Enhancing longevity of pacemakers through reprogramming. Underutilization and cost-effectiveness

Abstract: OBJECTIVE: This study was performed to observe the number of pacemakers that had never been reprogrammed after implantation, and the effect of optimised output programming on estimated longevity of pulse generators in patients with pacemaker METHODS: Sixty patients with Teletronics Reflex pacemakers were evaluated in a pacemaker clinic, from the time of the beginning of its activities, in June 1998, until March 1999. Telemetry was performed during the first clinic visit, and we observed how many pulse generators retained nominal output settings of the manufactures indicating the absence of reprogramming until that date. After evaluation of the capture threshold, reprogramming of pacemakers was performed with a safety margin of 2 to 2.5:1, and we compared the estimated longevity based on battery current at the manufacturer's settings with that based on settings achieved after reprogramming. RESULTS: In 95% of the cases, the original programmed setting was never reprogrammed before the patients attended the pacemaker clinic. Reprogramming the pacemaker prolonged estimated pulse generator life by 19.7±15.6 months (35.5%). CONCLUSION: The majority of the pacemakers evaluated had never been reprogrammed. Estimated pulse generator longevity can be prolonged significantly, using this simple, safe, efficacious, and cost-effective procedure.

Abstract: objective: to translate, to make the cultural adaptation and to evaluate reproducibility and validity of the portuguese version of the aquarel (assessment of quality of life and related events) questionnaire, which is a specific tool to assess quality of life in pacemaker patients. methods: we evaluated 202 pacemaker patients: 63 patients during the cross-cultural adaptation stage and 139 during the reproducibility and validity evaluation stages. the questionnaire translation was reviewed repeatedly until > 85% of patients correctly understood the questions. reproducibility of the final version was tested in 69 patients in whom the interview was performed twice by the same researcher. validity was checked by the correlation between scores obtained in aquarel domains and those obtained in sf36 domains, in the functional class and the distance walked in the six-minute test. results: the internal consistency of aquarel was adequate, with cronbach?s alpha coefficient varying between 0.59 and 0.85. reproducibility was good, with high correlation coefficients (0.68-0.89) and random distribution of data in bland and altman plots, without systematic bias. a significant association was observed among aquarel domains and those obtained in sf36 domains and the functional class (p<0.01), although significant correlations with the distance walked in the six-minute test were not found. conclusion: the portuguese version of the aquarel questionnaire is easy and rapid to apply, and could be used as a specific questionnaire to assess quality of life in pacemaker patients.

Abstract: The objective of this study is to determine the auditory gain, quality of life, audiological benefits, in bone-anchored hearing device users (BAHA). It is a retrospective and concurrent evaluation of thirty patients fitted unilaterally and seven fitted bilaterally for at least six months. Patients were assessed with audiometric testing and application of Glasgow Benefit Inventory (GBI) and Abbreviated Profile of Hearing Aid Benefit (APHAB). Regarding sound-field pure audiometry results, we found a statistically significant gain in all frequencies using the bone-anchored device. APHAB scores showed statistically significant subjective audiological gains in all subscales except for the aversiveness subscale. GBI mean scores for all items in both groups were all above 3, suggesting quality of life improvement in conductive and mixed hearing loss patients. BP100 users showed a greater clinical gain in the APHAB global score and subscales compared with Divino users. In conclusion the BAHA provides significant auditory gain, subjective audiological benefits and improves quality of life in all BAHA users. This study shows a significant clinical and statistical benefit of BAHA measured by audiometric testing and by the APHAB and GBI questionnaires.

Abstract: Se produjeron descargas eléctricas en un sistema de arco pulsado a baja presión, con una corriente máxima de 100 A con pulsos de 30 ms. Las descargas se llevaron a cabo aplicando un voltaje de 104 V entre los electrodos. Los materiales utilizados como cátodo fueron Ti, Zr, Ni, Cu, Mo y W. La morfología de cátodos después de la descarga fue estudiada mediante la técnica de microscopía electrónica de barrido (SEM). Los cátodos de Ti y Zr presentaron la mayor erosión. Por otra parte, se observaron cráteres circulares sobre cátodos de Ni y Mo y se analizó una región del cátodo Zr, con alta erosión y gran cantidad de cráteres. Se midió el voltaje de descarga para cada material, obteniendo valores de voltaje del arco. Finalmente, se observaron las relaciones entre los voltajes de arco y algunas características del material como punto de fusión y de ebullición, presentando una tendencia exponencial.

