific growth rate were used as response variables. the hplc analyses showed that under experimental conditions, the iaa amount that is released to the medium by s. obliquus depends only on the illuminance variable. the light effects and the carbon concentrations in culture medium were significant (p < 0.05) for specific growth rate.

Abstract: Microalgae are capable of producing biomolecules that have a wide variety of applications in agriculture, food industry, and medicine. In this study, three process variables are evaluated in order to determine its incidence on biomass and exopolysaccharides production. The effect of light intensity, agitation and carbon concentration on Scenedesmus obliquus (UTEX 393) growth and expolysaccaharides production is evaluated using 23 factorial design through the screening methodology. The simultaneous effect of level variation for three different experimental variables is examined in the present study in three levels for each parameter (Light intensity: 80, 130, 180 μE m-2 s-1, Agitation: 0, 600, 1200 rpm, carbon concentration 0, 2, 4% v/v Air-CO2). Specific growth rate and the exopolysaccharides concentration are the selected response variables. Results show that the optimal conditions for the two response variables correspond to the maximum levels of the three experimental variables (180 μE m-2 s-1, 4% air-CO2, and 1200 rpm), obtaining a specific growth rate of 0.64 d-1 and a exopolysaccharides concentration of 24.7 mg L-1. A significant interaction between the variables is observed, which has direct effects on cellular growth and exopolysaccharides production. The EPS production is facilitated by the turbulent flow (agitation maximum level), which is associated with a higher availability and better distribution of energy sources (light) and carbon dioxide. The validation of polynomials models verifies the relevance of the analysis performed. Las microalgas son capaces de producir biomoléculas que poseen diversidad de aplicaciones relacionadas con la agricultura, alimentos y medicina. En la presente investigación se evalúan tres variables de proceso para determinar su incidencia e interacciones en la producción de biomasa y exopolisacáridos. El efecto de la intensidad de la luz, la agitación y la concentración de carbono sobre el crecimiento y la producción de exopolisacáridos en Scenedesmus obliquus (UTEX 393) es evaluada aplicando un dise o experimental factorial 23 por metodología de Screening. Se evalúa el efecto en simultáneo de la variación de tres variables experimentales en tres niveles (Intensidad de luz: 80, 130, 180 μE m-2 s-1, Agitación: 0, 600, 1200 rpm, concentración de carbono: 0, 2, 4% v/v Air-CO2). El coeficiente específico de crecimiento y la concentración de exopolisacáridos son las variables de respuesta seleccionadas. Los resultados muestran condiciones óptimas para las dos variables de respuesta en los niveles máximos de las variable
