Comparing ‘remifentanil-propofol’ and ‘fentanyl-propofol’ in patients undergoing craniotomy for supratentorial space-occupying lesions

Abstract: Background: Control of intracranial pressure (ICP) before, during and after neurosurgical operations is crucially important. Therefore, trying different methods and drug combinations to attain this goal is an ongoing effort in anesthesiology. In this study we compared two combinations of a narcotic agent with propofol in neurosurgical operations to control intracranial pressure.Methods: In this prospective randomized double-blind clinical trial, we enrolled 34 patients with supratentorial brain tumors who were candidates for craniotomy in Alzahra Hospital in Isfahan, Iran from April 2008 to April 2009. The patients were randomly divided into two groups of 17, in whom the first and the second group, respectively, received a combination of "propofol and fentanyl" and a combination of "propofol and remifentanyl" as maintenance of anesthesia. The hemodynamic status, ICP during the surgery, and post-surgical complications in recovery unit were observed for and registered in a questionnaire.Results: Hemodynamic status was similar in both groups and they did not differ in recovery complications except for pain which was more prevalent in remifentanil group (P<0.03). Although the patients in fentanyl group better responded to the drug for lowering ICP than remifentanyl group, but the difference was not statistically significant.Conclusion: There is no difference between these two anesthetic agent combinations and both could be useful in the anesthesia of neurosurgical operations. However combination of propofol and fentanyl seem to be superior because of more pain relief and a smoother recovery period.
Assessing the existing context and proposed plan of Yazd historical texture from the point of access to emergency services using network analysis

Abstract: Extended 1- Introduction In addition to general diseases, texture and material oldness, complexity and density of the building, have exposed the inhabitants of the historical texture of cities like Yazd to dangers such as earthquake and fire. Thus, in this type of texture, the supplementary and health-medical land uses like emergency has special importance regarding the roles they play. Accessing the medical services especially the emergency requires special attention to the connection network structure. Thus, in the urban plans especially for the historical and time-worn textures, this requires more attention to the connection network structure and also the pattern of locating and quick and in time and comfortable access to emergency centers related to that and according to the standards and criteria, in order to act quickly and without facing any obstacles and limitations of the urban environment in the case of requirement.This study tries to analyze the existing context and the proposed plan of Yazd historical texture from the point of access to emergency services relating to its connection network structure.2- Theoretical basesThe main view point in the research methodology of this paper is analytic descriptive and the applied kind which is performed with the systematic trend and based on the theoretical information and geographical data. The tool which is used for such analysis is GIS tool and Network Analyst tool. First a network of all the connection paths of the historical texture based on the real directions of traffic is prepared and after creating topology, the other network supplemental information such as street type, length and width of the street, one-way, tow-way and cul-de-sac, traffic volume, average speed, and the crossroads and the existing limitations was added and the special relation between the network lines was created and the network became intelligent and according to this, the time factor, i.e. the duration of each paths was obtained and the accessibility to the emergency centers was analyzed base on that.3- DiscussionIn the current situation, 40.8 percent of the parcels of Yazd historical texture have access to less than 4 meters of the street, which has reduced to 24 percent by the proposed plan. Also, in the existing context, more than 70 percent of the historical textures are farther (3 minutes) from the global standards of accessing the emergency service centers and about 50 percent of the texture reach these services in a later time (8 minutes) compared with the common time in Iran. Abstract: A 2- year old girl is presented with dehydration, anemia, marasums, bilateral cataracts, short arms and short second fingers as well as syndactyly of the fourth and 5th fingers. Radiography showed stippled epiphysial cartilages. The patient died after seizures that were resistant to therapy. Conradi's syndrome, characterized by stippled epiphysial cartilages in the first years of life, may be associated with many other anomalies including dwarfness, asymmetrical shortness of the extremities, congenital hip dislocation, scoliosis, kyphosis and ectodermal lesions consisting of alopecia, cataracts, ichthyosis and hyperkeratosis. 2 genetic forms, the severe autosomal recessive and the more being autosomal dominant, and a non-genetic form of the disease are reported. The treatment includes orthopedic procedures and physiotherapy.
Drug Therapy in Diarrhea

Abstract: The drug used in diarrhea must be effective in that they reduce the secretion and increase the absorption of the intestinal mucosa. This seems to be only possible with morphine derivatives. But these are not recommended as they may cause ileus. Antibiotics are indicated in only few cases of severe intestinal infections. Other frequently used drugs such as adsorbents are practically of no effect. Thus, rehydration, electrolyte substitution and realimentation remain the most effective method of treatment of acute diarrhea in infants.

Abstract: A 2 -year-old girl with acrodermatitis Enteropathica is presented. She suffered of diarrhea, skin lesions and total alopecia since she was 2 months old. She was admitted to the hospital in poor general condition and severely malnourished. The zinc level in blood serum was remarkably low. The therapy was started with daily 15 mg zinc sulfate. The recovery was complete. The literature on the disease is reviewed and some aspects of the entity are discussed.
A Novel Effective Secure and Robust CDMA Digital Image Watermarking in YUV Color Space Using DWT2

Abstract: This paper is allocated to CDMA digital images watermarking for ownership verification and image authentication applications, which for more security, watermark W is converted to a sequence and then a random binary sequence R of size n is adopted to encrypt the watermark; where n is the size of the watermark. This adopting process uses a pseudo-random number generator to determine the pixel to be used on a given key. After converting the host image to YUV color space and then wavelet decomposition of Y channel, this adopted watermark is embedded into the selected subbands coefficients of Y channel using the correlation properties of additive pseudo- random noise patterns. The experimental results show that the proposed approach provides extra imperceptibility, security and robustness against JPEG compression and different noises attacks compared to the similar proposed methods. Moreover, the proposed approach has no need of the original image to extract watermarks.
Examining the Relevance of Emotional Intelligence and Organizational Commitment among Employees of Small and Medium Enterprise in Private Sector

Abstract: The relevance of emotional intelligence and organizational commitment is tested among 142 employees of small and medium enterprise (SME) in private sector in Iran. The results indicate that there is a significant relationship between emotional intelligence as an overall construct and organizational commitment. Strong and positive influence of self management and social awareness as two competencies of emotional intelligence on employees’ obligation to the organization is found. Meanwhile, positive but not significant impact of self awareness and relationship management as two abilities of emotional intelligence on employees’ organizational commitment is educated. Therefore, due to lack of financial resources and support for private sector organizations in Iran, the results have a great implication for SMEs in private sector to prevent from wasting their financial resources by hiring individuals who are more emotionally intelligence that would result in higher level of organizational commitment.

A Comparison between Digital Image Watermarking in Tow Different Color Spaces Using DWT2

Abstract: A novel digital watermarking for ownership verification and image authentication applications using discrete wavelet transform (DWT) is proposed in this paper. Most previous proposed watermarking algorithms embed sequences of random numbers as watermarks. Here binary images are taken as watermark for embedding. In the proposed approach, the host image is converted into the YCbCr color space and then its Y channel decomposed into wavelet coefficients. The selected approximation coefficients are quantized and then their four least significant bits of the quantized coefficients are replaced by the watermark using LSB insertion technique. At last, the watermarked image is synthesized from the changed and unchanged DWT coefficients. The experiments show that the proposed approach provides extra imperceptibility and robustness against wavelet compression compared to the traditional embedding methods in RGB color space. Moreover, the proposed approach has no need of the original image to extract watermarks.
