Phosphatidylethanol in Blood as a Marker of Chronic Alcohol Use: A Systematic Review and Meta-Analysis

Abstract: The present paper aims at a systematic review of the current knowledge on phosphatidylethanol (PEth) in blood as a direct marker of chronic alcohol use and abuse. In March 2012, the search through “MeSH” and “free-text” protocols in the databases Medline/PubMed, SCOPUS, Web of Science, and Ovid/Embase, combining the terms phosphatidylethanol and alcohol, provided 444 records, 58 of which fulfilled the inclusion criteria and were used to summarize the current evidence on the formation, distribution and degradation of PEth in human blood: (1), the presence and distribution of different PEth molecular species (2), the most diffused analytical methods devoted to PEth identification and quantization (3), the clinical efficiency of total PEth quantification as a marker of chronic excessive drinking (4), and the potential utility of this marker for identifying binge drinking behaviors (5). Twelve papers were included in the meta-analysis and the mean (M) and 95% confidence interval (CI) of total PEth concentrations in social drinkers (DAI ≤ 60 g/die; M = 0.288 μM; CI 0.208–0.367 μM) and heavy drinkers (DAI > 60 g/die; M = 3.897 μM; CI 2.404–5.391 μM) were calculated. The present analysis demonstrates a good clinical efficiency of PEth for detecting chronic heavy drinking.

Abstract: 9 patients (6 males; age 0.9-18 years, median 9.9) were identified by searching the AIEOP (Italian Association of Pediatric Hematology and Oncology) registry and the national databases of rare tumors, soft tissue sarcomas (STS) and neuroblastomas. The data on the cases included in STS treatment protocols were collected prospectively and histology was centrally reviewed; the data and histology concerning the other children were reviewed for the purpose of this analysis.All tumors occurred in the sinonasal region with bone erosion (7 patients) and intracranial (4) or intraorbital (4) extension. Three patients were in Kadish stage B, and 6 in stage C. Complete tumor resection was very difficult to achieve, but adding chemotherapy and radiotherapy enabled tumor control in 8 patients. Response to chemotherapy was evident in 5/7 evaluable cases. Radiotherapy (48.5-60 Gy) was delivered in all children but one, due to early disease progression. With a median follow-up of 13.4 years (range 9.2-22.9), 7 patients are alive in 1st and one in 2nd complete remission. All surviving patients developed treatment-related sequelae, the most frequent being endocrine dysfunctions (4 patients) and craniofacial growth impairments (4 patients).Our findings confirm that ENB in children has an aggressive presentation, but multimodal therapy can cure most patients. Abstract: among the third generation of human rights, the appeal for a diachronic solidarity between the human community and the extra-human natural world, and for a synchronstic one between the community of living human beings and hypothetical members of future human generations, has a significant place. this paper analyzes from a critical perspective the reasons for and against this proposal of duties and intergenerational ethical obligations in the context of a techno-scientific civilization, with the aim of grounding the current generation's responsibility regarding the elaboration of a global ethics that could face the demands of future generations.

Abstract: Dentro del conjunto de los derechos humanos de tercera generación se destaca la apelación a una solidaridad de tipo diacrónica entre la comunidad de seres humanos y el mundo natural extra-humano, y otra de tipo sincrónica entre la comunidad de los seres humanos efectivamente vivientes y otros hombres, integrantes hipotéticos de generaciones humanas aún no nacidas. Este artículo analiza críticamente los argumentos esgrimidos a favor y en contra de esa postulación de deberes / obligaciones éticas intergeneracionales en un contexto civilizatorio tecnocientífico, para fundamentar por último la responsabilidad que le cabe a la generación actual en la elaboración de una ética global que atienda seriamente a las demandas del futuro.
