-hCG en fluidos vaginales como marcador de rotura prematura de membranas

Abstract: Objetivo: Determinar la utilidad de la -hormona gonadotropina coriónica en fluidos vaginales para el diagnóstico de rotura prematura de membranas. Método: Estudio prospectivo y comparativo en 120 gestantes sanas divididas en 2 grupos, con rotura prematura de membranas y sin ella. Se determinaron las concentraciones de esta hormona en fluidos vaginales. Ambiente: Sala de Partos. Maternidad "Concepción Palacios", Caracas. Resultados: Las concentraciones de -hormona gonadotropina coriónica en fluidos vaginales superiores a 17,10 mUI/mL se relacionaron con rotura prematura de membranas, sensibilidad de 98,33 %, especificidad de 93,33 %, valor de predicción positivo 93,65 % y valor de predicción negativo 98,54 %. Conclusiones: Las concentraciones elevadas de hormona -hormona gonadotropina coriónica en fluidos vaginales constituyen un método confiable para el diagnóstico de rotura prematura de membranas. Abstract: we report the case of a 14 years and 4 months old patient, referred to our out clinic consultation foramenorrhea, short stature and turner stigmas at physical examination. peripheral blood and gonadal karyotype, as well as endocrinological profile and histopathological assessment of gonads were fundamental for establishing the differential diagnosis of mixed gonadal dysgenesis. hormonal replacement therapy with equine conjugated estrogens was indicated with a good clinical evolution.

Abstract: Se presenta el caso de una paciente de 14 a os y 8 meses, referida a la consulta por presentar amenorrea, talla baja y estigmas turnerianos al examen físico. El cariotipo realizado en sangre periférica y gónadas, así como el perfil endocrinológico y el resultado histopatológico de las gónadas fueron fundamentales para establecer el diagnóstico diferencial con disgenesia gonadal mixta. Se indicó terapia de reemplazo hormonal con estrógenos equinos conjugados presentando buena evolución clínica. Abstract: the case of a 22 year old patient followed in the high risk pregnancy clinic due to diagnosis of pheocromocytoma, with adecuate control of blood pressure, was reported. at 35 weeks of gestation, a cesarean section with adrenalectomy was indicated; however, during surgery she presented high blood pressure which did not respond to the pharmacological therapy, so adrenalectomy was postponed. after surgery, she presented uterine atony, hence subtotal hysterectomy and right adrenalectomy was performed. the histology confirmed the diagnostic of pheocromocytoma. the multidisciplinary management allowed in this case a satisfactory maternal-fetal outcome

Abstract: Se comunica el caso de gestante de 22 a os, controlada en la consulta de alto riesgo por diagnóstico de feocromocitoma, con control adecuado de las cifras de tensión arterial. Se realizó cesárea segmentaria más adrenalectomía a las 35 semanas; durante el acto operatorio presentó crisis hipertensiva de difícil manejo, por lo cual, se difirió la adrenalectomía. En el posoperatorio inmediato presentó atonía uterina que ameritó realizar histerectomía subtotal e inmediatamente adrenalectomía derecha. El estudio histopatológico confirmó el diagnóstico de feocromocitoma. Abstract: this article considers that the flow of new not patented innovations is difficult to occur among firms, especially when some inventions are caused by serendipity here i develop a model to analyze the conditions for a "market for inventions" to exist. with the help of game theory in particular, i show that the creation of such a market is quite unlikely because unfair buyer's be-haviourand the possibility of expropriation of the new innovation renders the seller reluctantto negotiate with a potential buyer. so i argue for a solution that involves a third referenced party into the negotiation so that it can guarantee the flow of ideas among institutions and genérate a surplus for the whole society.

Creating a “Market for Inventions”: A Referee Model