Abstract: objective: hereby, we analyze the characteristics of the clinical gleason 8-10 group of patients with in our series diagnosed of prostate cancer and treated by means of radical prostatectomy, and we try to ascertain which are the influence factors within this group upon progression and progression free survival. material and methods: from the global series of 781 patients with t1-t2 prostate cancer treated by means of radical prostatectomy between 1990 and 2004, we study 108 with a gleason score on the biopsy of 8-10. median psa was 12 ng/ml and 50% were t2. variables related to biochemical progression and progression free survival have been studied, comparing the group of gleason 8-10 with the rest and analyzing, within the gleason 8-10 group which are the related variables with progression and progression free survival, trying to find a predictive model. contingency tables and logistic regression have been employed. for the survival analysis, kaplan meyer curves, log-rank and cox models. results: actual state: 62,7% (490/781) are alive and free of biochemical progression, 24,8% (194/781) are alive with biochemical progression, 2,9% (23/781) are dead by cancer and 1,9% (15/781) are dead by other cause and 7,6% (59/781) are lost. biochemical progression study of the whole series (781 patients) clinical gleason score 8-10 is a influence factor on the univariate study (or:2,61 ic 95%: 1,7-4). in the progression free survival study (pfs) of the whole series (781 patients) the pfs in clinical gleason 8-10 at 3 and 5 years is 56 ± 5% y 35±7%, significantly worse than the rest of the group (p<0,0001). in the multivariate study of the influence factors on the pfs includes clinical gleason score 8-10 as an independent prognostic factor (or:2,6 ic 95%: 1,6-4,12) p=0,003, together with the clinical stage (or:1,81 ic95%:1,18-2,78) p<0,006, the psa (or: 1,03 ic 95%:1,025-1,046) p<0,0001 and the side of tumor on the biopsy (or:1,5 ic 95%:1,01-2,24) p=0,045. in the clinical gleas

Abstract: the study consists on the evaluation of the university campus drainage system by the application of a model for maxima hydrological events, that determined the design flows for return periods of 25 and 50 years. once defined the runoff, the existent natural and artificial drainages were evaluated, turning out to be inefficient for the transport of these flows; therefore, hydraulic structures were designed for new sections corresponding to the hydraulic design. the structural calculations for the system of collectors, bridges, sewers and channels were carried out with the purpose of determining their dimensions, using a maintenance approach, in order to drain efficiently the study area.

Abstract: El estudio consiste en la evaluación del sistema de drenaje de la Ciudad Universitaria mediante la aplicación de un modelo para eventos hidrológicos máximos, con el cual, se determinaron los caudales de dise o para períodos de retorno de 25 y 50 a os. Una vez definidos los escurrimientos se evaluaron los drenajes naturales y artificiales existentes, resultando ser ineficientes para el transporte de dichos caudales, por tanto, se dise aron las estructuras hidráulicas acordes a las nuevas secciones correspondientes al dise o hidráulico, se realizaron los cálculos estructurales del sistema de colectores, pasos de vía, alcantarillas y canales con la finalidad de determinar sus dimensiones, utilizando un criterio de mantenimiento, de manera que drenen eficientemente el área en estudio. Abstract: the methodological model described here is based on developments in sociolinguistic research which have resulted from a constant concern for the design and implementation of techniques that reflect the language in its social context, departing from the dialectological tradition to the analysis of language. the present monograph is a description of the methodology used in the study of the main phonological, morph-syntactic, lexical and discourse features of the spanish of bucaramanga ( colombia ). the methodological approach presented here is based on the experience of previous studies in the collection, selection, and analysis of data and methodological improvement from these experiences to better converge on the linguistic system that is to be described: the vernacular, the language in use and not an idealized system. it is a model that may be applicable to the study of other varieties of spanish in colombia , as an alternative accepting the view that sociolinguistics has not yet arrived at the ideal method in the study of sociolinguistic phenomena in urban contexts.
Honor e identidad en Maracaibo a fines del siglo XIX*

Abstract: we present in this paper the first results of on-going research aimed at studying the cultural representations that shaped the concept and practices related to feminine honor in the city of maracaibo an the end of the xixth century, considering the same as a privileged means to achieve a deeper understanding of this society. from the theoretical and methodological points of view, honor is approached as a notion related to both collective identities and personal identity, through the analysis of a series of lawsuits dealing with promises of marriage kept at the civil registrars office in zulia state. it was found that honor was an important element in the construction of personal identity and in social differentiation, incarnating a sense of virtue and a sense of social reputation. we conclude that the maracaibo residents involved in the cases studies seemed to share the symbolic codes expressed in the official discourse of cultural unity and national identity, addressed at ?regenerating? the venezuelan society and at protecting the family as the basis of society, making the honor-virtue of women, particularly their sexual honor, the moral foundation of the family.
