Cono extendido, nueva variante técnica en la cirugía de la hernia inguinal Extended cone a new technical variant in the inguinal hernia surgery

Abstract: INTRODUCCIóN. El tratamiento de la hernia inguinal continúa siendo un tema controvertido y no existe un punto común de acción en cuanto a la modalidad terapéutica que se emplea. Con el desarrollo de las técnicas protésicas la controversia continúa a pesar de las demostradas ventajas de estas. El objetivo de este estudio fue evaluar los primeros casos intervenidos quirúrgicamente mediante la técnica del cono extendido, una modificación de las técnicas clásicas de Liechtenstein y Rutkow. MéTODOS. Se realizó un estudio prospectivo lineal en 45 pacientes operados mediante una técnica protésica denominada cono extendido. Los pacientes fueron operados en un período de 2 a os en la Clínica Multiperfil de Luanda (Angola) y para clasificarlos se utilizó la clasificación de Gilbert modificada. RESULTADOS. Hubo un predominio del sexo masculino. La localización más frecuente fue la región inguinal derecha. Predominaron los pacientes clasificados de grado III según la clasificación de Gilbert. Hubo 6 complicaciones menores y hasta el momento no han ocurrido recidivas. CONCLUSIONES. La técnica del cono extendido es una opción más para el tratamiento de los pacientes que presenten hernias inguinales de grado III y VI de la clasificación de Gilbert. INTRODUCTION. Abstract: Genes encoding proteins involved in retrotransposition, reactive oxygen species (ROS) resistance, drug efflux transport and cell wall degradation were identified. The great number of genes encoding cytochrome P450 monooxygenases (1.15% of gene models) indicates that M. perniciosa has a great potential for detoxification, production of toxins and hormones; which may confer a high adaptive ability to the fungus. We have also discovered new genes encoding putative secreted polypeptides rich in cysteine, as well as genes related to methylotrophy and plant hormone biosynthesis (gibberellin and auxin). Analysis of gene families indicated that M. perniciosa have similar amounts of carboxylesterases and repertoires of plant cell wall degrading enzymes as other hemibiotrophic fungi. In addition, an approach for normalization of gene family data using incomplete genome data was developed and applied in M. perniciosa genome survey.This genome survey gives an overview of the M. perniciosa genome, and reveals that a significant portion is involved in stress adaptation and plant necrosis, two necessary characteristics for a hemibiotrophic fungus to fulfill its infection cycle. Our analysis provides new evidence revealing potential adaptive traits that may play major roles in the mechanisms of pathogenicity in the M. perniciosa/cacao pathosystem.Moniliophthora perniciosa, previously known as Crinipellis perniciosa (Singer) Stahel, is a hemibiotrophic basidiomycete (Tricholomataceae, Agaricales, Marasmiaceae) fungus that causes Witches' broom disease (WBD) in cacao (Theobroma cacao L.) [1-3]. WBD and frosty pod rot (FPR), caused by Moniliophthora roreri, are the most devastating diseases of cacao in the Americas [4]. Cacao production in southeastern Bahia, the main production area in Brazil, was severely affected by the introduction of WBD at the end of 1980's [5]. Abstract: crime provides the news media with headlines on a silver platter and is an inexhaustible source of reporting to serve up to the public. to a certain degree, the consistency across media, consensus among the reporters, the volume of news coverage, and the ?massaging? of the events into a form that is palatable to the public quickly generates a series of ?waves? or hypes of reporting. this is a phenomenon that has been neither placed into its sociological setting, nor studied with regard to its impact on that setting. this article examines the way in which the ?carcavelos mugging of 2005? conforms to the media hype model advanced by peter vasterman, and goes a step further by suggesting that when the tide of news enjoys widespread consensus, it is rare that the news coverage includes elements that go ?against the tide?.

Abstract: we live in a fluid world characterized by complexity. dynamism of the relationship between agents, open systems, unpredictability, nonlinearity, self-organization, adaptability, creativity, instability, emergence, uncertainty, connectivity and flow are some of the hallmarks of contemporaneity, where information and networks emerge as catalysts of reality. in the social context, the exchange of information resources takes place through social relations established between social agents. the formed intertwined relationships are the social networks through which information flows. from this point of view, we should try to answer the question, "how does social information flows relate to the topology and dynamics of social networks?" in this paper, we provide a historical overview of networks and social networks concepts, briefly describe the method of social networks analysis (sna) and illustrate the information behavior in organizations with a case study of information social networks.

Abstract: A regulamenta o da reforma do ensino da arquitectura em 1957 inicia um processo de transforma es sucessivas no quotidiano dos cursos de arquitectura que irá culminar com uma crise no ano de 1969.Ao longo de uma década de funcionamento, a nova organica proposta pela reforma n o conseguiu, na sua globalidade, introduzir uma nova pedagogia nem democratizar o funcionamento das escolas, ficando refém do autoritarismo do Ministério da Educa o. No entanto, a sua rigidez científica e administrativa estimulou professores e alunos a desenvolver estratégias, quer de projecto, quer de reflex o e debate, que procuraram promover a fun o social do arquitecto atento aos problemas da sociedade e da cidade, por oposi o ao arquitecto tecnocrático.Nos anos de 1968 e 1969 as duas escolas, tal como a universidade e a sociedade, politizam o seu discurso. Os estudantes e os professores reivindicam abertamente altera es profundas na gest o da escola, nos métodos pedagógicos e nos objectivos do ensino, exigindo a reforma da Reforma de 57.Este texto aborda os momentos críticos deste processo de consciencializa o política e social dos estudantes, que procura recolocar o papel da Escola no debate sobre a forma o e sobre a profiss o de arquitecto. The 1957 regulations for the reform of the teaching of architecture initiated a process of successive changes in everyday practices in architecture courses that would culminate in a crisis in 1969. During the decade in which it was in force, the new system proposed by the reform proved incapable of introducing new teaching methods or democratising the way in which the schools were run, and remained bound by the authoritarianism of the Ministry of Education. However, its academic and administrative rigidity encouraged teachers and students to develop strategies, involving both planning and reflection and debate, aimed at promoting the social function of the architect as a professional concerned with the problems of society and the city, as opposed to the technocratic architect.In 1968 and 1969 the architecture schools in Lisbon and Oporto, like the university and society in general, politicised their discourse. Abstract: O crime fornece aos meios de comunica o uma matriz de desvio comportamental e gera uma fonte inesgotável de notícias. Em contextos muito particulares, o grau de unanimidade entre meios de comunica o, o consenso entre definidores primários, o volume da cobertura jornalística e o exagero ou distor o inerentes à cataloga o de várias ocorrências numa categoria ressonante geram ondas noticiosas num curto intervalo temporal, um fenómeno que carece ainda de enquadramento sociológico. O artigo descreve como o “arrast o” de Carcavelos de 2005 acompanhou o modelo descrito por Peter Vasterman para as ondas noticiosas, sugerindo um elemento adicional: quando a onda noticiosa se forma perante um consenso alargado, é escassa a disponibilidade para incorporar na cobertura jornalística elementos que contradigam o enquadramento predominante.
